Background The median duration of hospital stays due to COVID-19 has been reported in several studies on China as 10−13 days. Global studies have indicated that the length of hospitalisation depends on different factors, such as the time elapsed from exposure to symptom onset, and from symptom onset to hospital admission, as well as specificities of the country under study. The goal of this paper is to identify factors associated with the median duration of hospital stays of COVID-19 patients during the second COVID-19 wave that hit Vietnam from 5 March to 8 April 2020. Method We used retrospective data on 133 hospitalised patients with COVID-19 recorded over at least two weeks during the study period. The Cox proportional-hazards regression model was applied to determine the potential risk factors associated with length of hospital stay. Results There were 65 (48.9%) females, 98 (73.7%) patients 48 years old or younger, 15 (11.3%) persons with comorbidities, 21 (16.0%) severely ill patients and 5 (3.8%) individuals with life-threatening conditions. Eighty-two (61.7%) patients were discharged after testing negative for the SARS-CoV-2 virus, 51 were still in the hospital at the end of the study period and none died. The median duration of stay in a hospital was 21 (IQR: 16–34) days. The multivariable Cox regression model showed that age, residence and sources of contamination were significantly associated with longer duration of hospitalisation. Conclusion A close look at how long COVID-19 patients stayed in the hospital could provide an overview of their treatment process in Vietnam, and support the country's National Steering Committee on COVID-19 Prevention and Control in the efficient allocation of resources over the next stages of the COVID-19 prevention period.
Background Methamphetamine use could jeopardize the current efforts to address opioid use disorder and HIV infection. Evidence-based behavioral interventions (EBI) are effective in reducing methamphetamine use. However, evidence on optimal combinations of EBI is limited. This protocol presents a type-1 effectiveness-implementation hybrid design to evaluate the effectiveness, cost-effectiveness of adaptive methamphetamine use interventions, and their implementation barriers in Vietnam. Method Design: Participants will be first randomized into two frontline interventions for 12 weeks. They will then be placed or randomized to three adaptive strategies for another 12 weeks. An economic evaluation and an ethnographic evaluation will be conducted alongside the interventions. Participants: We will recruit 600 participants in 20 methadone clinics. Eligibility criteria: (1) age 16+; (2) Alcohol, Smoking and Substance Involvement Screening Test (ASSIST) scores ≥ 10 for methamphetamine use or confirmed methamphetamine use with urine drug screening; (3) willing to provide three pieces of contact information; and (4) having a cell phone. Outcomes: Outcomes are measured at 13, 26, and 49 weeks and throughout the interventions. Primary outcomes include the (1) increase in HIV viral suppression, (2) reduction in HIV risk behaviors, and (3) reduction in methamphetamine use. COVID-19 response: We developed a response plan for interruptions caused by COVID-19 lockdowns to ensure data quality and intervention fidelity. Discussion This study will provide important evidence for scale-up of EBIs for methamphetamine use among methadone patients in limited-resource settings. As the EBIs will be delivered by methadone providers, they can be readily implemented if the trial demonstrates effectiveness and cost-effectiveness. Nghiên cứu mô tả cắt ngang về đặc điểm bệnh sởi tại Hà Nội năm 2018 nhằm tìm hiểu một số yếu tố liên quan với mắc sởi để giúp cho công tác phòng chống dịch tại Hà Nội trong những năm tiếp theo. Phân tích 571 trường hợp mắc sởi xác định, và 148 trường hợp sốt phát ban không phải là sởi, kết quả cho thấy các yếu tố làm tăng khả năng mắc sởi bao gồm các đối tượng không tiêm vắc xin phòng sởi (OR hiệu chỉnh: 4,7; (2,7-7,2)); tiêm chủng không đầy đủ (OR hiệu chỉnh=3,7 (1,7-8,1)), tiếp xúc với trường hợp sốt phát ban nghi sởi trước đó (OR hiệu chỉnh: 2,1 (1,2 -3,8)), có tiền sử nhập viện trước đó (OR hiệu chỉnh: 6,5 (1,3-31,9)) và sinh sống tại khu vực nội thành (OR hiệu chỉnh: 2,1 (1,01 – 4,6)). Để làm giảm nguy cơ mắc sởi cần phải tăng tỉ lệ tiêm chủng đầy đủ vắc xin sởi, phát hiện sớm các trường hợp nghi bệnh nhằm cách ly triệt để và đặc biệt phải kiểm soát tốt lây chéo trong bệnh viện.
Non - communicable diseases (NCDs) are the leading causes of death and disability globally. In Vietnam, NCDs account for 77% of all deaths. The purpose of this systematic review is to describe the patterns of mortality associated with NCDs in Vietnam from 2006 to 2016. We included 25 studies that reported on health impacts especially on mortality NCDs in Vietnam. From 2008 to 2012, NCDs deaths decreased by 2% (75% vs 73%) and from 2012 to 2016, it increased by 4% (73% vs 77%). The probability of premature deaths of NCDs in 2010, 2012 and 2016 were at 17.5%, 17.4% and 17.0%, respectively. Though the mortality patterns seem to decline over the years, it is not significant enough to conclude, and the probability of dying prematurely is still very high in the country. The percentage of deaths for male was 26.4% (2008), 54.3% (2012), 23.0% (2016) higher than those for female 19.4% (2008), 30.0% (2012), 11.0% (2016). Worldwide, 41 millions people die from non-communicable diseases each year with 15 millions people die between 30 - 69. Cardiovascular diseases remained the main leading cause of NCDs deaths, cancer was the second leading cause of NCDs deaths and projected to increase further in subsequent reports.
