BCL2 protein expression is a positive prognostic factor in BC. Better survival of patients with BCL2 positivity (BCL2+) has been explained by the correlation with estrogen receptor positive (ER+) status. BCL2+ is however not simply a surrogate marker for ER+. Moreover, BCL2 protein expression is also a positive prognostic marker in the TNBC subgroup. We and others show, that low BCL2 expression was associated with good outcome of TNBC patients treated with both adjuvant and neoadjuvant anthracycline-based chemotherapy. On the other hand, recent studies have shown that a subset of TNBC patients may benefit from the classical adjuvant CMF (cyclophosphamide, methotrexate, 5-fluorouracil) regimen. Given the heterogeneity of TNBC there is an urgent need to find and validate the sensitivity predictors to these regimens making them usable in clinical practice. BCL2 enrichment has been described in the mesenchymal stem-like (MSL) TNBC subgroup.
Over the past few years, new biomarkers have allowed a deeper insight into gliomagenesis and facilitated the identification of possible targets for glioma therapy. Isocitrate dehydrogenase (IDH) 1 and IDH2 mutations have been shown to be promising biomarkers for monitoring disease prognosis and predicting the response to treatment. This review summarizes recent findings in this field. Web of Science, Science Direct, and PubMed online databases were used to search for publications investigating the role of IDH in glioma. References were identified by searching for the keywords "IDH1 or IDH2 and glioma and diagnostic or predictive or prognostic" in papers published from January, 2008, to April, 2014. Only papers in English were reviewed. Publications available only as an abstract were not included. IDH1/2 mutations are tightly associated with grade II and III gliomas and secondary glioblastomas, with better prognosis and production of a recently described oncometabolite, 2-hydroxyglutarate (2HG). Although the contradictory positive effect of IDH mutation on prognosis and negative role of 2HG in tumor transformation remain unresolved, the future direction of personalized treatment strategies targeted to glioma development is likely to focus on IDH1/2 mutations.
Mutations in IDH1/2 genes are a marker of good prognosis for glioma patients, associated with low grade gliomas and secondary glioblastomas. Immunohistochemistry and Sanger sequencing are current standards for IDH1/2 genotyping while many other methods exist. The aim of this study was to validate Competitive amplification of differentially melting amplicons (CADMA) PCR for IDH genotyping by comparison with SNaPshot assay and two immunohistochemical methods. In our study, 87 glioma patients (46 from Olomouc and 41 from Ostrava) were analyzed. IDH1/2 mutations in native bioptical samples were analyzed at DNA level by CADMA and SNaPshot while IDH1 mutations in FFPE samples were analyzed at protein level by two IHC methods. CADMA PCR sensitivity for IDH1 was 96.4% and specificity 100% for 86 concluded samples. SNaPshot assay sensitivity was 92.9% and specificity of 100% for 85 concluded samples. IHC in the laboratory no. 2 reached sensitivity 85.7% and specificity 100% for 86 concluded samples. IHC in the laboratory no. 4 reached sensitivity of 96.4% and specificity of 79.7% in 74 concluded samples. Only one IDH2 mutation was found by SNaPshot while CADMA yielded false negative result. In conclusion, CADMA is a valid method for IDH1 p.(R132H) testing with higher sensitivity than SNaPshot assay. Also, molecular genetic methods of IDH1 testing from native samples were more robust than IHC from FFPE.
