Resumo Este artigo apresenta e resume os principais achados em relação às repercussões do Programa de Volta para Casa nos territórios existenciais de seus beneficiários. Para isso, insere-se no bojo dos estudos internacionais e nacionais sobre o impacto das políticas públicas e avaliação do próprio beneficiário sobre elas. Dado o volume e a extensão dos dados, optou-se por apresentar análises a partir das narrativas construídas, elencando-se quatro aspectos: história de vida, autonomia, o que o dinheiro faz poder e relação com a rede de saúde. Considerando que os(as) participantes contam com anos ininterruptos de internação psiquiátrica, os achados evidenciaram um perfil de pessoas majoritariamente em situação de vulnerabilidade socioeconômica, com predominância de raça/cor negra e baixa escolaridade. O histórico de exclusão e negligência, portanto, teria contribuído para o adoecimento, reclusão e permanência em instituições fechadas. Pode-se afirmar que o Programa de Volta para Casa, associado à moradia, possibilitou a essas pessoas em processo de desinstitucionalização o aumento de poder contratual quanto ao cuidado de si, ao estabelecimento de relações afetivas, à circulação na cidade, ao consumo de bens e serviços e, consequentemente, maior capacidade de expressão, comunicação e posicionamento crítico. Foi possível observar novas esferas de negociação engendradas pelo recebimento do dinheiro.
Resumo Apresenta-se o percurso metodológico de uma pesquisa multiterritorial e multimétodos que avaliou os efeitos do Programa de Volta para Casa (PVC) no processo de reinserção social de pessoas egressas de longos períodos de internação psiquiátrica. O PVC, instituído pela Lei no 10.708/2003 para cumprir papel fundamental no redirecionamento da atenção em saúde mental no Brasil, consiste em um auxílio pecuniário que pretende facilitar o processo de desinstitucionalização e reinserção social dos seus beneficiários. Em cada município pesquisado foi implantado um Comitê de Acompanhamento da Pesquisa (CAP), composto por beneficiários, trabalhadores e representantes da comunidade local. Pesquisou-se a história do PVC por meio de análise documental e entrevistas com os principais atores responsáveis pela implantação do programa. Por meio da observação participante e da realização de entrevistas individuais e coletivas, buscou-se uma aproximação ao cotidiano dos beneficiários, visando construir narrativas sobre cada trajetória. Por fim, analisou-se avanços e desafios na construção de redes afetivas e apoio psicossocial. Conclui-se que o PVC promoveu efetivamente ganhos individuais importantes aos beneficiários, contribuindo com a inserção social no cotidiano das cidades, revelando-se como importante dispositivo no caminho de construção de cidadania e de aumento do poder contratual no contexto da Reforma Psiquiátrica brasileira.
RESUMO Sob a perspectiva da Reforma Psiquiátrica, a presente pesquisa objetivou apresentar relatos de violência institucional em internações de longa duração em um manicômio a partir de itinerários de vida de pessoas em processo de desinstitucionalização. Foi realizada observação participante de nove beneficiários do Programa De Volta para Casa desospitalizados há 13 anos em município de grande porte da Bahia. Os diários de campo, assim como transcrições de entrevistas semiestruturadas com três profissionais de saúde, foram transformados em nove narrativas, que, analisadas qualitativamente, permitiram identificar violências sofridas no período anterior à admissão no hospital e da internação. As razões de internação diferiram por gênero: homens foram internados por crimes contra a pessoa; mulheres, por ‘quebra de resguardo’, frustrações amorosas e violência física e sexual. Durante a internação, emergiram o cotidiano administrado e a exposição à violência institucional (ausência de atividades socioterapêuticas, medicação em doses punitivas com efeito sedativo, negligência à saúde física, agressão física, confinamento, ‘injeção’ e ‘choque elétrico’, preconceito, estigma), em que os sujeitos tiveram neutralizadas suas possibilidades de reação à dominação. Os resultados proporcionaram reflexão sobre a violência do Estado, crucial no atual contexto de desconstrução da Política Nacional de Saúde Mental.
