Mục tiêu: Đánh giá hiệu quả phẫu thuật nang răng sừng hóa bằng phương pháp cắt nang mài xương và cắt nang nạo hóa học theo phương pháp tổng quan hệ thống. Đối tượng và phương pháp nghiên cứu: Tổng quan hệ thống và phân tích gộp dữ liệu tỉ lệ tái phát sau phẫu thuật của phương pháp cắt nang mài xương và cắt nang nạo hóa học từ các bài báo, luận văn, luận án trên trang cơ sở dữ liệu Pubmed, EBSCOhost Research Databases, ScienceDirect, thư viện trường Đại Học Y Hà Nội dạng tiếng Anh, tiếng Việt. Kết quả: Nghiên cứu được lấy từ 3 cơ sở dữ liệu chính (Pubmed, Science direct, Cochrane..) thu được tổng cộng 331 nghiên cứu (Pubmed: 191, Science direct: 129, Cochrane: 9, văn bản khác: 2), Sau khi loại bỏ các nghiên cứu trùng lặp (13 nghiên cứu), chúng tôi đã sàng lọc được 321 nghiên cứu. Tiến hành đánh giá tiêu đề, tóm tắt nhằm loại bỏ 276 nghiên cứu không phù hợp. 45 nghiên cứu được xem xét toàn văn dựa trên các tiêu chí đưa vào. Tổng cộng 25 nghiên cứu không phù hợp bị loại bỏ bao gồm 9 nghiên cứu tổng quan, 4 nghiên cứu gồm các ca NBCCS không tách biệt, 6 nghiên cứu không đủ thời gian theo dõi, 3 nghiên cứu số ca nhỏ hơn 10, một nghiên cứu không rõ tỷ lệ tái phát và 2 nghiên cứu không sử dụng phương pháp được chọn. 20 nghiên cứu đủ tiêu chuẩn được đưa vào để phân tích gộp với 591 tổn thương nang răng sừng hoá (PO: 122, CS: 511). Kết quả phân tích gộp cho thấy tỉ lệ tái phát sau điều trị của phương pháp cắt nang mài xương trong phân tích gộp của 8 nghiên cứu là 20% (KTC 95%: 10-31), của phương pháp cắt nang nạo hoá học bằng dung dịch Carnoy sau điều trị nang răng sừng hoá là 6% (KTC 95%: 1,0-12,0). Kết luận: Từ kết quả phân tích gộp cho thấy tỉ lệ gộp tái phát của phương pháp nạo hóa học bằng dung dịch Carnoy cho tỉ lệ tái phát thấp hơn, là phương pháp nên được sử dụng để tăng hiệu quả điều trị cho người bệnh.
Mục tiêu: Nghiên cứu này nhằm đánh giá chất lượng cuộc sống ở người mang phục hình răng dựa trên bộ công cụ WHOQoL-Bref. Đối tượng và phương pháp nghiên cứu: Nghiên cứu mô tả cắt ngang được thực hiện với 132 người mang phục hình răng tại tỉnh Nam Định năm 2019-2021. Chọn mẫu thuận tiện tại 03 vùng miền của tỉnh Nam Định. Thông tin được thu thập bằng bộ câu hỏi, thông qua hình thức hỏi trực tiếp. Kết quả: Nghiên cứu này cho thấy chất lượng cuộc sống trung bình ở người mang phục hình răng là 42,58 và hệ số Cronback's Alpha là 0,89. Kết luận: Bộ công cụ WHOQol-Bref phù hợp với nghiên cứu để đánh giá chất lượng cuộc sống của người mang phục hình răng tại tỉnh Nam Định.
Mục đích là mô tả kết quả điều trị của các phương pháp phẫu thuật và nhận xét một số yếu tố ảnh hưởng đến kết quả điều trị u xơ xương hóa xương hàm. Một cuộc tìm kiếm đã được thực hiện vào tháng 8 năm 2021. Tiêu chí đủ điều kiện bao gồm các bài báo phải có chẩn đoán bằng mô bệnh học, phương pháp phẫu thuật, thời gian theo dõi và kết quả sau phẫu thuật. Tổng cộng có 40 bài báo và 123 trường hợp. Tỷ lệ tái phát sau phẫu cao nhất ở phẫu thuật nạo là 11,5% với thời gian tái phát là 9,3 năm, phẫu thuật triệt để là 9% với thời gian tái phát là 2,7 năm, phẫu thuật bóc tách là 6,25% với thời gian tái phát là 1 năm và không có sự tái phát ở phẫu thuật kết hợp bóc tách và nạo. Tỷ lệ tái phát sau phẫu thuật ở Nam cao hơn ở nữ, hàm trên cao hơn hàm dưới. Tái phát sau phẫu thuật nhiều nhất ở độ tuổi từ 13 đến 18 tuổi, những khối u có diễn biến từ 1 đến 5 năm, kích thước khối u từ 2 cm đến 4 cm, khối u có đậm độ cản quang và có ranh giới không rõ ràng. Tỷ lệ tái phát sau phẫu thuật còn cao nhất ở những khối u làm xương vỏ mở rộng. Kết luận: Phẫu thuật bóc tách, phẫu thuật bóc tách kết hợp nạo nên được ưu tiên trong điều trị. Để tăng khả năng thành công sau khi phẫu thuật cần phải lưu ý đến các yếu tố ảnh hưởng đến kết quả phẫu thuật.
