El objetivo de este artículo es presentar los resultados del proyecto No. 421-B1-261 titulado Influencia de la capacitación de la Práctica Clínica Basada en la Evidencia sobre la práctica diaria del profesional de enfermería, aprobado por la Vicerrectoría de Investigación de la Universidad de Costa Rica. El compromiso de renovar la práctica de Enfermería en Costa Rica se relaciona de manera directa con el mejoramiento de la salud de la sociedad tanto en lo colectivo como en lo individual, con el fin de reducir los errores en la práctica al ajustarla a los avances científicos. Se realizó una búsqueda en las bases de datos de EBSCO Host, PubMed, Academic Research Complete, MedLine Plus y Scielo. Se llevó a cabo una búsqueda preliminar, luego se introdujeron límites de búsqueda por fecha y descriptores; por último, para el análisis crítico fueron incluidos sólo a texto completo por título y resumen 30 documentos que incluyeron revisiones de literatura, estudios primarios y marcos conceptuales que fueran relevantes para dar respuesta a la pregunta clínica. De los 30 documentos solo se consideró seis de ellos por su contenido, estructura metodológica y calidad. Aunque no se halló una respuesta contundente y directa a la pregunta clínica planteada en este estudio, por la revisión efectuada se puede concluir que los profesionales de Enfermería consideran esencial el enriquecimiento de una práctica clínica fundamentada en la investigación basada en la evidencia, lo cual genera una mayor calidad y seguridad en la prestación del cuidado ejecutado.
El objetivo de la presente investigación fue identificar la prescripción de medicamentos psicotrópicos: Clobazam, Tiopental, Clonazepam, Lorazepam, Fenobarbital y Diazepam en la Caja Costarricense del Seguro Social (CCSS) en Costa Rica en el periodo 2011-2015. Metodología. La población estuvo constituida por 1 488 989 prescripciones de psicotrópicos dentro de la CCSS en Costa Rica en el periodo de enero del 2011 a diciembre del 2015. El estudio es de tipo descriptivo retrospectivo. Resultado. Del total analizado, casi la mitad (un 47,15%) corresponden a Clonazepam. Le siguen el Lorazepam y el Diazepam con un 22,80% y un 22,53%, respectivamente. Finalmente, Fenobarbital, Clobazam y Tiopental son las menos prescritas con un 7,07%, 0,24% y 0,22% respectivamente. En cuanto a la distribución por sexo las mujeres recibieron el 62,65% de las prescripciones, el restante 37,35% fueron para hombres. Conclusión. Existe un aumento exponencial en el consumo de estos fármacos en el quinquenio en estudio. Las personas al final de su ciclo productivo (40 años o más) alcanzan los mayores porcentajes de consumo que luego decrecen alrededor de los 70 años o más; las mujeres son a quienes más se les prescriben este tipo de medicamentos. Por tanto, es necesaria una monitorización continúa que favorezca un uso adecuado de estos fármacos y evite generar potenciales problemas a la salud de la población especialmente a los adultos mayores
El presente ensayo tiene por objetivo hacer una reflexión en torno a la educación prenatal como parte de la Educación para la Salud. Para ello se toman diferentes enfoques y modelos educativos propuestos a través de la historia cuyo papel ha sido protagónico respecto de la toma de decisiones en distintos contextos sociales. Para este análisis se parte del concepto de educación en general y de Educación para la Salud como fundamento de los enfoques y modelos desarrollados; por otra parte, se presenta también algunos aspectos importantes relacionados con la complejidad en salud para abordar la situación de la Educación Prenatal, a partir de cuatro modelos muy actuales. De esta reflexión se concluye que el abordaje de la Educación para la Salud no responde a un solo enfoque, sino a una mezcla de ellos. Por otro lado, se tiene que la complejidad en salud puede verse como una construcción de un espacio en donde los grupos humanos se relacionan con el ambiente. Finalmente, la Educación Prenatal se ha visto influenciada por dicho enfoque y distintos modelos que promueven un proceso que inicia desde antes de la concepción, continúa a través de ésta y va más allá del parto y el puerperio.
This was a quantitative, descriptive, cross-sectional study. The participant population consisted of 69 students who were attending the fifth year of the Nursing Degree course. A self-administered questionnaire was applied, which was previously validated with a sample of nursing students that were attending other years of the course. The instrument was divided into five domains: general, sociodemographic, economic, personal, familiar and sociocultural information. The study evidence, that the phenomenon of drugs is multifactorial, is associated with sociodemographic, economic, personal, familiar and sociocultural characteristics. The students were predominantly female, of the middle-economic class, with good academic performance, but had unhealthy life styles. Concerning drug consumption, 44.9% had consumed alcoholic beverages and consumed at the time of the study, another smaller percentage smoked tobacco and 1.4% smoked marijuana; in addition, a clear knowledge of risk behaviors and of the consequences to which they are exposed did not exist within this group.
RESUMENObjetivo: analizar la mejor evidencia científica disponible en la intervención de enfermería a las personas con pie diabético para el mejoramiento de su calidad de vida. Método: revisión de literatura. La metodología para la práctica clínica basada en evidencia, que consta de cinco pasos: 1. Presentación de la pregunta clínica según el acrónimo PICO (población, intervención, comparación y resultado); 2. Buscar información en bases de datos indexadas: Medline, PubMed, EBSCo, CINAHL; 3. Análisis crítico de la documentación seleccionada; 4. y 5. Transferencia y evaluación de evidencia científica. Resultados: las intervenciones educativas y el empoderamiento al usuario sobre su enfermedad es esencial en la atención a los pacientes diabéticos para la prevención de complicaciones incidiendo en la calidad de vida y prevención de los reingresos hospitalarios. Conclusión: existe evidencia científica de buena calidad metodológica que indica que la atención al paciente con pie diabético debe ser multidisciplinar, donde la enfermería tiene un papel fundamental educando para el empoderamiento de la enfermedad al usuario y de esta manera mejorar la calidad de vida disminuyendo las complicaciones asociadas al pie diabéticico.ABSTRACT Objective: to analyze the best scientific evidence available in the nursing intervention for people with diabetic foot disease in order to improve their quality of life.
