L'absentéisme des personnels soignants des établissements de santé français est un phénomène en croissance et au niveau très élevé relativement aux chiffres du secteur industriel ou de l'ensemble du champ de la fonction publique. Les recherches sur l'absentéisme en général et sur l'absentéisme hospitalier en particulier ont en commun de montrer la part significative des facteurs exogènes pour expliquer le phénomène. Or, le secteur hospitalier a connu ces dernières années de multiples réformes qui visaient à le transformer en profondeur. Dans ce contexte, les établissements ont modifié leurs organisations du travail, et ont conçu de nouveaux leviers pour gérer les ressources hospitalières. L'objet de cet article est de montrer, à travers les résultats d'une recherche-intervention réalisée dans dix établissements de santé, les possibles impacts des réformes hospitalières sur l'accroissement de l'absentéisme des personnels soignants dans les établissements de santé.
Confrontés à un absentéisme important des soignants, les directions et l’encadrement des Etablissements d’Hébergement pour Personnes Agées Dépendantes (EHPAD) proposent souvent des modalités de gestion en décalage avec les réalités vécues par les soignants allant même jusqu’à entretenir le phénomène. Ces modalités perturbent fortement le fonctionnement des équipes qui apportent alors des solutions d’organisation temporaires pour faire face à l’augmentation de la charge de travail. Pour comprendre les phénomènes en jeu dans la gestion de l’absentéisme, nous mobilisons la théorie de la régulation sociale [1]. La méthodologie employée est qualitative et s’appuie sur trois études de cas menées au sein d’EHPAD. Il ressort de l’analyse que les directions et l’encadrement se cantonnent à des règles de contrôle très éloignées des règles autonomes développées par les soignants. Ces régulations autonomes, loin d’être des solutions pérennes se révèlent très instables, variables et temporaires. Au final, cette recherche questionne la capacité du management à créer des opportunités de rencontre entre régulations autonomes et régulations de contrôle.
