Zusammenfassung. Hintergrund: In Deutschland liegen nur vereinzelte Studien zur PTBS-Prävalenzquote bei Suchtpatienten und zu deren psychischen Befund vor. Fragestellung: Wie hoch ist die relative Häufigkeit einer PTBS bei Patienten mit substanzbezogenen Störungen und wie unterscheiden sich die Patienten mit und ohne PTBS hinsichtlich klinischer und psychosozialer Charakteristika? Methode: Mittels Chi-Quadrat- und t-Tests wurden 376 mittels Propensity Scores gematchte Patienten aus einer Stichprobe von 4105 konsekutiv aufgenommenen Abhängigen in diagnostischen und psychischen Merkmalen retrospektiv miteinander verglichen. Ergebnisse: Die relative Häufigkeit von PTBS lag bei den Patienten mit einer alkoholbezogenen Störung bei 3,8 %, bei den restlichen Suchtpatienten mit Störungen durch andere psychotrope Substanzen bei 10,5 %. Bei den PTBS Patienten lag häufiger eine Persönlichkeitsstörung vor. Außerdem waren die PTBS Patienten stärker psychisch belastet, in ihrem Interaktionsstil abweisender, introvertierter, und nachgiebiger sowie im Bindungsstil vermeidender. Schlussfolgerungen: Die Unterschiede verdeutlichen die Notwendigkeit von auf den Interaktions- und Bindungsstil individuell angepassten Interventionen im Rahmen der Standardbehandlungen für Suchtpatienten mit PTBS.
Zusammenfassung Fragestellung: Inwiefern besteht ein Zusammenhang zwischen psychischer Belastung, Selbstwirksamkeitserwartung und Abstinenz bei station?r behandelten Alkoholabh?ngigen. Methode: Naturalistische Feldstudie, in der retrospektiv mittels bin?rer logistischer Regression und Chi2-Tests Unterschiede in Patienten-Merkmalen zwischen zur Ein-Jahres-Katamnese durchgehend abstinenten (N=537) vs. r?ckf?llig gewordenen Alkoholabh?ngigen (N=410) analysiert wurden. Ergebnisse: Die Quote durchgehend abstinenter Patienten zur Ein-Jahres-Katamnese betrug f?r Patienten mit geringer psychischer Belastung zu Behandlungsbeginn 71%, bei Patienten mit hoher Selbstwirksamkeitserwartung zu Behandlungsende 76%, bei zus?tzlich weniger als 2 vorherigen Entgiftungen 80%. Eine Verbesserung der Psychischen Belastung w?hrend der Behandlung korrelierte nicht mit Abstinenz. Schlussfolgerungen: Die Selbstwirksamkeitserwartung f?r Abstinenz ist bedeutsamer als die bekannten Prognose-Merkmale Arbeits-, Partnersituation und Alter. Die hohe prognostische Valenz der Selbstwirksamkeitserwartung in Kombination mit vorherigen Entgiftungserfahrungen sprechen f?r die Bedeutung suchtspezifischer Interventionen.
Zusammenfassung. Fragestellung: Gestattet die Kombination von Patienten-Merkmalen eine ergebnisorientierte Klassifikation Alkoholabhängiger? Methode: Naturalistische Feldstudie, in der retrospektiv mittels binärer logistischer Regression Unterschiede in Patienten-Merkmalen zwischen zur Ein-Jahres-Katamnese durchgehend abstinenten (N = 431) vs. rückfällig gewordenen Alkoholabhängigen (N = 382) analysiert wurden. Anhand der identifizierten Prognose-Merkmale wurden vier Klassen gebildet. Getrennt für die vier Klassen wurden mittels geschichteter Chi Quadrat Tests weitere Merkmale auf ihre prognostische Valenz geprüft. Ergebnisse: Mit den beiden stärksten Prognosemerkmalen Therapieerfahrung und Depressivität konnten vier Klassen gebildet werden, die sich in ihren Rückfallquoten (30 bis 65 %) signifikant unterschieden. Die Behandlungsdauer korrelierte nur bei den therapieerfahrenen nicht-depressiven Patienten (N = 239) mit späterer Abstinenz. Nur therapieerfahrene, depressive Patienten (N = 50) hatten bei Vorliegen einer Persönlichkeitsstörung höhere Rückfallquoten. Alter, Geschlecht, Schulbildung, Arbeitslosigkeit, Familienstand, Partnersituation und suchtspezifische Komorbiditäten waren für die gesamte Stichprobe und für die vier Klassen prognostisch nicht relevant. Schlussfolgerungen: Die Validität von Prädiktoren ist abhängig von der prozentualen Verteilung der Patientenmerkmale einer Stichprobe, der Therapie, der Stichprobengröße und der Erfolgsquote. Eine ergebnisorientierte Klassifikation gestattet die Ableitung verbesserter therapeutischer Strategien für hoch rückfallgefährdete Patienten. Verbesserte Therapiepläne werden die ergebnisorientierte Klassifikation im Rahmen des lernenden Kliniksystems verändern, so dass nicht von einer Zeitstabilität und Allgemeingültigkeit der klinikspezifischen Prädiktoren ausgegangen werden kann.
