Resumo Este artigo apresenta e resume os principais achados em relação às repercussões do Programa de Volta para Casa nos territórios existenciais de seus beneficiários. Para isso, insere-se no bojo dos estudos internacionais e nacionais sobre o impacto das políticas públicas e avaliação do próprio beneficiário sobre elas. Dado o volume e a extensão dos dados, optou-se por apresentar análises a partir das narrativas construídas, elencando-se quatro aspectos: história de vida, autonomia, o que o dinheiro faz poder e relação com a rede de saúde. Considerando que os(as) participantes contam com anos ininterruptos de internação psiquiátrica, os achados evidenciaram um perfil de pessoas majoritariamente em situação de vulnerabilidade socioeconômica, com predominância de raça/cor negra e baixa escolaridade. O histórico de exclusão e negligência, portanto, teria contribuído para o adoecimento, reclusão e permanência em instituições fechadas. Pode-se afirmar que o Programa de Volta para Casa, associado à moradia, possibilitou a essas pessoas em processo de desinstitucionalização o aumento de poder contratual quanto ao cuidado de si, ao estabelecimento de relações afetivas, à circulação na cidade, ao consumo de bens e serviços e, consequentemente, maior capacidade de expressão, comunicação e posicionamento crítico. Foi possível observar novas esferas de negociação engendradas pelo recebimento do dinheiro.
This study addresses mental health and, based on a conceptual review, offers considerations on the management of deinstitutionalization processes regarding individuals interned in long-stay psychiatric institutions. Elements concerning asylum formation and logic are discussed, along with the mechanisms necessary for the effective change in paradigm and practices, with deinstitutionalization and psychosocial rehabilitation as the core issues. Reflections are offered regarding management actions committed to the psychosocial model, linking such actions to the application of the components of care and going beyond the articulation of the tools of mental health policy. Theoretical reflection offers suggestions referring to the qualification processes of mental health professionals, deinstitutionalization in the management of the Unified Health System and tripartite action with co-accountability in actions and financing. The final considerations recognize the bureaucratic obstacles in the public realm and propose facing these challenges as a management challenge, along with processes of change that can radically commit to the lives of people, thereby broadening the discussion to the ethical realm.
O referido artigo relata a experiência de um grupo de pessoas que atuaram como ativadores de rede e tutora no Projeto Engrenagens Ativação de Redes no ano de 2016, envolvendo dentre outros, os municípios de Colinas em Tocantins, Morrinhos em Goiás e São Bernardo do Campo em São Paulo. A partir de diferentes narrativas, expressa a constituição de um processo coletivo balizado na perspectiva da desinstitucionalização que, em contextos políticos e técnicos adversos, pautou-se em intervenções, estratagemas e rupturas para o tecimento cotidiano da rede de atenção psicossocial no território. O principal resultado do processo paira na materialização do cuidado psicossocial a partir da produção de articulações e relações que costuram uma rede viva e, ao mesmo tempo, a sustentam. A possibilidade dos profissionais da rede ter contado com um apoiador no cotidiano e por um período prolongado de tempo (alguém que ajudou na problematização, articulação e construção de respostas diante dos desafios diários), produziu, mesmo que parcialmente, experiências concretas de organização e sustentação de situações de crise nos serviços substitutivos, a partir de um pacto e operação coletiva. As experiências e narrativas apontam, ao final, que o cuidado em liberdade se desenha processualmente e a partir de entrelaçamentos de ações permeadas pela dimensão técnica e da gestão. Reafirmam ainda que a luta por uma sociedade sem manicômios não é o ponto de chegada, mas o ponto de partida que se constitui como uma operação permanente ética, clínica e política de transformação das realidades.Palavras-chave: Reforma Psiquiátrica. Este artículo analiza las repercusiones del Programa De Vuelta a Casa, un programa brasileño de transferencia de recursos para personas que reciben el alta de hospitalizaciones psiquiátricas. El objetivo fue relevar las repercusiones de este programa en sus beneficiarios, con relación a las implicancias de acceder a un ingreso para su vida cotidiana, y poner en evidencia algunos entrecruzamientos de sus contextos de vida con los territorios existenciales: sexualidad, género, envejecimiento y violencias urbanas. Entre 2017 y 2018, se realizó un estudio evaluativo, centrado en los itinerarios de vida de 20 personas. Como resultado, se observó que acceder a un ingreso es fundamental para la experiencia de vivir fuera del hospital, para el ejercicio del intercambio material y para incursionar en el ámbito comunitario. Se discute acerca de los desafíos de garantizar un apoyo asistido a los beneficiarios con base en la contractualidad y calificación de los procesos de rehabilitación psicosocial, articulados a los entrecruzamientos que marcan los modos de vivir.
