Glucose-6-phosphate dehydrogenase (G6PD) deficiency is an enzymopathy in which reduced NADPH concentrations are not maintained, resulting in oxidative damage. We evaluated G6PD activity, oxidative stress levels and Trolox equivalent antioxidant capacity in individuals with the A-(202G>A) mutation for G6PD deficiency. Five hundred and forty-four peripheral blood samples were screened for G6PD deficiency; we also analyzed lipid peroxidation products measured as thiobarbituric acid reactive species and Trolox equivalent antioxidant capacity. Men with the A-(202G>A) mutation had lower G6PD activity than women with the same mutation. Individuals with the A-(202G>A) mutation also differed in mean Trolox equivalent antioxidant capacity values but not for thiobarbituric acid reactive species values. We concluded that A-(202G>A) mutation is associated with reduced G6PD activity and increased Trolox equivalent antioxidant capacity.
ABSTRACT. Fetal hemoglobin (Hb F) is characteristic of the fetal development period. However, in some genetic conditions, such as hereditary persistence of fetal hemoglobin (HPFH) and deltabeta thalassemia (δβ-thalassemia), Hb F continues to be produced in adulthood. We evaluated the frequency of two mutations of HPFH, HPFH-1 and HPFH-2 African, and two mutations in δβ-thalassemia, Sicilian and Spanish, in a Brazilian population. Peripheral blood samples were collected from adults from hospitals and blood centers in southeast and northeast Brazil. These individuals were healthy and without complaints of anemia, but had increased Hb F. Samples were submitted to electrophoretic and chromatographic analyses to quantify Hb F values and, subsequently, to molecular analyses to verify the mutations. In the molecular analysis, 16 of the 60 samples showed a heterozygous profile for the HPFH mutations, two for HPFH-1 and 14 for HPFH-2. In the same sample set, three were heterozygous for Spanish δβ-thalassemia and none were heterozygous for Sicilian δβ-thalassemia. The Hb F values in the HPFH-2 heterozygotes differed from those previously reported for this mutation. In this group, the HPFH mutations were more frequent than the δβ-thalassemia mutations. The finding of these mutations in this Brazilian population reflects the mixing process that occurred during its formation.
As hemoglobinopatias podem ser resultantes de mutações que afetam os genes reguladores promovendo um desequilíbrio no conteúdo quantitativo das cadeias polipeptídicas e conseqüentemente nos tipos normais de Hb, causando as talassemias, ou, ainda, de alterações envolvendo genes estruturais que promovem a formação de moléculas de Hb com características bioquímicas diferentes das Hb normais, também denominadas Hb variantes. 1 Mais de 800 variantes de Hb já foram descritas até o momento. 2 A maioria delas é originada por simples substituições de aminoácidos, resultantes de mudanças nas seqüências de nucleotídeos. 1 Atualmente, o número de Hb anormais identificadas tem aumentado devido à melhoria nas metodologias de análises; no entanto, muitos laboratórios de rotina não estão preparados para a correta identificação das Hb variantes. Deste modo, objetivou-se caracterizar os mutantes de Hb das amostras de sangue enviadas ao Laboratório de Hemoglobinas e Genética das Doenças Hematológicas (LHGDH), utilizando-se diversas metodologias de análises para evidenciar sua aplicabilidade.Foram analisadas 83 amostras de sangue periférico colhidas com EDTA, após consentimento informado, provenientes de diferentes estados brasileiros e da Colômbia, sem distinção de sexo, idade, etnia ou classe social.As amostras foram avaliadas pelos testes para identificação de hemoglobinopatias que consistiram da análise da morfologia eritrocitária; resistência globular osmótica em NaCl a 0,36 %; eletroforese em pH alcalino em acetato de celulose; eletroforese em gel de ágar, pH ácido e cromatografia líquida de alta pressão (HPLC) em Sistema Variant automatizado da Bio-Rad com kit de análise para beta talassemia heterozigota. 1,3,4,5,6 Para a identificação das cadeias globínicas mutantes foram realizadas as metodologias de eletroforese de cadeias em pH alcalino em acetato de celulose e eletroforese de cadeias globínicas em pH ácido em gel de poliacrilamida 12%. 7,8 Após a realização dos procedimentos clássicos para o diagnóstico de hemoglobinopatias, verificou-se a presença de Hb com padrões de migração diversos, sendo que 37 (45%) delas migraram em posição semelhante à Hb S em pH alcalino. Após a combinação dos resultados obtidos nas metodologias clássicas citadas, as Hb variantes foram submetidas a análises para definição da cadeia globínica alterada.Foram realizadas as eletroforeses de cadeias globínicas em pH ácido e pH alcalino. Em pH ácido, a maioria dos mutantes não apresentou separação das globinas normais (α A β A ), sendo possível reconhecer a fração mutante de Hb C, utilizada como padrão para esses procedimentos eletroforéticas, Hb E Saskatoon, Hb J Oxford e Hb Hasharon, sendo as duas primeiras mutantes de globina beta e as outras, mutantes de globina alfa. Também foram detectadas as cadeias gama alanina e gama glicina nas amostras que apresentaram Hb F aumentada. Em pH alcalino, foi possível identificar a cadeia globínica alterada em 68 (82%) amostras. A variante de globina delta (δ) chamada Hb B2, originalmente identificada como Hb A2, resulta da substituição de uma glicina por uma arginina na posição 16 da cadeia δ. 1 É a variante de cadeia δ mais freqüente no mundo. Tem sido descrita na África e em populações de descendência africana e encontrada em combinação com Hb S, Hb C, beta talassemia, e em homozigose.2 Sua freqüência varia de 1% a 3% em negros americanos, acima de 9% em negros africanos e tem rara distribuição em caucasianos, o que inicialmente levou pesquisadores a assumir a teoria de que a variante era africanoespecífica. 1A ausência de Hb B2 em algumas regiões africanas sugere que esta variante é específica de negros e não necessariamente de africanos. que o gene δ-globina mutado está associado a um único haplótipo, e sua origem poderia ser africana, mas mutações recorrentes não poderiam ser descartadas para explicar esta hipótese devido a ligação entre o agrupamento de genes β com a Hb B2 ser um dos haplótipos mais freqüentes em toda população africana.O gene da δ-globina está localizado próximo ao gene da β-globina no braço curto do cromossomo 11, e devido a esta proximidade, existe uma probabilidade baixa de ocorrer recombinação entre esses dois genes, e uma alta probabilidade dos genes serem herdados juntos, levando à ocorrên-cia de variantes estruturais de cadeia δ e β em conjunto.4
