(1) Background: Currently, the scientific evidence on the benefits of assisted therapy with dogs in dementia is not clear. In this study, we want to evaluate such benefits through a randomized controlled clinical trial in multiple centers across the country. (2) Methods: The participants were people over 65 years old with dementia, residing in senior centers in Spain (n = 334). The experimental group underwent assisted therapy with dogs based on the Comprehensive Cognitive Activation Program in Dementia, for 8 months, with weekly sessions of 45 min. Data were collected at the commencement, middle, and end of the program, to evaluate the aspects using the Mini-Examination Cognitive, the modified Bartell Index, the Cornell Scale for Depression in Dementia and the Neuropsychiatric Inventory. (3) Results: The results show significant improvements in the experimental group versus the control group in the affective (T1 = p 0.000; T2 = p 0.000) and behavioral (T1 = p 0.005; T2 = p 0.000) aspects, with the affective aspect displaying greater progress in participants with additional depressive (p = 0.022) or anxiety (p = 0.000) disorders, shorter institutionalization periods (r = −0.222, p = 0.004), and those undergoing complementary psychotherapy (p = 0.033) or alternative therapy (p = 0.011). (4) Conclusions: Dog therapy is effective in improving the affective and behavioral aspects of institutionalized patients with dementia.
ResumenEl objetivo del artículo ha sido explorar, desde la óptica de las propias parturientas, los posibles conflictos surgidos entre las necesidades maternas y las rutinas médicas propias del parto hospitalario e intervencionista. La metodología elegida ha sido la realización de una serie de entrevistas semiestructuradas, cuyos resultados muestran fundamentalmente tres tipos de tensiones asistenciales: la interacción asimétrica, la subordinación tecnológica y la asunción de praxis de "abuso institucional". La conclusión final ha sido que estamos ante un ritual medicalizado impregnado de vivencias de obediencia pasiva, en el que el miedo al peligro siembra la desconfianza corporal, a la vez que la voluntad de la mujer es moldeada por los expertos. Realidad que las parturientas asumen como inevitable, e incluso demandan, como consecuencia de la seguridad que les aporta el control biotecnológico, una vez asumidos los discursos clínicos que representan al parto como un proceso patoló-gico y de riesgo. The chosen methodology has been a set of semi-structured interviews, and the result shows principally three types of relief aid: the asymmetrical interaction, technological subordination and the acceptance of "institutional-abused" praxis. The final conclusion is that we have a medicalized ritual influenced by passive obedience experiences, where the fear creates distrust, while the women wishes are moulded by the expert's. This fact is assumed by the women who have just given birth as inevitable, and they even demand it, as a result of the safety given by the biotechnological control, and once they have assumed the clinical discourses which present the childbirth as a pathological and risk process.
RESUMENEl objetivo del artículo ha sido explorar, a través de un exhaustivo análisis documental, las características y bases antropológicas del parto institucionalizado. Los resultados muestran cómo los nacimientos hospitalarios se han convertido en un ritual clínico cuyo fin es controlar la procreación a través del uso indiscriminado de la tecnología y las nociones tayloristas. Las madres se han transformado en sujetos pasivos de sus propios partos, mientras la injerencia anatómica y el control biométrico de su dimensión corporal se convertían en norma. Se trata, en definitiva, de un auténtico patrón cultural de asistencia, caracterizado por la medicalización, el control biomecánico y la mecanización del proceso, atributos alimentados, a su vez, por toda una serie de ideologías y bases antropológicas de fondo como la biopolítica, el posthumanismo o el productivismo. Se concluye, pues, que la institucionalización de los partos en Occidente se ha debido más a causas culturales e ideológicas que a la supuesta practicidad epidemiológica defendida desde las ciencias médicas.Palabras clave: Parto institucionalizado, Parto hospitalario, Parto medicalizado, Procreación. SUMMARYThe objective of this article was to explore, through a comprehensive documentary analysis, features and anthropological bases of institutionalized delivery. The results show as hospital births have become a clinical ritual whose purpose is to control procreation through the indiscriminate use of technology and taylorist notions. Mothers have become passive subjects of their own births, while the interference anatomic and biometric control of their body became standard. It is, in short, a true cultural attendance pattern characterized by medicalization, biomechanical control and mechanization of the process, attributes fed, in turn, by a range of ideologies and anthropological bases background as biopolitics, the posthumanism or productivism. It is therefore concluded that the institutionalization of deliveries in the West has been due more to cultural and ideological causes than the alleged practicality epidemiological defended from medical science.
Resumen: Desde hace décadas el parto de bajo riesgo ha sido abordado, fundamentalmente en el ámbito clínico-hospitalario, desde una perspectiva intervencionista, caracterizada por la medicalización, tecnificación y mecanización asistencial. En este contexto se sitúa el presente estudio, cuyo objetivo ha sido explorar las limitaciones bioéticas de este paradigma asistencial intervencionista, mediante el análisis de su pragmatismo clínico frente a otros abordajes más humanistas. Para ello se ha llevado a cabo un profundo análisis documental en diferentes bases de datos del ámbito de las ciencias médicas y sociales, seleccionando tanto artículos de difusión científica como libros, declaraciones, estrategias y guías de práctica clínica. Los resultados muestran un patrón asistencial medicalizado, que parece incrementar las intervenciones no justificadas y con ello los efectos iatrogénicos en los partos de bajo riesgo. Se concluye que estamos ante una cultura del nacimiento que, en cierta medida, vulnera los principios bioéticos fundamentales, al subyugar la dignidad de la corporalidad femenina a intereses económicos contingentes e ideologías transhumanistas, biopolíticas y tayloristas cosificadoras. A bioethical debate on interventionist treatments in low risk childbirths Abstract: For decades in low-risk deliveries has been addressed, mainly in the clinical-hospital setting, from an interventionist perspective characterized by medicalization, mechanization and machining care. In this context the present study lies, whose aim was to explore the bioethical limitations of this interventionist paradigm of care through an analysis of its clinical pragmatism compared to other approaches more humanistic. For this it has conducted a thorough analysis of documents in different databases in the field of medical and social sciences, selecting both items of scientific diffusion as books, statements, strategies and clinical practice guidelines. The results show a medicalized care pattern appears to increase the iatrogenic effects in lowrisk deliveries. We conclude that this is a culture of birth which, to some, it violates basic bioethical principles to subjugate the dignity of female corporeality to contingent economic interests and transhumanists, biopolitics and taylorist ideologies reifying. Desde la perspectiva de la «sociología del espacio» el estudio explora las conexiones que se dan entre los factores ideológicos (paradigmas de representación del parto), los sistemas de organización clínica (culturas del parto) y las evoluciones arquitectónicas (tipologías espaciales de los paritorios), y sus repercusiones en clave de humanización asistencial. Para ello se ha realizado una investigación etnográfica de veinticuatro meses de duración en seis paritorios del servicio estatal de salud de una región del sur de España. La técnica de investigación fue la observación participante, acumulando un total de trescientas veinticuatro horas de análisis. Los resultados muestran cómo las disposiciones arquitectónicas están íntimamente ligadas a los paradigmas y culturas obstétricas preexistentes, tendiendo desde las perspectivas tecnocráticas, en clave orgánica, a una fragmentación del espacio asistencial que deshumaniza el proceso, y desde las cosmovisiones holístico-humanísticas, en clave de continuum, a una reintegración espacio-asistencial que contribuye a la humanización.
