The findings can help nurses during the diagnostic process, as they identify which defining characteristics can be used to confirm or rule out the probability of occurrence of the diagnosis.
OBJECTIVE: to analyze the accuracy of the defining characteristics of the Impaired gas
exchange nursing diagnosis in children with acute respiratory infection. METHOD: open prospective cohort study conducted with 136 children monitored for a
consecutive period of at least six days and not more than ten days. An instrument
based on the defining characteristics of the Impaired gas exchange diagnosis and
on literature addressing pulmonary assessment was used to collect data. The
accuracy means of all the defining characteristics under study were computed. RESULTS: the Impaired gas exchange diagnosis was present in 42.6% of the children in the
first assessment. Hypoxemia was the characteristic that presented the best
measures of accuracy. Abnormal breathing presented high sensitivity, while
restlessness, cyanosis, and abnormal skin color showed high specificity. All the
characteristics presented negative predictive values of 70% and cyanosis stood out
by its high positive predictive value. CONCLUSION: hypoxemia was the defining characteristic that presented the best predictive
ability to determine Impaired gas exchange. Studies of this nature enable nurses
to minimize variability in clinical situations presented by the patient and to
identify more precisely the nursing diagnosis that represents the patient's true
clinical condition.
Objectives: To identify the prevalence of nursing diagnoses: ineffective breathing pattern (IBP) (00032), ineffective airway clearance (IAC) (00031), impaired gas exchange (IGE) (00030) and impaired spontaneous ventilation (ISP) (00033), their defining characteristics and related factors, in children with acute respiratory infection. Methods: A quantitative, transversal approach with 151 children. Data were collected by means of interviews and pulmonary evaluation. For data analysis, we used descriptive and inferential statistics. Results: The most prevalent diagnosis was IBP. Adventitious breath sounds and ineffective cough were the most prevalent defining characteristics. Adventitious breath sounds increased by 80% the chance for IAC. Among the related factors, the highlights were: secretions in the bronchi and retained secretions. Secretions in the bronchi favored in 80% the occurrence of IBP and in 60% of IAC. This verified a statistically significant association between the defining characteristics and related factors of the diagnoses studied. Conclusion: The four diagnoses were identified in children with acute respiratory infections, with different prevalences. Métodos: Estudo de abordagem quantitativa, transversal, com 151 crianças. Os dados foram coletados por meio de entrevista e de avaliação pulmonar. Para análise dos dados, foi utilizada a estatística descritiva e inferencial. Resultados: O diagnóstico mais prevalente foi PRI. Ruídos adventícios respiratórios e tosse ineficaz foram às características definidoras mais prevalentes. Ruídos adventícios respiratórios aumentam em 80% a chance para DIVA. Entre os fatores relacionados, destacaram-se: secreções nos brônquios e secreções retidas. Secreções nos brônquios favorecem em 80% a ocorrência de PRI e em 60% de DIVA. Verificou-se associação estatisticamente significante entre as características e fatores relacionados com os diagnósticos estudados. Conclusão: Os quatro diagnósticos foram identificados em crianças com infecções respiratórias agudas, apresentando prevalências distintas. Objectives: to verify the association between knowledge and adherence to foot self-care practices performed by patients with diabetes mellitus type 2. Methods: cross-sectional, descriptive study carried out with 197 patients in basic health units located in the Northeast region of Brazil. For data collection, we used a semi-structured questionnaire that addressed issues inherent to knowledge and Diabetes Self-Care Activities. Results: we observed that patients with moderate knowledge about self-care practices were more likely to perform foot self-examination, dry the interdigital spaces, moisturize their feet with creams and oils, observe the presence of mycosis and ingrown toenail when compared to patients with insufficient knowledge. Conclusions: the patients' level of knowledge was closely related to the self-care activities carried out, which reinforces the importance of nurses working on training those on essential health care.
Measures of accuracy can contribute to an accurate diagnostic inference process, improving patient assessment and facilitating rapid, accurate diagnosis.
PURPOSETo verify the relevance of the clinical indicators, the clarity and precision of the conceptual, and operational definitions for Ineffective breathing pattern (IBP).METHODSA content analysis by 39 judges.FINDINGSThe results showed 28 clinical indicators for IBP. However, only seven were not considered relevant for the diagnosis. These are not listed in NANDA International taxonomy. All conceptual and operational definitions were adequate, according to the analysis of the judges.CONCLUSIONThe list of 28 clinical indicators of IBP was submitted for analysis by judges, which then resulted in the validation of 21 of these elements.IMPLICATIONS FOR NURSING PRACTICEThis study clarifies that gaps in the structure of diagnoses, helping nurses' diagnostic reasoning process in clinical practice.
RESUMO A pesquisa objetivou analisar as evidências científicas disponíveis sobre controle e vigilância epidemiológica de contatos de hanseníase. Trata-se de uma revisão integrativa norteada pela pergunta: qual a produção científica sobre controle e vigilância epidemiológica de contatos de hanseníase? Incluíram-se estudos primários, publicados em inglês, português ou espanhol nas bases de dados PubMed, Lilacs e Cinahl, entre 2008 e 2018, disponíveis na íntegra. A amostra foi composta por 19 estudos. Identificaram-se três categorias temáticas: fatores de risco para infecção e adoecimento entre os contatos de hanseníase, vigilância ativa de contatos de hanseníase e novas estratégias de controle e vigilância de contatos de hanseníase. Os resultados mostraram estudos voltados à prevenção e controle da hanseníase entre os contatos, entretanto, as evidências apontam novas estratégias que podem ser incorporadas à prática clínica visando à eliminação da hanseníase como problema de saúde pública no País.