UP a FN Olomouc 2 Onkologická klinika LF UP a FN Olomouc 3 Laboratoř molekulární patologie, Oddělení patologie, Ústav molekulární a translační medicíny, LF UP a FN Olomouc 4 Ústav molekulární a translační medicíny, LF UP v Olomouci Souhrn Východiska: Maligní nádor pochvy periferního nervu (malignant peripheral nerve sheath tumour-MPNST), zvaný též maligní schwannom, je velmi vzácný nádor, který tvoří pouze 2 % všech sarkomů. Nádor má poměrně špatnou prognózu, kdy 5leté celkové přežití dosahuje 46-69 %. Léčba MPNST není nadále standardizována. Základem je radikální resekce. Onkologická adjuvantní či neoadjuvantní léčba má nejasné indikace s nejasným efektem. Případ: Naše kazuistika prezentuje 55letého pa cienta vyšetřovaného pro rezistenci na medio-ventrální ploše levé dolní končetiny. MRI ukázala nádor levého stehna nasedajícího na n. femoralis. Byla provedena radikální exstirpace nádoru. Histologický nález odhalil maligní schwannom (MPNST) a resekce byla hodnocena jako stupeň R0. Pooperační celotělové PET/ CT neodhalilo viabilní nádorovou tkáň. Nemocný byl dále dispenzarizován. Na kontrolním MRI, 53 měsíců od úvodní operace, se objevil nález recidivy nádoru. Provedena exstirpace recidivy nádoru na stehně levé dolní končetiny. Histologické vyšetření potvrdilo MPNST, radikalita resekce byla hodnocena jako R2. Pooperační PET/ CT objevilo ve stěně resekční dutiny rezidua nádoru. Proto proběhla 58 měsíců od první operace další operace zbývajícího tumoru, a to do stupně R0. Do resekční dutiny byly umístěny tři aplikátory intersticiální brachyterapie. Po operaci proběhla radioterapie-intersticiální brachyterapie (18 Gy) a následně zevní frakcionované radioterapie (50 Gy). Poslední MRI 66 měsíců od dia gnózy MPNST neukázala žádnou nádorovou tkáň. Pa cient je nadále bez neurologického deficitu. Závěr: Základem terapie MPNST je radikální en bloc resekce. Použití následné onkologické léčby závisí právě na radikalitě resekce. V našem případě vzhledem k dobré radikalitě první operace byla adjuvantní onkologická terapie postponována. V rámci řešení recidiv jsme se opět snažili o dosažení co nejradikálnější resekce s následnou adjuvantní radioterapií. U MPNST jako u všech sarkomů měkkých tkání se z důvodu potenciální radiorezistence volí vysoké dávky radioterapie. Vzhledem k ohraničenosti onemocnění jsme zvolili tento lokálně intenzifikovaný strategický postup, který vedl v této chvíli k remisi onemocnění. Vzhledem k nízké incidenci MPNST není možné ověřit efektivitu jednotlivých onkologických léčebných postupů na větším souboru pa cientů. Východiska: Gliosarkom je vzácný maligní nádor CNS s velmi špatnou prognózou. Gliosarkom je variantou glioblastomu multiforme charakterizovaný přítomností jak gliální, tak mezenchymální komponenty. Léčebná strategie u gliosarkomů není dosud jednoznačně stanovena. Pozorování: Naše kazuistika prezentuje 23letou pa cientku, která byla vyšetřována pro narůstající bolesti hlavy, nauzeu, zvracení a diskrétní motorické oslabení levé paže. MRI mozku odhalilo nádor vyplňující přední část pravé postranní komory s extenzí do pravého čelního laloku. Byla provedena exstirpace nádoru. Histologický nález odhalil gliosarkom. Následně absolvovala konkomitantní chemoradioterapii s temozolomidem ve Stuppově režimu. Po čtvrtém cyklu pokračovací chemoterapie temozolomid solo byla osm měsíců od dia gnózy na MRI nalezena progrese rezidua nádoru. Pa cientka podstoupila druhou operaci a poté absolvovala 10 cyklů 2. linie chemoterapie v režimu ICE (ifosfamid, karboplatina, etoposid). Pa cientka zvládla onkologickou léčbu s minimální toxicitou a na kontrolních MRI došlo prakticky k vymizení reziduálního nádoru. Na kontrolním MRI, 28 měsíců od dia gnózy, došlo opětovně k progresi rezidua nádoru. Třetí operace nádoru byla provedena 29 měsíců od dia gnózy. Histologický nález opět potvrdil gliosarkom. Časné pooperační MRI ukázalo subtotální resekci opět s reziduem nádoru v elokventních oblastech a nově i suspektní implantační metastázu v páteřním kanále ve výši C2. Z neurologického pohledu byla pa cientka plně soběstačná, pozůstává jen velmi lehký motorický deficit na levé paži. Nyní, 