RESUMO -Análise de nove casos de cisto aracnóideo intracraniano operados no período de 1974 a 1995. Os procedimentos cirúrgicos consistiram de derivação cistoperitoneal em sete pacientes e craniotomia com membranectomia nos outros dois casos. Complicações pós-cirúrgicas, do tipo infecção bacteriana e falha no sistema de drenagem, ocorreram em três casos, as quais foram debeladas e corrigidas, respectivamente. Baseados em revisão de literatura, alguns aspectos fisiopatológicos, diagnósticos e terapêuticos são discutidos.PALAVRAS-CHAVE: cisto aracnóideo, derivação cistoperitoneal, craniotomia, membranectomia, mecanismo valvular, tomografia computadorizada, ressonância magnética, neuroendoscopia. Intracranial arachnoid cysts: report of nine operated casesABSTRACT -Analysis of nine patients with intracranial arachnoid cysts surgically treated from 1974 to 1995. The treatment consisted of cystoperitoneal shunt in seven patients and craniotomy with excision of cyst membrane in the other two cases. After surgery, bacterial meningitis and failure shunt device were observed in three cases. Both the infectious process and the shunt device were respectively treated and corrected at once. It has been observed total clinical improvement in all nine cases. A review of the literature has been made and some pathophysiological, diagnostic and therapeutic aspects are discussed.
RESUMO -A impressão basilar ocupa lugar de destaque entre as malformações occipitocervicais. A forma primária decorre de erro embriogênico, enquanto a forma secundária é causada por afecções ósseas diversas, como doença de Paget, displasia fibrosa, osteoporose e osteomalacia. A descrição original da impressão basilar foi feita por Ackermann 1 , em 1790, em estudos sobre o cretinismo. Virchow 32 , em 1857, denominou-a de platibasia e rejeitou a relação entre impressão basilar e cretinismo, como fora admitida por Ackermann 1 . Virchow 33 , em 1876, denominou a malformação de impressão basilar, tendo descrito detalhadamente as modificações ósseas e sendo o primeiro autor a admitir a origem congênita em muitos de seus casos. Homén 23 , em 1901, foi o primeiro autor a correlacionar os dados clínicos com os achados anátomo-patológicos. A necropsia de seu paciente evidenciou achatamento do bulbo e cerebelo, bem como presença de cavidade cística na medula espinhal. Explicou a patogenia dos sintomas clínicos através da compressão das estruturas comprometidas na invaginação da apófise odontóide. Quanto à clínica, a impressão basilar sintomática pode apresentar síndromes clínicas diversas, isoladas ou combinadas, tais como síndrome dos nervos intracranianos, síndromes sensitivas superficial e profunda, cerebelar, de hipertensão intracraniana, bulbar, medular, de insuficiência vertebrobasi 1 ar 2 , 6 , 2 4 ' 2 5 .
Prevalence rates of multiple sclerosis (MS) suggest an interrelationship between genetic and environmental factors, ranging worldwide.Objectives Clinical and epidemiological characterization of MS patients in João Pessoa, Paraíba city.Methods Study involving patients treated in five services in the city.Results It included 87 patients with MS, representing a prevalence of 12.0 cases/100,000 population, mainly women (77%) and white people (66.7%) with mean age of 43 years and average age of the first outbreak of 32.2 years. Motor symptoms (65.5%) and relapsing-remitting clinical form (78.2%) predominated; the average of the Expanded Disability Status Scale (EDSS) scores was 3.5 and 72% used a type of immunomodulatory drug. There was a positive correlation between the number of outbreaks and the duration of the disease with EDSS scores.Conclusions The prevalence of the disease is considered average. The clinical and epidemiological characteristics are in line with most similar Brazilian studies.
RESUMO -Homem de 25 anos de idade foi internado com sintomatologia polimorfa típica das afecções dos gânglios da base, associada a manifestações psiquiátricas. Fez uso de periciazina; no entanto, a suspensão do medicamento não melhorou a sintomatologia. Foi estabelecido o diagnóstico de doença de Wilson pela visualização do anel de Kayser-Fleischer através de exame com lâmpada de fenda e pelos exames laboratoriais que mostraram diminuição da ceruloplasmina plasmática e aumento de excreção de cobre urinário. A ressonância magnética, ponderada em T2, em cortes axiais do mesencéfalo e ponte, evidenciou imagens das "faces do panda". Case report ABSTRACT -A 25 year-old man was admitted with polimorph symptomatology resembling basal ganglia disease associated with psychiatric manifestations. The patient had been treated with pericyazine. The drug was stopped but the symptomatology did not improve. The diagnosis of Wilson's disease was established through ophthalmologic examination with slit-lamp that revealed the Kayser-Fleischer ring and laboratory abnomalities showing a low serum ceruloplasmin level and increased urinary copper excretion. T2-weighted axial magnetic resonance imaging demonstrated the " face of panda signs" in the midbrain and pons.KEY WORDS: Wilson's disease, magnetic resonance imaging, "faces of panda" signs.A doença de Wilson é conhecida desde 1883, quando Westphal a denominou de pseudoesclerose ao observar a presença de tremores e rigidez em dois pacientes. Strümpell, em 1898, evidenciou d o e n ç a de fígado em autópsia de dois pacientes portadores de tremores. Em 1912, Kinnier Wilson descreveu a forma clássica da doença, correlacionando a sintomatologia dos gânglios da base à doença do fígado, denominando-a, então, degeneração lenticular p r ogressiva. Hall, em 1921, a descreveu como degeneração hepatolenticular 1,2 . A degeneração hepatolenticualar origina-se de distúrbio genético, de t r a n smissão autossômica recessiva, que provoca alterações no metabolismo do cobre. Estas alterações c o mprometem a síntese de ceruloplasmina e provocam deposição de cobre em vários locais do organismo, principalmente, no fígado, cérebro, córnea (membrana de Descemet) e rins. É uma doença rara, com início entre os 11 e 25 anos de idade. A sintomatologia é caracterizada por manifestações neurológicas, hepáticas, psiquiátricas e oculares 3 -8 . O anel de K a y s e r-Fleischer e as alterações do nível de ceruloplasmina no sangue e da excreção urinária de c o b r e representam os dados de maior importância para o diagnóstico da doença. Todavia, nas últimas d é c adas, achados radiológicos, especialmente as alterações do tronco cerebral reveladas pela ressonância magnética (RM) e conhecidas como sinais das "faces do panda" têm demonstrado significativo valor no estudo da degeneração hepatolenticular [9][10][11][12][13][14][15] .
-We studied 97 inpatients with myelopathy at the University Hospital Lauro Wanderley from 1985 to 2002. Ages ranged from 10 to 78 years and average age was 39.8 years. All patients were examined by neurologist. Cerebrospinal fluid (CSF) was analysed in 44 (45.3%) cases and immunofluorescence reactions and/or ELISA in CSF were made in 32 (32.9%) cases. Radiologic investigation of the vertebral spine and spinal cord of the 32 (32.9%) cases consisted of 8 radiographies, 10 computed tomography and 14 magnetic resonance image. In 41 (42.2%) cases the lesions were situated in the lower dorsal and lumbar level and complete transverse lesion were registred in 61 (62.8%) cases. There was predominance of transverse myelitis (43.2%) and spinal cord compression (15.46%). Spinal cord schistosomiasis was the form more often of the transverse myelitis (13.4%).
