Over the past decade, there have been an increasing number of studies on the association between vitamin D deficiency and anthropometric state. However, we did not identify any meta-analyses of the relationship between obesity and vitamin D deficiency in different age groups. Thus, we evaluated the association between obesity and vitamin D deficiency. We searched for observational studies published up to April 2014 in PubMed/Medline, Web of Science and Scopus databases. We performed a meta-analysis in accordance with the random-effects model to obtain the summary measurement (prevalence ratio, PR). Among the 29,882 articles identified, 23 met the inclusion criteria. The prevalence of vitamin D deficiency was 35% higher in obese subjects compared to the eutrophic group (PR: 1.35; 95% CI: 1.21-1.50) and 24% higher than in the overweight group (PR: 1.24; 95% CI: 1.14-1.34). These results indicate that the prevalence of vitamin D deficiency was more elevated in obese subjects. The vitamin D deficiency was associated with obesity irrespective of age, latitude, cut-offs to define vitamin D deficiency and the Human Development Index of the study location.
There is still limited evidence regarding the influence of vitamin D in people with COVID-19. In this systematic review and meta-analysis, we analyze the association between vitamin D deficiency and COVID-19 severity, via an analysis of the prevalence of vitamin D deficiency and insufficiency in people with the disease. Five online databases-Embase, PubMed, Scopus, Web of Science, ScienceDirect and pre-print Medrevix were searched. The inclusion criteria were observational studies measuring serum vitamin D in adult and elderly subjects with COVID-19. The main outcome was the prevalence of vitamin D deficiency in severe cases of COVID-19. We carried out a meta-analysis with random effect measures. We identified 1542 articles and selected 27. Vitamin D deficiency was not associated with a higher chance of infection by COVID-19 (OR ¼ 1.35; 95% CI ¼ 0.80-1.88), but we identified that severe cases of COVID-19 present 64% (OR ¼ 1.64; 95% CI ¼ 1.30-2.09) more vitamin D deficiency compared with mild cases. A vitamin D concentration insufficiency increased hospitalization (OR ¼ 1.81, 95% CI ¼ 1.41-2.21) and mortality from COVID-19 (OR ¼ 1.82, 95% CI ¼ 1.06-2.58). We observed a positive association between vitamin D deficiency and the severity of the disease.

This article discusses the relationship between both poverty and food insecurity and the COVID-19 pandemic, as well as presenting possible strategies and actions for increasing social protection in the fight against these conditions in the current epidemiological context, especially for low-income countries. This is a narrative review concerning COVID-19, poverty, and food and nutritional insecurity. The COVID-19 pandemic may increase poverty and food insecurity levels, resulting from the absence of or weak political, economic, and social interventions to maintain jobs, as well as compromised food production and distribution chains and reduced access to healthy foods in different countries around the world, especially the poorest ones, where social and economic inequality was already historically high; the pandemic heightens and uncovers the vulnerability of poor populations. Public policies focused on guaranteeing the human right to adequate food must be improved and implemented for populations in contexts of poverty with the aim of providing food security.
Resumo O surgimento da COVID-19 no Brasil explicitou ainda mais a enorme discrepância entre diferentes realidades sociais que coexistem no país, reacendendo as discussões acerca da segurança alimentar e nutricional, à semelhança do que vem acontecendo em outros países que enfrentam a mesma situação de pandemia. Argumenta-se neste trabalho que os riscos para a Segurança Alimentar e Nutricional (SAN) e a fome dos brasileiros já vinham se apresentando desde 2016, sendo agora aprofundados pela emergência da epidemia da COVID-19, passando a exigir compreensão da extensão e da magnitude dos problemas e articulação de medidas governamentais nas três esferas de gestão (federal, municipal e estadual), que possam assegurar o acesso à alimentação adequada e saudável, com vistas a reduzir os impactos negativos da doença na condição de alimentação, saúde e nutrição dos mais vulneráveis. Assim, este texto pretende contribuir para o debate sobre as medidas a serem adotadas pelos governos e sociedade para promover e garantir a SAN e impedir que a insegurança e a expansão da fome avancem durante e após a crise social e sanitária gerada pela pandemia.
