Nas Diretrizes Curriculares nacionais, a saúde é considerada uma área interdisciplinar, pois seu objeto "o processo saúde-doença humano " envolve as relações sociais, a biologia e as expressões emocionais. Este estudo analisa os programas e atividades do currículo implantado na Faculdade de Medicina da PUC-Campinas a partir de 2001, no tocante às ações interdisciplinares e multiprofissionais que envolvem os demais cursos da saúde. Procedeu-se à análise documental e de depoimentos de diferentes atores envolvidos no curso. Na perspectiva dos entrevistados, apesar das dificuldades relativas à fragmentação dos saberes e práticas, ao desconhecimento e preconceito acerca dos campos e núcleos das profissões da saúde e à precarização do vínculo docente, tem havido iniciativas de integração, destacando-se os ciclos morfofisiológicos, de correlação clínica e as práticas de Saúde da Família e Comunidade. Concluiu-se que a interdisciplinaridade está presente na proposta curricular e como intenção da Universidade, mas acontece por iniciativas individuais, nas quais se viabilizam "encontros" entre discentes, docentes, funcionários e usuários, que se tornam significativos e demonstram um processo em construção, ainda distante da transdisciplinaridade.
We describe a rare hepatic collision tumor composed of a hepatocellular carcinoma and a high-grade neuroendocrine carcinoma. The patient, a 50-year-old man, underwent a partial hepatectomy because of a 5.0-cm mass. The tumor had two distinctive patterns. The majority of the tumor was a high-grade neuroendocrine carcinoma with features of a small cell carcinoma that was positive for chromogranin, synaptophysin, and cytokeratin 19 and negative for hepatocellular antigen and alpha-fetoprotein (AFP). The second component was a moderately differentiated hepatocellular carcinoma that was positive for hepatocellular antigen and AFP and negative for neuroendocrine markers. The two tumors were separated by fibrous bands. In areas where they collided, there was no transition or intermingling of cells between the two components, thus, it is different from the combined type of tumors. After removal of the tumor, the patient had intrahepatic and mesenteric recurrences within a follow-up period of 16 months.
Este artigo aborda a questão do processo ensino-aprendizagem em serviços de saúde, em especial os integrados à rede pública. Repensando algumas teorias pedagógicas, discute aspectos do ensino experimentado no trabalho cotidiano. O trabalho realizado nestes serviços possibilita articular a educação a suas dimensões sociais, ideológicas e técnicas e definir não somente pelas necessidades dos profissionais, mas como resposta à realidade e às finalidades que orientam a institucionalização do sistema de saúde brasileiro. A dialética entre a prática educacional e o cuidado em saúde tem gerado correntes de pensamento e de ação inovadoras, podendo-se dizer que a educação em serviços ressalta o potencial pedagógico do trabalho cotidiano, orienta a aprendizagem enquanto um processo contínuo e de natureza participativa e possibilita a articulação entre o fazer, o educar, o saber. Por integrar o individual, o grupal, o institucional e o social, o cognitivo e o afetivo, esse processo pedagógico pode representar um espaço comprometido com a transformação das práticas, necessária à efetivação de uma Reforma Sanitária em defesa da vida.
Acute vascular rejection (AVR) in human small-bowel transplantation is an inadequately characterized entity whose frequency and severity is not well understood. As compared to severe AVR, changes identifying early, mild or evolving AVR are not known. We created a scoring system to evaluate subtle mucosal vascular changes and examined 188 biopsies from 21 patients obtained in the first 3 months post transplant. A majority of patients had a transient rise in vascular injury, often within 30 days of transplant. Small-vessel congestion and erythrocyte extravasation were the most common alterations. The vascular injury score was not related to acute cellular rejection, HLA type or HLA antigen disparities. However, the patients with the vascular changes had significantly higher peak panel reactive antibodies (PRA) and a higher incidence of positive T-cell and B-cell crossmatch. Finally, graft survival was significantly lower in the patients demonstrating the early vascular lesions. These data suggest that the vascular injury is partially associated with humoral presensitization of the recipient and may be a form of acute vascular rejection. Since these vascular changes are frequent, we advocate early post-transplant monitoring to identify and manage potentially high-risk patients.
