Desempenho de uma população brasileira no teste de alfabetização funcional para adultos na área de saúde ABSTRACT OBJECTIVE:To analyze the scoring obtained by an instrument, which evaluates the ability to read and understand items in the health care setting, according to education and age. METHODS:The short version of the Test of Functional Health Literacy in Adults was administered to 312 healthy participants of different ages and years of schooling. The study was conducted between 2006 and 2007, in the city of São Paulo, Southeastern Brazil. The test includes actual materials such as pill bottles and appointment slips and measures reading comprehension, assessing the ability to read and correctly pronounce a list of words and understand both prose passages and numerical information. Pearson partial correlations and a multiple regression model were used to verify the association between its scores and education and age. RESULTS:The mean age of the sample was 47.3 years (SD=16.8) and the mean education was 9.7 years (SD=5; range: 1 -17). A total of 32.4% of the sample showed literacy/numeracy defi cits, scoring in the inadequate and marginal functional health literacy ranges. Among the elderly (65 years or older) this rate increased to 51.6%. There was a positive correlation between schooling and scores (r=0.74; p<0.01) and a negative correlation between age and the scores (r=-0.259; p<0.01). The correlation between the scores and age was not signifi cant when the effects of education were held constant (rp=-0.031, p=0.584). A signifi cant association (B=3.877, Beta=0.733; p<0.001) was found between schooling and scores. Age was not a signifi cant predictor in this model p=0.584). CONCLUSIONS:The short version of the Test of Functional Health Literacy in Adults was a suitable tool to assess health literacy in the study population. The high number of individuals classifi ed as functional illiterates in this test highlights the importance of special assistance to help them properly understand directions for healthcare.
Introdução: O climatério, período de transição entre a fase reprodutiva e não-reprodutiva, ocasiona mudanças biopsicossociais nas mulheres que o vivenciam. A associação entre a maior prevalência de depressão nesse período é, no entanto, ainda controversa. O objetivo deste estudo foi avaliar a prevalência de depressão em mulheres climatéricas atendidas em um hospital universitário numa cidade da Região Nordeste do Brasil e identificar fatores associados. Método: Foi realizado um estudo prospectivo e analítico de 70 mulheres climatéricas. O diagnóstico de depressão foi dado segundo critérios diagnósticos da Classificação Internacional de Doenças em um período de seguimento mínimo de 3 meses. Foram investigadas as seguintes variáveis: escolaridade, situação conjugal, renda pessoal, gravidade da depressão segundo escala de Hamilton, presença e intensidade de sintomas climatéricos, menopausa (natural ou cirúrgica), dependência econômica do parceiro, antecedentes familiares de depressão, história prévia de depressão pós-parto, episódios depressivos e transtorno disfórico pré-menstrual, alterações da função sexual e visão positiva ou negativa da menopausa. Resultados: Um percentual de 34,3% das pacientes apresentou depressão, sendo 70,8% destas na categoria leve da escala de Hamilton. Não houve associação estatisticamente significante entre variáveis socioeconômicas, diminuição da libido e antecedentes familiares de depressão com a presença de depressão. Houve associação entre a presença de depressão e pacientes com sintomas vasomotores (p = 0,03), insônia (p < 0,001), menopausa (p = 0,05), com histórico de depressão pós-parto (p = 0,04) e transtorno disfórico pré-menstrual (p = 0,05) e visão negativa da menopausa (p = 0,001). Conclusões: Foi encontrada uma alta prevalência de depressão nas pacientes estudadas. Múltiplos fatores (impacto da menopausa, antecedentes psiquiátricos e visão pessoal sobre a menopausa) foram associados ao seu surgimento.
The differential diagnosis between frontotemporal lobar degeneration (FTLD) and Alzheimer’s disease (AD) is often challenging.ObjectivesTo verify the usefulness of behavioral and activities of daily living inventories in the differential diagnosis between FTLD and AD.MethodsCaregivers of 12 patients with FTLD (nine with frontotemporal dementia, two with semantic dementia and one with progressive non-fluent aphasia) and of 12 patients with probable AD were interviewed. The Brazilian version of the Frontal Behavioral Inventory (FBI) and Disability Assessment for Dementia (DAD ) were used.ResultsThe mean of the MMSE score was 12.4±10.7for patients with FTLD and 11.9±6.2for patients with AD (p=0.93). Mean scores on the DAD were 33.7±27.7in patients with FTLD and 55.6±29.7in patients with AD (p=0.06), while for the FBI they were 42.6±10.0for FTLD and 16.7±11.7for AD (p<0.01).ConclusionsIn this study, FBI was found to be a helpful tool for the differential diagnosis between FTLD and AD. Although the DAD was not useful in differential diagnosis in our sample we believe it to be important for measuring the severity of the disease through quantitative and qualitative assessment of functional deficits of the patients.
In everyday clinical practice, neurologists frequently encounter complex clinical cases in which a full analysis of several variants are essential to reach the right diagnosis. The resolution of a doubtful case may rely on the recognition of a very specific diagnostic clue. The objective of this report was to describe a case in which the prompt recognition of a very subtle imaging feature in the emergency room could have precluded further extensive diagnostic investigations. case rePorTA 34-year-old woman came into the Emergency department with a worsening headache. The symptoms had begun two weeks earlier, with a holocranial pulsatile headache that gradually worsened and became unbearable in the preceding three days. In association with the pain, she complained of nausea, vomiting and intolerance to movement, light and sound. As relevant medical history, she had dyslipidemia and was taking anphepramona for obesity. She had suffered occasional headaches in the past, but never of that intensity. She had already used several over the counter analgesics and nonsteroidal anti-inflammatory drugs with no relief.Neurologic examination disclosed left eye ptosis, which she claimed to be preexisting (Fig A), confirmed on an old photograph. Cerebrospinal fluid analysis was normal. A magnetic resonance angiogram (MRA) of the internal carotid artery (ICA) demonstrated no signal within left ICA (Fig B) absTracT At times in clinical neurology, the identification of a subtle clinical or radiological sign can lead to prompt diagnosis of a very rare or difficult case. We report on a patient who presented with untreatable headache and unilateral ptosis. Computed tomography (CT) scan of the head did not reveal any structural cause. Magnetic resonance angiogram showed absence of left internal carotid artery, which was eventually confirmed by a catheter angiography. Reviewing the case, it emerged that a feature on the initial CT scan "bone window" would have confirmed the diagnosis, had it been searched for: the underdeveloped carotid canal, which is a consequence and a marker of internal carotid artery agenesis.Key words: carotid artery agenesis, Horner syndrome, carotid canal underdevelopment, computed tomography. resumo Em algumas circunstâncias, o reconhecimento de um sinal clínico ou radiológico sutil pode tornar simples o diagnóstico de um caso raro ou muito difícil em neurologia clínica. Relatamos o caso de uma paciente que apresentava cefaleia intratável e ptose palpebral unilateral. A tomografia computadorizada (TC) de crânio não permitiu identificar nenhuma causa estrutural. A ressonância magnética evidenciou ausência da artéria carótida interna esquerda, posteriormente confirmada por arteriografia convencional.
