Myasthenia gravis (MG) is an autoimmune disorder characterized by a defect in synaptic transmission at the neuromuscular junction causing fluctuating muscle weakness with a decremental response to repetitive nerve stimulation or altered jitter in single-fiber electromyography (EMG). Approximately 80% of all myasthenia gravis patients have autoantibodies against the nicotinic acetylcholine receptor in their serum. Autoantibodies against the tyrosine kinase muscle-specific kinase (MuSK) are responsible for 5-10% of all myasthenia gravis cases. The autoimmune target in the remaining cases is unknown. Recently, low-density lipoprotein receptor-related protein (LRP4) has been identified as the agrin receptor. LRP4 interacts with agrin, and the binding of agrin activates MuSK, which leads to the formation of most if not all postsynaptic specializations, including aggregates containing acetylcholine receptors (AChRs) in the junctional plasma membrane. In the present study we tested if autoantibodies against LRP4 are detectable in patients with myasthenia gravis. To this end we analyzed 13 sera from patients with generalized myasthenia gravis but without antibodies against AChR or MuSK. The results showed that 12 out of 13 antisera from double-seronegative MG patients bound to proteins concentrated at the neuromuscular junction of adult mouse skeletal muscle and that approximately 50% of the tested sera specifically bound to HEK293 cells transfected with human LRP4. Moreover, 4 out of these 13 sera inhibited agrin-induced aggregation of AChRs in cultured myotubes by more than 50%, suggesting a pathogenic role regarding the dysfunction of the neuromuscular endplate. These results indicate that LRP4 is a novel target for autoantibodies and is a diagnostic marker in seronegative MG patients.

We obtained data on lecture practices from 100 of the 110 university-affiliated anesthesiology residency programs certified in the United States in 1988. Of these residency programs, 36% had a majority of their lectures before the operating room schedule began, 57% had no lectures at all in this early time slot, and 78% had morning lectures at least once a week in conjunction with a delayed operating room start. Seventy-one percent of programs had one or more afternoon lectures each week. An attendance of more than 80% was reported in 66% of the programs for morning lectures and in 50% of the programs for afternoon lectures, which is a significant difference. Aggregate pass rates on the American Board of Anesthesiology written examinations in 1987 and 1988 correlated significantly with morning-lecture attendance, but not with afternoon-lecture attendance, number of lecture days per week, or mandatory lecture attendance. These findings suggest the need for further study and definition of the role of lectures in resident education in anesthesiology.
Einleitung Patientensicherheit ist ein hohes Gut und verpflichtet in der stationären und ambulanten Behandlung zur Beachtung und Umsetzung aller erforderlichen, dem aktuellen wissenschaftlichen Stand der Medizin entsprechenden Präventionsmaßnahmen zum Schutz vor nosokomialen Infektionen. In Deutschland sind mit dem Infektionsschutzgesetz auf Bundesebene Rechtsgrundlagen zur Vermeidung und Kontrolle von im Krankenhaus erworbenen Infektionen geschaffen worden, deren Umsetzung auf Län-derebene festgelegt und auf kommunaler Ebene kontrolliert wird. Die Durchführung der gesetzlich geforderten Kontrollen bei nosokomialen Infektionen obliegt in Deutschland dem öffentlichen Gesundheitsdienst. Mit diesem Beitrag soll anhand von Beispielen eine Übersicht über die dezentrale infektionshygienische Überwachung in Deutschland im Vergleich zur zentralen Überwachungsstruktur in den Niederlanden vermittelt werden, und zwar am Beispiel der Krankenhausbegehung unter folgenden Aspekten: █ zu berücksichtigende Rechtsvorschriften █ Prozesse █ Ergebnisse █ Rechtsfolgen Abschließend werden einige kritische Aspekte des bestehenden Überwachungsverfahrens der Aufsichtsbehörden angesichts der zunehmenden Infektionen mit antibiotikaresistenten Erregern gesondert aufgezeigt. Rahmenbedingungen Das Gesundheitswesen befindet sich im Umbruch. Die demografische Entwicklung mit steigendem Anteil äl-terer, multimorbider Patienten sowie kürzere Liegezeiten und Personalmangel erhöhen das Risiko einer Infektion im Krankenhaus. Die Deutsche Krankenhausgesellschaft berichtet 2009 von einer durchschnittlichen Verweildauer von 7,5 Tagen in einem allgemeinen Krankenhaus [1]. Geringere Personalressourcen bei gleichzeitig kürzeren Betreuungszeiträumen bedingen häufigere Aufnahmen und Entlassungen. An der Schnittstelle zum ambulanten Behandlungssektor spielt das Entlassungsmanagement im Infektionsschutz eine wichtige Rolle, um z. B. Informationen zu erhalten über im Krankenhaus begonnene Maßnahmen wie die antibiotische Behandlung eines Patienten. Beim Infektionsschutz spielt das Entlassungsmanagement eine wichtige Rolle.
