Approved labels for second-generation AP agents cover a much narrower range of indications than any of the first-generation agents, and this explains the higher proportion of off-label prescriptions. In contrast, the everyday use of first- and second-generation APs cover a similar range of clinical indications.
La complessità del dibattito sulla qualità della vita si riflette nelle definizioni proposte (Oliver el al., 1996); alcune sono ampie ma vaghe, tali da non specificare il contenuto del concetto e quindi da ostacolarne la misurazione: “La qualità della vita è un ampio concetto che comprende tutti gli aspetti dell'esistenza di un individuo” (Torrance, 1987); “Nell'epoca moderna qualità della vita significa salute e benesse re, idee ampiamente comprese ed accettate dalle persone” (Oliver, 1991). Alcune definizioni si focalizzano sugli indicatori oggettivi di benessere: “La qualità della vita consiste nell'ottenere le condizioni necessarie per la felicità in una data società” (McCall, 1975). Altri autori definiscono la qualità della vita come senso di benessere legato alla situazione contingente o ad una qualche combinazione di benessere soggettivo ed oggettivo: “La qualità della vita è uno stato di benessere che riflette le condizioni di vita, la soddisfazione e l'adattamento a queste condizioni di vita” (Franklin et al., 1986); “La qualità della vita è il senso di benessere sperimentato da una persona all'interno delle sue attuali condizioni di vita” (Lehman, 1983). La centralità dell'aspetto soggettivo si ritrova anche nella definizione che recentemente la WHO ha dato di “qualità della vita”: “…..è una percezione individuale della propria posizione nella vita all'interno del conteste della cultura e dei valori in cui si vive, in relazione ai propri scopi, aspettative, standard ed interessi” (WHOQOL Group, 1995).
La ricerca psichiatrica ha investito molte risorse per definire, in termini operazionali, i sintomi, le diagnosi, il funzionamento sociale dei pazienti. Ha invece sino ad ora sottovalutato una grande massa di informazioni utili per lo studio delle malattie e per la comprensione della persona affetta da disturbi psichici, informazioni che il paziente comunicherebbe facilmente o che sarebbero facilmente osservabili se solo si diventasse sensibili ad esse (Strauss, 1989). Spesso, poi, gli strumenti utilizzati per la quantificazione di questi fenomeni forniscono informazioni grossolane, che mancano di profondita, di dettaglio, di finezza, cioe di caratteristiche fondamentali per la comprensione dei fenomeni osservati. Per favorire nuove conoscenze e quindi necessario superare questi approcci rigidi e stereotipati, attraverso uno sforzo creativo che riguardi sia gli oggetti che i metodi di studio, senza che, per questo, venga meno il rigore scientifico (Ruggeri, 1996).
L'importanza della qualità della vita come parametro di valutazione dei servizi psichiatrici è ormai riconosciuta da una vasta letteratura (Lehman, 1995; Oliver et al., 1996; Barry & Zissi, 1997; Katschnig et al., 1997; de Girolamo et al., 1999). Ciò nonostante non vi è ancora completo accordo sulla sua definizione e misurazione, al punto che da parte di pressoché tutti i ricercatori che si sono cimentati nello studio della qualità della vita vengono rilevate non indifferenti difficoltà metodologiche. Si riscontra tuttavia un generale accordo su alcuni punti fondamentali, con un crescente riconoscimento della soggettività della qualità della vita. Il concetto di qualità della vita si è modificato, infatti, nel corso degli anni da una prospettiva puramente sociologica ed oggettiva di standard di vita ad una prospettiva psicosociale, nella quale è attribuita altrettanta importanza all'aspetto soggettivo di benessere e soddisfazione personale.
Lo studio della qualità della vita nei pazienti affetti da disturbi mentali risale ai primi anni '80 (Malm et al., 1981; Lehman et al., 1982; Lehman, 1983; Baker & Intagliata, 1982; Bigelow et al., 1982). Negli stessi lavori compare per la prima volta un tentativo di riflessione teorica sul concetto di qualità della vita in psichiatria: un elemento condiviso dagli autori che si sono occupati di qualità della vita in quel periodo è il superamento delle tradizionali misure legate alla malattia (sintomi, compromissione, disabilità) per includere nell'indagine l'esperienza che il paziente ha della propria condizione di vita; tuttavia, vi sono notevoli differenze tra i vari autori nella concettualizzazione del rapporto tra le condizioni obiettive di vita e la loro percezione soggettiva e circa i fattori che influenzano questa relazione. Descriveremo di seguito alcuni modelli di qualità della vita facendo riferimento alla recente opera di Katschnig et al. (1997).
