The present study aims at elaborating and applying an education plan for nurses in an Unit for Burnt Patients, with the proposal of analysing and reformulating the practice of the nursing process in this service. The framework proposed by Charles MAGUEREZ, was employed as a guideline, which led to the selection and application of the teaching and learning strategies implemented in this plan. We concluded that the framework proposed by MANGUEREZ made possible a substantial improvement in the nursing process in this service, and that this new reality claims for further studies.
RESUMO -Uma análise do comportamento dos enfermeiros, enquanto desempenham fun ções administrativas numa organização hospitalar burocrática, é seguida por uma expo sição da visão da autora sobre o modo pelo qual este profissional pode situar-se e ocupar seu espaço na burocratização do seu trabalho.ABSTRACT -An analysis of nurses' behavior while carrying out administrative functions in a bureaucratic hospital organization is followed by a presentation of the author's view of the manner in which these professionals my situate themselves and occupy their spa ce in the bureaucratization of their work. INTRODUÇÃOAo longo de nossa vivência profissional freqüen temente constatamos que as metas do serviço e do en sino de enfermagem são divergentes, e que a atuação idealizada para o enfermeiro não é correspondida na prática -o exercício preponderantemente administra tivo, por parte deste profissional, na unidade de inter nação não tem representado um veículo para a conse cução de metas ditadas pela profissão. No desempe nho da função administrativa o enfermeiro tem se li mitado a solucionar problemas de outros profissionais e a atender às expectativas da instituição hospitalar, relegando a plano secundário a concretização dos ob jetivos de seu próprio serviço.Assim exercida, esta função não é compreendida e nem aceita pelos enfermeiros de ensino e também por muitos enfermeiros de serviço, uma vez que há uma descaracterização de sua função específica como administrador.Por outro lado, em nosso contexto, não aceitamos também a assertiva de muitos enfermeiros de ensino, segundo a qual a função primordial do enfermeiro de ve ser a assistência direta ao paciente .Após muita reflexão, decorrente de questionamen tos, embasamentos teóricos e observações práticas, as sumimos uma posição que visualiza a administração da assistência ao paciente como função primordial do en fermeiro. E esta administração compreende a união dos fins da Administração aos fins da Enfermagem; é a su bordinação da Administração aos fins da Enfermagem.É a utilização da Administração como um instrumen to para o desenvolvimento da Enfermagem.No entanto, a prática nos demonstra que o enfer meiro ocupa um espaço subalterno e tem sido utiliza do como substituto em funções administrativas de ou tros órgãos ou disciplinas. Crescente número de autores reconhece a centralidade das interações verbais na prestação da assistência de enfermagem. Verificase na literatura especí-fica da nossa profissão uma diversificação no uso de instrumentos ou sistemas de análise de interações. Descreve-se neste trabalho o uso de um sistema de análise de interação no contexto da enfermagem hospitalar: O Interaction Process Analysis, de Bales. O objetivo do estudo foi investigar a existência de um padrão de interação entre o grupo da enfermagem e pacientes. Utilizou-se a técnica de observação direta do comportamento resultante da interação entre os componentes do grupo da enfermagem e pacientes recèm-admitidos em unidades de internação. Os sujeitos de interação foram observados durante dez períodos de quatro horas e seus episódios verbais foram registrados em formulários pelos observadores. As interações verbais oriundas do registro foram classificadas à luz do Sistema de Bales e agrupadas nas áreas positiva, neutra e negativa. Foi constatado preponderância de interações da área neutra (85,4%) sobre as demais: positiva (8,4%)''e negativa (6,1%), confirmando a existência de um padrão no comportamento de interação entre os sujeitos estudados e demonstrando que a interação entre o grupo da enfermagem e os pacientes se dá em torno de problemas funcionais. A interação ocorre na medida do desempenho do papel instrumental dos integrantes do grupo profissional estudado. O sistema de análise de Bales mostrou grande utilidade na detecção de padrões de interação do grupo da enfermagem com pacientes hospitalizados. UNTTERMOS:Relações enfermeiro-paciente. INTRODUÇÃOObserva-se, no campo da saúde, que a educação é um processo, pelo qual se partilham conhecimentos, atitudes, valores, crenças e práticas; busca-se motivar e estimular o indivíduo a melhorar a sua saúde pelo próprio esforço, levando-se em conta o meio ambiente em que vive, bem como a sua individualidade 7,15,21 . ALVIM 1 comenta que "o trabalho educativo do paciente e da família é um processo contínuo que só poderá ser bem sucedido se houver uma ação conjunta entre os serviços comumente chamados de saúde pública e os serviços hospitalares", uma vez que a continuidade do cuidado de enfermagem depende de bom relacionamento, comunicação e cooperação dentro da própria equipe de saúde.Os altos coeficientes de mortalidade neonatal devidos a causas perinatais sugerem, claramente, uma situação bastante desfavorável no tocante à assistência pré-natal, ao parto e ao recém-nascido. De fato, PUFFER e SERRANO 18 mostraram cabalmente que a maioria das causas perinatais responsáveis pela alta mortalidade neonatal são evitáveis se houver adequada assistência pré-natal. Como ressaltam AND RI ASO LA et alii 3 , esta mortalidade, passível de prevenção, é proveniente da deficiente atenção â saúde materna, tanto na assistência ambulatorial como na hospitalar. Within the hospital context, the nurse cannot avoid the administrative function, even though this is a controversial issue in Brazilian nursing. To focus on the problem, in the present study we set out to identify the set of functions of nurses working on the admission floors and to determine the distribution of administrative functions according to their bureaucratic and non-bureaucratic characteristics. The data were collected at four admission units by intermittent observation of work performed by nurses in 1985. The following set of functions were identified: administration (62.5%), patient assistance (19.2%), teaching and research (3.8%), functions that could be delegated to others (6.8%) and personal (7.6%). The administrative functions were classified into bureaucratic and non-bureaucratic with a respective distribution of 77% and 23%. The study indicates that the administrative function is one of the elements defining the nurse's profile, even though the authors believe that this function should be better oriented.