Hành vi quan hệ tình dục không an toàn là một trong những hành vi nguy cơ cao có thể dẫn tới lây truyền HIV/AIDS đặc biệt ở các quần thể có nguy cơ cao. Nghiên cứu mô tả cắt ngang tiến hành trên 318 đối tượng tiêm chích ma tuý tại thành phố Thái Nguyên nhằm mô tả thực trạng sử dụng bao cao su trong hoạt động quan hệ tình dục và một số yếu tố liên quan đến hành vi không sử dụng bao cao su ở nhóm đối tượng này. Trong lần quan hệ tình dục gần đây nhất tỷ lệ không sử dụng bao cao su chiếm tỷ lệ cao (59,7%). Chỉ có 14,6% tỷ lệ người sử dụng bao cao su khi quan hệ tình dục với vợ/người yêu ở tất cả các lần. Những người chưa lập gia đình có nguy cơ không sử dụng bao cao su khi quan hệ tình dục thấp hơn 60% so với những người có gia đình (KTC 95% 0,22 - 0,76). Những người tiêm chích ma tuý được điều trị ARV và khám STI trong vòng 3 tháng qua có nguy cơ không sử dụng bao cao su khi quan hệ tình dục thấp hơn. Những người tiêm chích mà túy là quần thể có nguy cơ cao nhiễm HIV, đặc biệt những người có hành vi dùng chung bơm kim tiêm. Nghiên cứu này được thực hiện nhằm mô tả đặc điểm nhóm đối tượng tiêm chích ma túy và một số yếu tố liên quan đến tình trạng sử dụng chung bơm kim tiêm tại Thái Nguyên năm 2019. Nghiên cứu mô tả cắt ngang trên 318 đối tượng tiêm chích ma túy (TCMT) tại thành phố Thái Nguyên, tỉnh Thái Nguyên từ tháng 04/2019 đến tháng 06/2020. Kết quả nghiên cứu cho thấy tỷ lệ đối tượng tham gia chủ yếu là nam giới (99,7%), tập trung chủ yếu ở nhóm tuổi từ 35 - 49 tuổi; đa số đối tượng có trình độ văn hóa từ trung học cơ sở trở lên. Loại ma túy được sử dụng nhiều nhất là heroin (96,2%), 310 người tiêm chích ma tuý có thời gian sử dụng ma túy trên 3 năm. Tỷ lệ người tiêm chích ma túy đã từng dùng chung bơm kim tiêm tại Thái Nguyên khá cao (43,4%). Các yếu tố tình trạng hôn nhân, xét nghiệm HIV, điều trị ARV, nhận được bơm kim tiêm sạch có liên quan tới hành vi dùng chung bơm kim tiêm của những người tiêm chích ma tuý. Do đó cần tăng cường cung cấp và tiếp cận dịch vụ chăm sóc sức khỏe cho các nhóm tiêm chích ma tuý nói riêng và các đối tượng nguy cơ cao nói chung.
Nghiên cứu nhằm mô tả thực trạng các vấn đề sức khoẻ có can thiệp phục hồi chức năng (PHCN) tại các bệnh viện tuyến tỉnh và tuyến huyện tại 7 tỉnh đaị diện cho 7 vùng sinh thái trên cả nước. Kết quả cho thấy, tại 26 bệnh viện nghiên cứu, tỷ lệ các vấn đề sức khoẻ có can thiệp PHCN ở nhóm khuyết tật về vận động có tổng số ca được báo cáo nhiều nhất (khoảng 70%), đặc biệt là các vấn đề về cơ xương khớp là 16,705 ca, các vấn đề về cột sống là11,655 catheo thống kê trong năm 2020, chiếm tỷ lệ thấp nhất là các vấn đề sức khoẻ có can thiệp PHCN ở nhóm giảm cảm giác với 1,539 ca (2,6%) năm 2020.Mô hình bệnh tật các vấn đề sức khoẻ có can thiệp PHCN ngày càng thay đổi và đa dạng hơn qua các năm, trong đó các vấn đề về cơ xương khớp và tự kỉ ở trẻ em có xu hướng ngày càng giảm,các vấn đề về cột sống và tai biến mạch máu não ngày càng tăng. Vì vậy, việc hiểu rõ mô hình các vấn đề sức khoẻ có can thiệp PHCN là cần thiết để nâng cao chất lượng khám chữa bệnh, can thiệp PHCN cũng như định hướng đào tạo chuyên ngành PHCN trong tương lai.
