Цель: изучить физическую активность детей школьного возраста и ее влияние на их физическое развитие и адаптационные возможности сердечно сосудистой системы. Материалы и методы. Обследовано 290 детей в возрасте 10-17 лет-учеников 5-11 х классов двух школ г. Харькова; мальчиков было 141, девочек-149. Проводилась антропометрия, изучение физической активности с помощью анкеты МАОФА, оценка толерантности к физической нагрузке по пробе Руфье. Результаты. Установлено, что дисгармоничное физическое развитие наблюдалось у 24,18% детей 11-16 лет, преимущественно за счет избыточной массы тела (у 10,25%) и недостатка массы тела (у 9,84%). Ожирение выявлено у 4,10% школьников. Низкая масса тела, избыточная масса тела и ожирение отмечались преимущественно у детей в 11-12 лет независимо от пола. У мальчиков возраст от 14 до 16 лет также характеризуется увеличением числа лиц с избыточной массой тела и ожирением. В целом 58,3±2,9% школьников имели низкую физическую активность в течение недели. За период от 11 до 17 лет (т.е. с 5 по 11 класс) с 45% до 88% возрастает число мальчиков и девочек с гиподинамией. Низкая физическая активность сопровождалась снижением толерантности к физической нагрузке у детей с разным уровнем физического развития. Большинство мальчиков имели удовлетворительные адаптационные возможности сердца. Снижение толерантности к физическим нагрузкам наблюдалось у 20,83% мальчиков с избыточной массой тела и у 28,57% с гармоничным физическим развитием. Среди девочек низкую толерантность к физической нагрузке имели 53,85% с избыточной и 42,37% с нормальной массой тела. Дефицит массы тела у школьников также часто сопровождался снижением адаптационных возможностей: 57,14% мальчиков и 54,55% девочек с низкой массой тела имели сниженную толерантность к физической нагрузке. Неудовлетворительные результаты пробы Руфье в 89% случаев демонстрировали дети с низкой физической активностью. Выводы. Физическая активность и физическое развитие детей влияют на адаптационные возможности растущего организма. Достаточная физическая активность поддерживает удовлетворительные параметры адаптационных возможностей, что наблюдалось у большинства мальчиков с избыточной массой тела. Исследование было выполнено в соответствии с принципами Хельсинской Декларации. Протокол исследования был одобрен Локальным этическим комитетом (ЛЭК) всех участвующих учреждений. Juvenile idiopathic arthritis (JIA) is the term used for descriptions of a group of inflammatory diseases beginning before the age of 16 years. In the last 25-30 years many studies are devoted to the study of cardiovascular risk in adult patients with rheumatoid arthritis, since with the lengthening of the life expectancy of these patients, change in the structure of their mortality. Purpose - to establish the features of morphofunctional characteristics of the heart in adolescents with juvenile idiopathic arthritis on the background of a long (more one year) basic treatment. Materials and methods. Were examined 91 adolescents with polyarticular variant of JIA, of which - 70 girls and 21 boys aged 10 to 18 years old and the duration of the disease is about 80 months. All patients received methotrexate for more than 12 months, the dosing regimen was from 6.51 mg/m2 to 22.72 mg/m2 per week. The control group consisted 45 healthy peers, of which 29 girls and 16 boys. An ultrasound examination of the heart with determination of structural and functional parameters of the left (LV) and right ventricles (RV) of the heart was done for all patients. Results. In patients with a polyarticular JIA, LV functional parameters of the heart were within normal values, but compared with similar indicators of adolescents in the control group showed a decrease in of the end-diastolic size (EDS) and of the end-diastolic volume (EDV) of the LV, a decrease in emission fraction (EF) and stroke volume (SV) of the LV, but normal values of the minute blood volume (MVC) of the LV due to an increase in the heart rate. In patients with JIA, a significant increase in both systolic (the end-systolic size and the end-systolic volume) and diastolic (EDS, EDV) parameters of the RV, thickening of it’s myocardium (TM), increase in SV and the MVC, but decrease in EF. Conclusions. Development morphofunctional disorders of the left and especially the right ventricles of the heart in patients with JIA occurs with long periods of illness and the use of basic therapy, as well as with their delayed appointment and low doses of use. The study was carried out in accordance with the principles of the Declaration of Helsinki. Study Protocol adopted by the local ethics committee of the institution specified in the work. For holding studies obtained the informed consent of parents and children. No conflict of interests was declared by the authors.
Summary. The article is devoted to the study of the state of vascular reactivity, morphofunctional parameters of the heart and adaptive capabilities in adolescents with arterial hypertension. The goal is to study the the state of endothelium-dependent vasodilation and determine the adaptive capabilities of boys with arterial hypertension without metabolic risk factors, depending on the presence of endothelial dysfunction. The study included 22 boys with arterial hypertension aged 13-17 years, 10 of them had endothelial dysfunction on a sample with reactive hyperemia. The study included clinical examination, anthropometry, ECG, EchoCG, functional tests (Ruffier, klinoorthotest). Serum total cholesterol levels were also assessed. Statistical analysis was performed using the program s / n SPSS 17 4a 180844250981. Results. It was found that 58.4% of boys with arterial hypertension had endothelial dysfunction without metabolic risk, which in most cases combined with reduced exercise tolerance and insufficient physical activity. In such adolescents, in comparison with peers with hypertension and normal endothelial function, higher growth, a high frequency of dolichomelia, an increase in the ejection fraction and pressure in the left atrium are noted, indicating an increase in the pumping function of the left ventricle. The frequency of registration of endothelial dysfunction was not substantially correlated with the type of vegetative support for activities in children with arterial hypertension. However, in 60% it was registered against insufficient sympathoadrenal supply - asympathicotonic and hyperdiastolic variants of the klinoorthotest. The presence of endothelial dysfunction is recorded in more than half of adolescents with arterial hypertension. Higher ejection fraction and pressure in the left atrium during systole, which are determined in patients with arterial hypertension and insufficient vasodilation with a decrease in heart rate reflect adaptive changes in the functioning of the myocardium with a deterioration in endothelial function.
