The decrease in fetal heart rate which is seen occasionally in late pregnancy andespecially during labor is usually related to uterine contractions. Since the fall in heart rate is associated with fetal hypoxia [2,5] an attempt was made to use the dip area (DA) for predicting fetal condition. Dip area was related to the pH in the umbilical vein [9] and to one scalp blood sample taken after measuring DA [8]. Neither the pH in the umbilical vein gives a reliable index for fetal condition nor one scalp blood sample can indicate the changes in pH and BE which are expected during the presence of deceleration. This can only be a vague approximation describing the relationship of DA and acid base observations. Maternal Pco 2 an d blood lactate transfer from mother to the fetus and reverse äs shown by KASTENDIECK and MOLL [4] in animal experiments can influence the fetal pH and base excess. Maternal acid base observations were neverstudied in connection with DA. In previous experiments [3] we have already undertaken an attempt to quantify the relationship between dip area measured by planimetry and the decrease in base excess i.e. the difference in base excess of the scalp blood and the umbilieal arterial blood measured during the respective time.The present experiments are concerned with the relationship of the fall in fetal base excess and dip area during the first stage of labor and the influence of the change of maternal base excess upon fetal base excess.The present results reveal a relationship between the dip area of fetal heart rate and the decrease in base excess for the first stage of labor which is similar to that of the second stage of labor, if the same dip-area is measured. The metabolic parameters in the maternal blood seem to influence the base excess of the fetal blood. l Patient material and methodsThe study comprises 39 women during labor. The gestational age was 40.4 weeks (SD 1.1) and the time of labor 7.7 hours (SD 4.7). The babies were delivered 37 times by spontanous delivery and in one case a vacuum extraction and in one case a caesarean section was performed. In 35 cases the APGAR score was 8 and more, in 3 cases 7 and in one case 6. The weight of the babies was 3450 gr (SD 460) and the weight of the placenta was 530 gr (SD 123). Scalp blood was sampled and analyzed for pH, PCOO an(^ BE as pr ev iously described by WULF, KÜNZEL and LEHMANN [10]. The pH was measured with a commercial pH-meter (RADIO-METER Copenhagen) and the SIGGAARD-ANDERsoN-Nomogram (1962) was used for base-excess (BE) and P CO2 determination. Blood was collected simultanously from the hyperemized earlobe of the mother using FINALGON® and analyzed for the same parameters. A new modification of the "coated-tube"-technique for the radioimmunoassay of human placental lactogen is described. Polyvinylchloride capillaries are used as reaction-vessels. Most working steps of the test procedure are mechanized. The possibilities and limitations of the new technique are discussed. The reliability of the method was shown by a recovery test, dilution tests, reproducibility and a comparison of the results for 71 sera, ranging from the 6th to the 42nd week of pregnancy, with those obtained by a conventional assay procedure. Reaktionsgefäße sind Kapillaren aus Polyvinylchlorid. Die Durchführung des Tests ist weitgehend mechanisiert. Die Arbeitstechnik wird beschrieben, Möglichkeiten und Grenzen der Technik werden diskutiert. Wiederfindung, Verdünnungsreihen, Reproduzierbarkeit, sowie ein Methodenvergleich an 71 Schwangerenseren zeigen die Zuverlässigkeit der Methode.
Erstmals berichteten der Pathologe Vuong und der Gynäkologe Proust aus Frankreich im Jahre 1993 über eine neue Entität, welche in der klinischen Symptomatik durch das Auftreten einer anämisierenden Hypermenorrhö bzw. einer rezidivierenden, therapieresistenten Metrorrhagie gekennzeichnet ist [10]. Histopathologisch liegen dabei typische Veränderungen in Form dilatierter Kapillaren im Endometrium sowie Ektasien venöser Blutgefäûe im Myometrium vor. Ein weiteres ± bereits hysteroskopisch erkennbares ± morphologisches Kriterium ist eine pseudopolypös aufgebaute Korpusschleimhaut mit deutlicher Gefäûzeichnung [11,12]. Seit der Erstbeschreibung dieser so genannten uterinen Angiodystrophie gibt es in der Literatur nur vereinzelte Fallberichte, jedoch liegt die Vermutung nahe, dass die histopathologischen Kriterien noch nicht ausreichend etabliert sind [4]. Im folgenden Fall können wir über das Auftreten einer uterinen Angiodystrophie bei einer 48-jährigen Patientin berichten, die in der Gynäkologischen Klinik des Städtischen Krankenhauses Sindelfingen (Chefarzt: Prof. Dr. med. M. P. Cornely) behandelt wurde. Die histopathologische Untersuchung des Hysterektomiepräparates erfolgte am Institut für Pathologie des Universitätsklinikums Tübingen (Direktor: Prof. Dr. med. B. Bültmann). Zusammenfassung Erstmalig berichteten die Franzosen Vuong und Proust über die uterine Angiodystrophie im Jahre 1993. Diese neue Entität zeigt spezifische histomorphologische Veränderungen der venösen Blutgefäûe im Endo-und Myometrium. Kürzlich beobachteten wir einen solchen Fall, wobei klinisch eine anämisierende Hypermenorrhö vorlag.
Im Jahre 1984 berichteten Soler und Khardori erstmalig über das Auftreten einer fibrösen Erkrankung der Brust in Assoziation mit einem langjährigen Diabetes mellitus Typ I [32]. Drei Jahre später wurde der Terminus ¹diabetic mastopathyª (Mastopathia diabetica) eingeführt [5]. In der bildgebenden Diagnostik imponiert diese Veränderung als umschriebener Tumor unklarer Dignität. Die definitive Diagnose kann trotz sich ständig verbessernder gerätetechnischer Ausrüs-tungen (hochauflösende Ultraschallgeräte, drei-dimensionale [3-D] Mammasonographie) erst histologisch durch den Pathologen gestellt werden. In der folgenden Kasuistik berichten wir über eine Patientin mit einer beidseits aufgetretenen diabetischen Mastopathie im Abstand von vier Jahren. Die Therapie erfolgte in der Gynäkologischen Abteilung des Städtischen Krankenhauses Sindelfingen (Chefarzt Prof. Dr. M. P. Cornely), Akademisches Lehrkrankenhaus der Universität Tübingen. Die histopathologische Aufarbeitung der Operationspräparate wurde am Institut für Pathologie Zusammenfassung In der vorliegenden Kasuistik wird über das Auftreten einer Mastopathia diabetica bilateralis im Abstand von vier Jahren berichtet. Sowohl die klinischen als auch histopathologischen Befunde zeigen jeweils die typischen Charakteristika dieser Erkrankung. Die klinische Relevanz und therapeutische Strategien unter Einbezug minimal-invasiver Techniken werden diskutiert.
