RA patients with long-term exposure to TNF-blocking agents were at a greater risk of developing clonal expansions of LGLs. This finding may prompt clinicians to refrain from using these substances in RA patients with known T cell aberrations.
Zusammenfassung
Blutbildver?nderungen sind h?ufige Komplikationen bei Patienten mit Rheumatoider Arthritis (RA). Eine seltene, aber relevante Differenzialdiagnose vor allem anhaltender Neutropenien sind niedrig maligne T-LGL-Leuk?mien. Nachfolgend sollen Diagnose und Therapie dieser Erkrankung sowie m?gliche Zusammenh?nge mit dem bereits 1924 beschriebenen Felty-Syndrom beleuchtet werden. Zur Diagnose sind neben der Anamnese und bildgebenden Verfahren eine antik?rpergest?tzte Durchflusszytometrie und eventuell molekularbiologische Verfahren notwendig. Therapeutisch stehen diverse konventionelle immunsuppressive Basistherapeutika zur Verf?gung, von denen MTX in niedriger Dosis der Goldstandard ist. Die Rolle der Biologika ist hingegen noch nicht ausreichend erforscht.