IntroduçãoDiante da complexidade do processo saúde-doença, além do reconhecimento da cidadania como fundamental no enfrentamento da realidade socioeconômica e sanitária, ressaltamos a necessidade da reflexão permanente acerca da formação em saúde. Acreditamos que esta deve contemplar muito mais que as habilidades técnicas, as quais são importantes para a prática profissional em saúde, porém são insuficientes para promover mudanças consistentes nos fatores condicionantes e determinantes da saúde, bem como para sustentação dos preceitos do Sistema Único de Saúde (SUS).Ao referirmos o termo formação, ainda que permeado por ambiguidades, múltiplos sentidos e práticas nos diversos contextos, sendo recorrente o debate quanto à separação da competência técnica e do compromisso político, coadunamos com Barros 1 ao afirmar a indissociabilidade entre o técnico e o político desse processo formativo. Tal afirmação implica repensar a concepção de formação, que não pode ser resumida aos processos de aquisição, transmissão e difusão de conhecimentos que ocorrem nos ambientes acadêmicos, exigindo diferenciar formação e escolarização, tal como referem Guimarães e Silva 2 . De acordo com tais autores, na atualidade, as concepções de formação universitária contêm modelos de ciência, formação e atuação profissionais com sérias limitações relativas aos problemas que se apresentam na cena contemporânea 2 .A tradição cultural brasileira privilegia a condição da universidade como lugar de ensino, entendido e, sobretudo, praticado como transmissão de conhecimentos 3 . Contudo, a universidade vive um momento de transformação efetiva, permeada pela crise de legitimidade e pelos questionamentos de seu papel na produção e construção de conhecimentos, sendo um desafio formar profissionais com perfil adequado às necessidades sociais. Isso implica propiciar, aos estudantes, a capacidade de aprender a aprender, de trabalhar em equipe, de comunicar-se, de ter agilidade frente às situações e de ter capacidade propositiva, que não combinam com a formação tradicional ou com a pedagogia de transmissão, ainda presentes nas instituições universitárias 4 .Ao refletirem sobre questões acerca de uma universidade socialmente relevante, Mello et al. 5 apontam desafios para a universidade pública brasileira espaço aberto
Resumo O contexto brasileiro de desigualdades sociais e barreiras no acesso aos serviços de saúde pode agravar a situação da pandemia de COVID-19, que já afeta todos os estados da federação, com a curva crescente de aumento de casos confirmados e mortes. O governo dos países e os agentes do campo científico têm buscado evidências para as melhores práticas de prevenção e controle da transmissão, e cuidado da infecção e doença, incluindo medidas de diagnóstico, tratamento e de atenção à saúde. A estratégia de testagem em larga escala, visando o diagnóstico precoce, quarentena dos casos leves identificados, bem como dos contactantes, e cuidado adequado dos casos graves, tem sido revisada e indicada como uma das medidas eficientes para o controle da pandemia em vários países do mundo. O artigo tem como objetivo discutir os desafios da testagem e do diagnóstico de COVID-19 no Brasil.
Studies conducted among populations of tropical countries have reported high prevalences of vitamin D deficiency and insufficiency. Information resulting from meta-analyses on the spatial distribution of vitamin D deficiency and insufficiency in tropical countries is still rare. The aim of this review was investigated the prevalence of vitamin D deficiency and insufficiency among the Brazilian population. Observational studies were searched in eight electronically databases. Additionally, theses and dissertations and abstracts were screened. Details on study design, methods, population, mean and data on serum concentrations of vitamin D in different age groups in Brazil were extracted. Data were pooled using a random-effects model and choropleth maps were created based on the geopolitical regions of the country. 72 published paper met the inclusion criteria. The mean vitamin D concentration among the Brazilian population between 2000 and 2017 of 67.65 nmol/L (95% CI: 65.91, 69.38 nmol/L).The prevalences of vitamin D deficiency and insufficiency were 28.16% (95% CI: 23.90, 32.40) and 45.26% (95% CI: 35.82, 54.71), respectively, for the Brazilian population. The highest prevalence of deficiency were observed in the southern and southeastern regions and the highest occurrence of vitamin D insufficiency was among the populations of the southeastern and northeastern regions. Finally, there are high prevalence of inadequate vitamin D concentrations among the population, regardless of age group in Brazil. The development of vitamin D food fortification policies in needs to be cautious and carefully planned.
