Objective: to evaluate the accuracy and to determine the prevalence of the defining characteristics of the nursing diagnosis Impaired gas exchange in children with respiratory diseases. Methods: cross-sectional study carried out with 236 children who were in the initial 48 hours of hospital admission. A questionnaire was used with sociodemographic data and physical examination of the respiratory system to identify the defining characteristics of the studied diagnosis. For the analysis of the accuracy, the sensitivity and specificity measures were calculated. Results: the diagnosis Impaired gas exchange was present in 21.6% of the sample and the most prevalent defining characteristics were dyspnea and tachycardia. Regarding the accuracy measures, the characteristics that presented high sensitivity values were dyspnea and abnormal respiratory pattern, while cyanosis, hypoxemia, and restlessness obtained high values of specificity. Conclusion: the characteristics that stood out both for the accuracy measures and for the prevalence were dyspnea and abnormal respiratory pattern. Objetivo: avaliar a acurácia e determinar a prevalência das características definidoras do diagnóstico de enfermagem Troca de gases prejudicada em crianças com doenças respiratórias. Métodos: estudo transversal realizado com 236 crianças que estavam nas 48 horas iniciais de internação hospitalar. Utilizou-se questionário com dados sociodemográficos e exame físico do sistema respiratório para identificação das características definidoras do diagnóstico estudado. Para análise da acurácia foram calculadas as medidas de sensibilidade e especificidade. Resultados: o diagnóstico Troca de gases prejudicada esteve presente em 21,6% da amostra e as características definidoras mais prevalentes foram Dispneia e Taquicardia. No que se refere às medidas de acurácia, as características que apresentaram altos valores de sensibilidade foram Dispneia e Padrão Respiratório anormal, enquanto Cianose, Hipoxemia e Inquietação obtiveram elevados valores de especificidade. Conclusão: as características que se destacaram tanto pelas medidas de acurácia quanto pela prevalência foram Dispneia e Padrão respiratório anormal. Objectives: To elaborate operational defi nitions for the Nomenclature nursing diagnoses, interventionsand outcomes of the Pediatric Clinic at a University Hospital and carry out a validation of the content and clinical aspects of this Nomenclature. Method: Methodological research, developed in two stages: documentary study, for the validation of content of the operational defi nitions of nursing diagnoses; and secondly applied research, via clinical cases, performed with children from 0 to 5 years. Results: The 126 diagnoses were submitted to consensus validation and 55.6% were validated. Six clinical case studies were carried out, using the phases of the nursing process, based on the theory of Basic Human Needs, identifying 24.3% of the validated diagnoses and 54.5% of the nursing interventions listed in the Nomenclature. Conclusion: The study showed the effectiveness of using the Nomenclature in the Pediatric Clinic, in order to optimize the care andquality of health services.

AimHypoxic Ischemic Encephalopathy (HIE) is one of the principal causes of neonatal mortality and long‐term morbidity worldwide. The neonatal signs of mild cerebral injury are subtle, making an early precise diagnosis difficult. Delayed detection, poor prognosis, and lack of specific biomarkers for the disease are increasing mortality rates. In this study, we intended to identify specific biomarkers using comparative proteomic analysis to predict the severity of perinatal asphyxia so that its outcome can also be prevented.Experimental DesignA case‐control study was conducted on 38 neonates, and urine samples were collected within 24 and 72 h of life. A tandem mass spectrometry‐based quantitative proteomics approach, followed by validation via sandwich ELISA, was performed.ResultsThe LC‐MS/MS‐based proteomics analysis resulted in the identification of 1201 proteins in urine, with 229, 244, and 426 being differentially expressed in HIE‐1, HIE‐2, and HIE‐3, respectively. Axon guidance, Diseases of programmed cell death, and Detoxification of reactive oxygen species pathways were significantly enriched in mild HIE versus severe HIE. Among the differentially expressed proteins in various stages of HIE, we chose to validate four proteins – APP, AGT, FABP1, and FN1 – via sandwich ELISA. Individual and cumulative ROC curves were plotted. AGT and FABP1 together showed high sensitivity, specificity, and accuracy as potential biomarkers for early diagnosis of HIE.ConclusionEstablishing putative urinary biomarkers will facilitate clinicians to more accurately screen neonates for brain injury and monitor the disease progression. Prompt treatment of neonates may reduce mortality and neurodevelopmental impairment.
