Objetivo: Identificar a prevalência de hemoglobinopatias em recém-nascidos triados pelo Serviço de Referência do estado do Piauí e determinar à distribuição em diferentes regiões de saúde do estado, no período de 2016 a 2017. Métodos: Foram analisados 3.560 recém-nascidos (RN) com padrões hemoglobínicos alterados, triados no laboratório de Referência em Saúde Pública do Piauí, no período de 2016 a 2017. Os dados foram organizados em banco de dados no Microsoft Office Excel. E a análise realizada utilizando o programa Statistical Package for the Social Sciences® (SPSS) versão 20.0. Resultados: Dos 3.560 recém-nascidos (RN) com padrões hemoglobínicos alterados, 1.826 eram do sexo masculino, enquanto 1.734 eram do sexo feminino. A maioria dos recém-nascidos (RN) estudados que apresentaram hemoglobinopatias era constituída na sua grande maioria pela etnia parda (69,3%), seguidos por brancos (26,1%), cor preta (4,2%), amarela (0,4%) e indígena (0,1%). Entre as hemoglobinas alteradas, a mais prevalente foi a Hb S, totalizando 2.748 (77,19%). A análise da distribuição das hemoblobinopatias nas diferentes regiões de saúde do estado do Piauí foi constatada a maior prevalência de hemoglobinopatias na região de saúde entre rios, que concentra 39,9 %, seguido de 11,2% na região de cocais. Conclusão: A prevalência das hemoglobinas anormais nos neonatos triado identificou a anemia falciforme que consiste em uma doença crônica que comumente têm alta morbidade e mortalidade. O grande número de heterozigotos encontrados evidencia a necessidade de intervenções, como o aconselhamento genético e investigação de membros da família.
ObjectiveTo evaluate the incidence of variant hemoglobins in different health regions.MethodsA descriptive, observational, and cross-sectional study with a quantitative approach based on secondary data in the internal records of the neonatal screening service - Laboratório Central de Saúde Pública do Estado do Piauí (PI, Brazil). The variables related to sex, ethnicity and positive diagnosis for variant hemoglobins were analyzed, with further population distribution of hemoglobinopathies among the macroregions of the state.ResultsA total of 69,180 samples of newborns were analyzed, and 3,747 were diagnosed as hemoglobinopathies, from February 1st, 2014 and December 31st, 2015. Sickle cell trait was the most frequent (4.1%), followed by hemoglobinopathy C in 0.9%; homozygous hemoglobin S cases 0.1% stood out and there were no cases of hemoglobinopathy D in the state. It is also worth noting that the highest frequencies of hemoglobin alterations in Piauí were in males (49.8%) and of parda skin color (38.5%). The region of Piauí presenting the highest incidence of heteroygous variant hemoglobins was Tabuleiros do Alto Parnaíba and Vale do Sambito, due to importance of the region's population Entre Rios.ConclusionNeonatal screening programs are important for screening, orientations regarding health actions and monitoring of families with hemoglobinopathies, in order to reduce morbidity and mortality rates.
This survey aimed to assess the presence of triatomine vectors of Chagas disease within the rural communities of São João do Piauí, Northeast Brazil. Intradomiciliary and peridomiciliary collection strategies were implemented wherein 279 specimens of Triatoma brasiliensis macromelasoma, both nymph and adult were found in 15 (50%) of the studied homes. Of the intradomiciliary insects, 73 (67.6%) were identified as nymph instars (1st instar [N1]= 6, N2= 14, N3= 28, N4= 7 and N5= 18). In the studied communities, a continuous interaction between triatomine bugs and humans could be shown. It is therefore urgent that suitable strategies for the control of the triatomine vector are implemented in this area.
Este artigo tem como objetivo identificar o perfil de hemoglobinopatias em gestantes diagnosticadas durante o pré-natal no Estado do Piauí através de uma distribuição espacial e análise temporal com base nos dados de um laboratório de saúde pública. O estudo analisou os resultados de pacientes gestantes do Laboratório Central de Saúde Pública do Piauí no período entre janeiro de 2013 a dezembro de 2019. Ao todo 39.616 exames de pesquisa de hemoglobinopatias foram realizados; 2.480 (6,26%) das gestantes testaram positivo para hemoglobinopatias. Verificou-se prevalência de gestantes portadoras de Hb AS (73,63%) e Hb AC (10,56%); já os genótipos de doenças falciformes foram Hb SC: 5,48% , Hb SF: 0,77% e Hb SS: 0,44%; hemoglobinas variantes menos prevalentes: Hb AE (0,12%), Hb AF (0,08%) e Hb CC (0,04%). A maioria das gestantes identificadas com hemoglobinopatias tinham idade entre 13 a 25 anos e localizavam-se na mesorregião Centro-Norte Piauiense. A análise temporal revelou alterações percentuais discretas ao longo dos anos e obteve maior índice no ano de 2018 com 6,7% de gestantes diagnosticadas. A distribuição espacial identificou hemoglobinas variantes em 56 municípios, entre eles, Teresina, União, José de Freitas, Bom Jesus, Oeiras e Valença do Piauí apresentaram maior quantidade de casos. Os resultados obtidos indicam o potencial risco de traço falciforme e traço de hemoglobina C entre gestantes em diversas regiões do Piauí, aumentando a probabilidade de recém-nascidos portadores da doença, seja em homozigose ou heterozigose, refletindo uma importante questão de saúde pública no Estado.
SC Hemoglobinopathy is the result of heterozygous inheritance of the HbS and HbC alleles and can be the cause of anemic episodes. Under certain circumstances, the use of drugs may induce anemia as in membranopathies, erythroenzymopathies or in hemoglobinopathies. The present study reports the case of a 52 year old patient with infection at a surgical site after cervical arthrodesis following a C6 cervical spine fracture. The patient developed severe anemia (hemoglobin 5.8 g/dL) after administration of vancomycin and cefepime. After high performance liquid chromatography, it was elucidated that the patient was a carrier of hemoglobin SC. The use of antimicrobials showed a correlation with anemia, indicating the relevance of the rational use of drugs and the appropriate correlation between clinical and test results, especially in hospital settings.
