AbstractObjective: To investigate the association between type of delivery and immunoglobulin concentration in maternal colostrum.
Methods:We studied 82 women who were giving birth. Age was between 21 and 41 years, gestational age was 37 or more weeks and parity up to IV pregnancies. The women were in good nutritional condition and did not have any pregnancy or puerperium-associated diseases. The following aspects were also considered as inclusion criteria for the newborn: weight > 2,500 g, Apgar score > 7 in the first minute and exclusive breastfeeding until discharge from the nursery. The women were divided into three groups: A -vaginal delivery, Bcaesarean section with labor and C -elective caesarean section. Colostrum was collected manually between 48 and 72h after delivery. Immunoglobulins were dosed using the ELISA technique.Results: There were no differences between the three groups in terms of time of colostrum collection. The shorter the time of colostrum collection, the greater the concentration of immunoglobulin A. Primiparous women had higher concentrations of IgA and IgM in maternal colostrum than did multiparous women. The group submitted to caesarean section with labor had higher concentrations of IgA in maternal colostrum than did the normal delivery group. IgM and IgG concentrations in colostrum were not influenced by type of delivery.
Conclusion:The occurrence of labor together with surgical stress induce higher IgA concentrations in the colostrum of women submitted to caesarean section with labor.
In order to demonstrate the occurrence of CMV perinatal infection in a middle socioeconomic class population, the authors conducted a 8-month prospective study in 37 children, not infected congenitally, born in a public hospital of São Paulo city, Prevalence of CMV-IgG antibodies in mothers, detected by immunoenzymatic assay (ELISA), was 92.7%. Survival analysis showed that the risk of acquiring CMV perinatal infection diagnosed by virus isolation in human fibroblasts was 30.9%. When the diagnostic method was detection of IgM class antibodies by indirect immunofluorescence the risk was 8.1% (p < 0.05). Milk samples inoculated in human fibroblasts failed to demonstrate the presence of virus. The infected children did not present any signal of disease in a 4-month follow-up.

Determinaram-se os valores hematológicos (Hb, Ht, VCM, HCM, CHCM, reticulócitos) e as concentrações séricas de ferro e ferritina no sangue de cordão umbilical de recém-nascidos (RN) no Hospital Albert Einstein. A amostra continha 32 RN de termo (T) e 30 RN pré-termo (PT). Os RN apresentavam-se clinicamente sadios, sendo excluídos os que apresentaram intercorrências clínicas ou no momento do parto. O sangue foi colhido por punção da veia umbilical até 5 minutos após a dequitação e encaminhado ao laboratório de patologia clínica para processamento laboratorial. Os valores hematológicos encontram-se mais elevados no grupo de RNT em relação ao grupo de RNPT (p<0,05). Ajustando-se um modelo de regressão logística múltipla, o Ht tem valor preditivo para a prematuridade, observando-se uma relação inversamente proporcional entre o valor do Ht e a idade gestacional.

OBJETIVO: Identificar as concentrações das imunoglobulinas no sangue do funículo umbilical comparando-as às do sangue materno no momento do parto. MÉTODOS: Foram analisadas amostras colhidas de 323 RN e de 321 mães pelo método de turbidimetria. Os resultados foram submetidos à análise estatística, estabelecendo-se coeficiente de correlação r e nível de significância p 0,05. As retas de regressão obedeceram ao limite de 95%. RESULTADOS: Houve aumento progressivo de IgG sérica do RN, diretamente proporcional à idade gestacional ( r=0,31;p=0,0001); quanto ao peso de nascimento, esta relação não foi tão evidente (r=0,14;p=0,008), possivelmente por estarem incluídos na amostra RNPT e RN com restrição ao crescimento intra-uterino. O aumento da IgG do RN foi maior que o da mãe (r=0,20;p=0,0001). Houve queda de concentração da IgM com o aumento da idade materna e aumento da IgM e IgA maternas comparadas à IgG do RN. Observou-se queda da concentração de IgG do funículo umbilical quando relacionada ao tempo maior de ruptura da bolsa (r=-0,13;p=0,018). CONCLUSÕES: A concentração de IgG no funículo umbilical aumentou proporcionalmente à idade gestacional e diminuiu com tempo maior de ruptura de membranas.Concentrações de IgM e IgA no sangue materno aumentaram significativamente quantdo comparadas à IgG do funículo umbilical e a IgM diminuiu significativamente com o aumento da idade materna.
OBJETIVO: A avaliação do fluxo sangüíneo mesentérico utilizando-se a Dopplervelocimetria transcutânea de artéria mesentérica superior (AMS) é método não invasivo que permite avaliar fatores envolvidos na perfusão sangüínea intestinal. Este estudo teve como objetivo determinar as variáveis Dopplervelocimétricas da AMS em recém-nascidos normais a termo, relacionando-as aos fatores ligados a gestação e crescimento fetal. MÉTODOS: A amostra continha 58 mães e 59 recém-nascidos a termo (1 gestação gemelar), cujos partos ocorreram no Hospital Israelita Albert Einstein. Utilizou-se equipamento Toshiba SSH 140 com sistema duplex-Doppler colorido de alta resolução, avaliando-se a AMS na sua origem, determinando-se calibre, velocidade sistólica, velocidade diastólica, velocidade média e índices de resistência e pulsatilidade. RESULTADOS: Verificou-se que a média do calibre da artéria mesentérica superior foi 1.94 mm, a média da velocidade sistólica. 86.47 cm/s, a média da velocidade diastólica 17.66 cm/s e a média da velocidade média, 36.93 cm/s. Os valores médios dos índices de resistência e pulsatilidade foram respectivamente 0.79 e 1.91. O calibre da AMS correlacionou-se positivamente com o peso de nascimento e negativamente com o perímetro cefálico ; velocidades e índices guardam relação com os dados antropométricos do recém-nascido, com o número de horas de vida, com o número de minutos em jejum e com o peso da mãe no fim da gestação. CONCLUSÃO: As variáveis Doppleervelocimetricas estão relacionadas a variáveis maternas e dos recém-nascidos, discutem-se fatores possivelmente implicados na associação. Sexo do recém-nascido e peso da mãe no inicio da gestação não alteraram nossos resultados.
