Prevalence and Characteristics of Persistent Clonal T Cell Large Granular Lymphocyte Expansions in Rheumatoid Arthritis

Abstract: RA patients with long-term exposure to TNF-blocking agents were at a greater risk of developing clonal expansions of LGLs. This finding may prompt clinicians to refrain from using these substances in RA patients with known T cell aberrations.

“…In our study, the number of LGL cells was lower than the usual threshold used to define LGL leukaemia (≥500/mm 3 ) in 4/8 patients. However, LGLe represented ≥10% of total lymphocytes, a threshold recently proposed in RA 12. In fact, these four patients had a global lymphopenia (<1000/mm 3 ) due to disease activity and/or associated treatments.…”

“…Surprisingly, Schwaneck et al did not identify RTX as a risk factor for LGLe in RA 12. Also, RTX has been used in the treatment of LGL leukaemia associated with RA, sometimes with success 13.…”

“…Thus, in one study, 3.6% of patients with RA had >10% LGL (CD3 + CD56 + ) in the PB. These patients were not clinically distinct, other than developing cytopenias, but significantly more were under anti TNFα therapy and~60% of the LGL expansions were γδ T cell clones [29]. Furthermore, among 14 patients with γδ T cell-LGL leukemia, (11 men and three women), six had a history of RA.…”