Closure of large abdominal defects after extensive abdominal surgery is a major technical surgical problem. Failure to close the abdomen leaves the patient at risk for grave complications. Full-thickness abdominal wall skin transplantation appears to solve this problem. This is the first time that detailed histopathologic features of skin abdominal wall transplantation from cadaver donors are described. Five adults and four children underwent 10 transplants because of large abdominal wall defects. Twenty-two posttransplantation skin specimens were evaluated during a mean follow-up of 23.5 weeks, and the findings were compared with the clinical appearance of the skin. Rejection was manifested as a maculopapular rash. The histologic features were categorized as perivascular infiltrates, epidermal changes, and stromal changes. A grading system is proposed based on the number of cases encountered: No rejection, grade 0 (n = 9): No perivascular infiltrates. Indeterminate for rejection, grade 1 (n = 2): Up to 10% of vessels show infiltrates of small lymphocytes. No eosinophils, large lymphocytes, spongiosis, epidermal, or stromal inflammation are seen. Mild rejection, grade 2 (n = 5): 11% to 50% of vessels are infiltrated by small lymphocytes. Eosinophils and mild spongiosis may or may not be present. No epidermal infiltrates, stromal infiltrates, or large lymphocytes are seen. Moderate rejection, grade 3 (n = 4): Greater than 50% of vessels show lymphocytic infiltrates that may be accompanied by epidermal and stromal inflammation. Spongiosis is absent or mild. Endothelial plumping, eosinophils, and large lymphocytes may be seen. Severe rejection, grade 4 (n = 2): Greater than 50% of vessels show infiltrates, but different from moderate rejection, there is dyskeratosis and the epidermis shows heavier lymphocytic infiltrates and moderate to severe spongiosis. The stroma shows infiltrates extending into the base of the epidermis. Endothelial plumping, eosinophils, and large lymphocytes are present. The mean number of weeks after transplantation for the development of clearcut rejection (grades 2-4) was 8.36. Among the 9 nonrejection cases, 4 specimens from 3 patients had thrombosis of the vessels feeding the graft. A grading system serves to better assess skin allograft rejection.
-Phyllophaga cuyabana is a univoltine species and its development occurs completely underground. Its control by conventional methods, such as chemical and biological insecticides, is difficult, so it is important to understand its dispersion, reproduction, and population behavior in order to determine best pest management strategies. The objective of this work was to study the behavior of adults of P. cuyabana. This study was carried out in the laboratory, greenhouse and field sites in Paraná State, Brazil (24 o 25' S and 52 o 48' W), during four seasons. The results obtained demonstrate that: a) P. cuyabana adults have a synchronized short-flight period when mating and reproduction occurs; b) adults tend to aggregate in specific sites for mating; c) the majority of adults left the soil on alternate nights; d) the choice of mating and oviposition sites was made by females before copulation, since after copulation adults did not fly from or bury themselves at nearby locations; e) females that fed on leaves after mating, oviposited more eggs than females that had not fed; f) plant species such as sunflower (Helianthus annuus) and the Crotalaria juncea are important food sources for adults.Index terms: Glycine max, white grub, host plants, oviposition, pests of plants. Comportamento de vôo, de alimentação e de reprodução de adultos de Phyllophaga cuyabana (Moser) (Coleoptera: Melolonthidae)Resumo -Phyllophaga cuyabana é uma espécie univoltina cujo desenvolvimento ocorre no solo. Seu controle por inseticidas químicos e biológicos é difícil, assim é importante entender sua dispersão, reprodução e comportamento populacional a fim de identificar estratégias potenciais de manejo dessa praga. O objetivo deste trabalho foi estudar o comportamento de adultos de P. cuyabana. O estudo foi realizado em laboratório, casa de vegetação e campo, no Estado do Paraná, Brasil (24 o 25' S e 52 o 48' O), durante quatro safras. Os resultados obtidos demonstraram que: a) os adultos de P. cuyabana têm um vôo sincronizado durante um curto período quando ocorre o acasalamento e reprodução; b) os adultos tendem a se agregar em sítios específicos para o acasalamento; c) a maioria dos adultos deixa o solo em noites alternadas; d) a escolha do sítio de acasalamento e oviposição é feita pelas fêmeas antes da cópula; após a cópula os adultos não voam e se enterram em locais próximos; e) as fêmeas que ingerem folhas após o acasalamento ovipositam mais que as fêmeas que não se alimentam; f) espécies vegetais como girassol (Helianthus annuus) e Crotalaria juncea são importantes fontes de alimento para os adultos.Termos para indexação: Glycine max, coró, planta hospedeira, postura, praga das plantas.