Mục tiêu: Nhằm mô tả các đặc điểm nhân khẩu học và một số đặc điểm về cân nặng, răng miệng hay những khó khăn mà trẻ mắc dị tật khe hở môi và/hoặc vòm miệng thường gặp phải. Phương pháp nghiên cứu: Nghiên cứu mô tả cắt ngang trên 196 trẻ dưới 15 tuổi có dị tật ke hở môi và/hoặc vòm miệng đến khám và điều trị tại Bệnh viện Răng Hàm Mặt Trung ương Hà Nội. Kết quả: Tỷ lệ trẻ nam mắc dị tật (64,8%) lớn hơn nữ (35,2%). Phần lớn trẻ sinh ra trong gia đình có địa vị kinh tế và học vấn thấp hoặc trung bình, người cha có thói quen sử dụng rượu và thuốc lá. Khoảng 60% trẻ gặp khó khăn khi ăn và bú, tiếp đến là vấn đề về nói, giao tiếp và bệnh nhiễm trùng. Phần lớn trẻ có cân nặng bình thường nhưng tỷ lệ thiếu cân tương đối cao, chiếm 12,8%. Nhìn chung, trẻ thường gặp các vấn đề về răng miệng, trong đó được báo cáo nhiều nhất là thiếu chỗ mọc răng (45,9%), bất thường về vị trí (38,8%) và chậm mọc răng (25,5%), trong khi hơn 16% cha mẹ không biết về các vấn đề răng miệng. Kết luận: Tỷ lệ trẻ nam mắc dị tật nhiều hơn trẻ nữ. Tỷ lệ trẻ mắc dị tật khe hở môi hoặc vòm miệng đơn thuần nhiều gấp hơn 2 lần tỷ lệ trẻ mắc đồng thời hai dị tật này. Cha mẹ của trẻ có trình độ học vấn và kinh tế ở mức thấp hoặc trung bình. Tỷ lệ người mẹ hút thuốc lá thụ động khá cao. Khó khăn khi cho ăn, khi bú và vấn đề răng miệng thường gặp nhất và cần sự quan tâm của cha mẹ.
Mục tiêu: Nhằm đánh giá kết quả điều trị và phục hồi chức năng sau phẫu thuật của trẻ mắc dị tật khe hở môi và/hoặc vòm miệng. Phương pháp nghiên cứu: Tiến hành nghiên cứu mô tả cắt ngang trên 196 trẻ dưới 15 tuổi có dị tật ke hở môi và/hoặc vòm miệng đến khám và điều trị tại Bệnh viện Răng Hàm Mặt Trung ương Hà Nội năm 2019-2021. Kết quả: Tỷ lệ xuất hiện biến chứng sớm sau mổ 1 tuần khá cao (58,7%), trong đó tụ máu, bầm tím thường gặp nhất (83,5%). Khả năng nhai được cải thiện rõ rệt nhất sau phẫu thuật và đạt trên 85% sau 6 tháng, tiếp đến là khả năng phát âm. Tỷ lệ sẹo co kéo nhẹ và sẹo to mất thẩm mỹ sau 1 tháng và 6 tháng mổ vẫn chiếm đa số. Trẻ có biểu hiện, thay đổi tích cực để hòa nhập với cộng đồng, xã hội. Kết luận: Nghiên cứu cho thấy tác động tích cực của phẫu thuật trong cải thiện sức khỏe thể chất, tâm thần và chức năng của trẻ mắc dị tật khe hở môi và/hoặc vòm miệng mặc dù tỷ lệ biến chứng sớm sau phẫu thuật khá cao. Tuy nhiên trẻ vẫn phải đối mặt với một số rào cản để hòa nhập, thích nghi với cộng đồng.
