Este trabalho analisou a primeira experiência de consórcios verticais na saúde, a do Ceará, Brasil, para compreender sua implantação. No campo teórico, o artigo dialoga com os estudos sobre o papel do governo estadual no federalismo e a cooperação intergovernamental na saúde. Realizou-se um estudo de natureza qualitativa mediante entrevistas com gestores vinculados aos consórcios nos níveis municipal, regional e estadual, além de análise documental e bibliográfica. Os resultados apontam que o processo de implantação dos consórcios foi idealizado pelo governo estadual e foi conduzido, no primeiro momento, de maneira centralizada. Uma relação de coordenação se estabelece com a definição do desenho institucional do arranjo. Identificaram-se quatro fatores que favorecem a cooperação vertical no estado: (i) a trajetória da política de saúde do Ceará; (ii) a atuação do governo estadual como ente coordenador do processo de regionalização; (iii) a existência de empreendedores políticos e coesão entre a burocracia; e (iv) a sobreposição entre o desenho institucional dos consórcios e a estrutura decisória do SUS. Os consórcios verticais constituem um passo adiante na superação dos efeitos negativos da descentralização.
O Sistema Único da Saúde foi construído para ser operado através da gestão compartilhada. Todavia, a descentralização pós-CF/88 concentrou-se na municipalização dos serviços, fortaleceu o poder normativo na União e provocou a subvalorização estadual (KEINERT; ROSA; MENEGUZZO, 2006). A cooperação intergovernamental aparece como instrumento capaz de superar esses dilemas. Neste sentido, o objetivo desta pesquisa é analisar o desenho das relações intergovernamentais entre os entes integrantes do Consórcio Público de Saúde na Região do Maciço de Baturité (CPSMB), no estado do Ceará. Este estudo de caso constituiu-se numa investigação de caráter qualitativo, com o auxílio da pesquisa bibliográfica e documental. Foram coletadas informações junto a atores-chave do processo de gestão do CPSMB, por meio de entrevistas semiestruturadas. Foram identificadas três motivações para criação e manutenção do Consórcio: o princípio da integralidade pregado pelo SUS; a pressão social exercida nos municípios por serviços de saúde satisfatórios e a carência de recursos enfrentada pelos municípios. Constatou-se que a formatação das relações intergovernamentais não se constitui em um único desenho, mas se diferencia entre a Dimensão Gerencial e a Dimensão Política. A primeira envolve questões operacionais e tem a Diretoria do Consórcio e seus equipamentos de saúde como eixo-central. A segunda abrange questões estratégicas e tem o governo estadual como centro de influência. Por fim, os municípios evoluíram na ideia de ação cooperada, mas ainda necessitam da ação direta de um motivador da ação coletiva. Portanto, faz-se necessário considerar a construção de uma rede híbrida de cooperação, com abertura para a sociedade civil organizada e iniciativa privada.