31 měsíců od dia gnózy, je pa cientka ve stabilizovaném stavu, plně soběstačná. Závěr: Naše kazuistika ukazuje dlouhodobé přežívání u pa cientky s gliosarkomem, který měl všechny nepříznivé klinicko-patologické znaky. Je obecně akceptováno, že maximální resekce následovaná pooperační radioterapií je minimální doporučenou léčbou. Naše pa cientka podstoupila chemoradioterapii s temozolomidem ve Stuppově režimu. Recidiva osm měsíců od dia gnózy byla řešena opakovanou operací následovanou intenzivní kombinovanou chemoterapií, která od prvního relapsu po dobu 20 měsíců udržela onemocnění v remisi. Absence jednoznačných pravidel pro chemoterapii dává možnost použití i méně obvyklých režimů. Breast cancer consists of multiple diff erent molecular subtypes and diff erent biological processes, and consequently diff erent molecular markers are associated with prognosis and chemotherapy sensitivity in the distinct disease subsets [1]. A large number of biological processes including cell cycle regulation, DNA replication, mitotic spindle checkpoint, and p53 function are strongly prognostic in ER + cancers but not among ERcancers [2,3]. Interestingly, the number of biological pathways, and therefore genes, that are associated with prognosis or treatment sensitivity are substantially larger and more consistent in ER + cancers than among ERtumors [1,4]. This implies that it is easier to discover prognostic and predictive markers for ER + than for ERcancers. In ERcancers, the single most consistent, but still modestly accurate, good prognostic predictor is the presence of immune cell infi ltration [5]. Immune cell signatures are also associated with more favorable prognosis in highly proliferative ER + cancers but not in ER + cancers with low proliferation [6]. It is also increasingly clear that the same molecular marker can be associated with several diff erent outcome endpoints in various and often opposing manners. For example, high Ki67 expression is predictive of worse prognosis in the absence of any systemic therapy in ER + cancers, but at the same time it is also predictive of higher sensitivity to chemotherapy. Similar opposing bidirectional associations with treatment response and prognosis exist for many other markers including histologic grade, Tau protein expression and almost all prognostic gene signatures [7]. It is important to be aware of these complex multi-directional interactions between molecular markers and various clinical endpoints that may also vary from breast cancer subtype to subtype. Ignoring these potential marker-disease subset-outcome interactions can lead to contradictory and confusing results across studies (due to diff erences in patient composition and heterogeneity of therapy between studies) and may also lead to the discovery of biomarkers that are clinically less useful (because of unrecognized subtype-restricted performance) [8,9]. IL-6 signaling is involved in the pathogenesis of a number of serious diseases, including chronic inflammation and cancer. Targeting of IL-6 receptor (IL-6R) by small molecules is therefore an intensively studied strategy in cancer treatment. We describe the design, synthesis, and characteristics of two new bis-pentamethinium salts 5 and 6 (meta and para) bearing indole moieties. Molecular docking studies showed that both compounds have the potential to bind IL-6R (free energy of binding −9.5 and −8.1 kcal/mol). The interaction with IL-6R was confirmed using microscale thermophoresis analyses, which revealed that both compounds had strong affinity for the IL-6R (experimentally determined dissociation constants 26.5 ± 2.5 nM and 304 ± 27.6 nM, respectively). In addition, both compounds were cytotoxic for a broad spectrum of cancer cell lines in micromolar concentrations, most likely due to their accumulation in mitochondria and inhibition of mitochondrial respiration. In summary, the structure motif of bis-pentamethinium salts represents a promising starting point for the design of novel multitargeting compounds with the potential to inhibit IL-6 signaling and simultaneously target mitochondrial metabolism in cancer cells.
