High-intensity PRMST is as safe and well tolerated as a similarly structured low-resistance exercise regimen for very frail elderly patients, but produces greater muscle strength improvements. The addition of testosterone leads to greater muscle size and a trend toward greater strength but did not produce a synergistic interaction with exercise. Neither intervention had a significant effect on functional performance.
For older recuperative care patients, prealbumin and its change during hospitalization are positively associated with protein intake, but inflammation or changes in inflammation appear to exert a much more-powerful influence on prealbumin concentration. Given the potential confounding effects of inflammation, monitoring the change in prealbumin is not an adequate substitute for a more-detailed nutritional assessment in this population.
Many nutritionally compromised elderly nursing home residents experience significant bidirectional fluctuations in their weight from month to month. How much of this variability is due to measurement error is not known. However, a weight loss of > or =5% in any month has important prognostic implications.
The diagnosis of and intervention in under-nutrition in nursing home patients is frequently disorganized. In addition, nutritional assessment, either for screening or for following intervention, is hampered by the lack of convenient and unambiguous assessment tools. OS use is associated with weight gain in many nursing home patients and also improves other nutritional parameters in selected individuals.
Most after-school physical activity programs for youth focus on aerobic games and activities. The purpose of this study was to evaluate the efficacy of an after-school resistance training program on improving the physical fitness of middle school-age boys. 22 boys (M = 13.9 yr., SD = .4 yr.) participated in a periodized, multiple-set, 9-wk. (2x/week) resistance training program. All subjects were pre- and post-tested on their 10-repetition maximum squat, 10-repetition maximum bench press, vertical jump, medicine ball toss, flexibility, and also percentage of body fat and the progressive aerobic cardiovascular endurance run (PACER). Statistical analysis indicated that subjects significantly improved performance on the squat (19%), bench press (15%), flexibility (10%), vertical jump (5%), medicine ball toss (12%), and the PACER (36%). Although this design minus a control group limits interpretation, this after-school resistance-training program can improve muscular fitness and cardiovascular fitness in boys and should be replicated with appropriate experimental controls.
Among elderly patients admitted to a TCU, inflammation appears to be a more powerful determinant of albumin and its change during the hospitalization than is nutrient intake. Further study is needed to prove causality and to determine whether the relative importance of inflammation on the albumin concentration diminishes with more prolonged periods of observation.
