Os autores fazem um histórico sobre as pesquisas com células-tronco embrionárias e do cordão umbilical, suas respectivas vantagens e desvantagens. Seguem com as discussões sobre células-tronco adultas, sua definição, histórico, fontes e participação nos processos de regeneração tecidual, particularmente no endotélio. Ressaltam a importância de fatores que mobilizam as células-tronco adultas a partir da medula óssea: citocinas, angiopoietinas e outros fatores de crescimento. As células-tronco adultas mobilizam-se sob a forma de células endoteliais progenitoras, que têm origem comum com as células endoteliais a partir dos hemangioblastos. Os fatores de mobilização manifestam-se em condições de hipoxia e fazem com que as células endoteliais progenitoras se localizem nos locais de isquemia para produzir a neovasculogênese, que se faz por três possíveis mecanismos: a angiogênese (formação de novos capilares a partir de brotos de capilares já existentes), a arteriogênese (relacionada à circulação colateral) e a vasculogênese (vasos realmente novos). Fazem, a seguir, uma análise da literatura relativa à experimentação animal e aos estudos clínicos. Concluem ressaltando que as células-tronco adultas, embora tenham um grande potencial de uso, ainda demandam muito estudo e pesquisa para se firmar como método terapêutico.
CONTEXTO: A realização de programas de triagem para o aneurisma da aorta abdominal de forma indiscriminada auxiliará uma pequena porcentagem de indivíduos, mas ao considerarmos grupos com fatores de risco relacionados à dilatação da aorta, aumentando a probabilidade da doença, este permitirá um direcionamento adequado dos recursos e um benefício maior à população. Programas direcionados pelas sociedades médicas, permitindo um diagnóstico precoce das doenças vasculares e conseqüentemente um melhor preparo do doente, promoveriam melhores taxas de sobrevida com menor morbidade. OBJETIVO: Avaliar a prevalência da dilatação da aorta abdominal em uma amostra de pacientes idosos com mais de 60 anos de idade, portadores de coronariopatia aterosclerótica diagnosticada por cineangiocoronariografia. MÉTODOS: Para a seleção dessa amostra, levou-se em consideração o fato de que a avaliação pré-operatória de cirurgia vascular não tenhasido a indicação do cateterismo. Procedeu-se então a avaliação, baseada na anamnese, exame físico e Doppler ultra-som da aorta abdominal. A análise estatística iniciou-se com o teste qui-quadrado, com a posterior análise de regressão logística multivariada e regressão logística univariada, considerando significativo um p < 0,05. RESULTADOS: Dos 180 pacientes, 57 (31,7%) dos casos pertencem ao sexo feminino, e 123 (68,3%) ao masculino. A faixa etária variou entre 60 e 80 anos, com idade média de 66,7 anos. Dos 16 indivíduos portadores de dilatação da aorta abdominal (10 aneurismas e 6 ectasias), apenas um era do sexo feminino. O risco para um indivíduo com 1 lesão aterosclerótica coronariana de apresentar dilatação da aorta abdominal foi de 0,4% no grupo avaliado. Da mesma forma, nos portadores de 2 ou 3 lesões, o risco foi de 1,7%, e naqueles com mais de 3 lesões, de 4,5%. Quando associados ao tabagismo, estes valores alteraram-se respectivamente para 6,9, 11,8 e 27,1%. CONCLUSÃO: O presente estudo permite concluir que a prevalência de dilatação da aorta abdominal foi de 8,9% (16 de 180 pacientes) nesta amostra específica. Apresentou-se de forma mais freqüente nos indivíduos do sexo masculino, tabagistas e em presença de lesões ateroscleróticas difusas das artérias coronárias.
Relatamos o caso de um paciente masculino de 49 anos, portador de oclusão embólica de todas as artérias da perna esquerda. Após múltiplas e fracassadas tentativas de revascularização e, também, simpatectomia lombar em outros serviços, chegou a nós com indicação de amputação no nível de perna. Propusemos o implante de células-tronco autógenas de medula óssea na panturrilha. Obteve-se um excelente resultado, com a supressão da dor em repouso, melhora da hipotermia e palidez, elevação do índice tornozelo/braço, da velocidade sistólica de pico, dos índices cintilográficos e angiográficos e sensível melhora na granulação no local da amputação do quarto e quinto pododáctilo.
Scaffolds of chitosan and collagen can offer a biological niche for the growth of adipose derived stem cells (ADSC). The objective of this work was to characterize the physico-chemical properties of the scaffolds and the ADSC, as well as their interactions to direct influences of the scaffolds on the behavior of ADSC. The methodology included an enzymatic treatment of fat obtained by liposuction by collagenase, ASDC immunophenotyping, cell growth kinetics, biocompatibility studies of the scaffolds analyzed by the activity of alkaline phosphatase (AP), nitric oxide (NO) determination by the Griess-Saltzman reaction, and images of both optical and scanning electron microscopy of the matrices. The extent of the crosslinking of genipin and glutaraldehyde was evaluated by ninhydrin assays, solubility tests and degradation of the matrices. The results showed that the matrices are biocompatible, exhibit physical and chemical properties needed to house cells in vivo and are strong stimulators of signaling proteins (AP) and other molecules (NO) which are important in tissue healing. Therefore, the matrices provide a biological niche for ADSC adhesion, proliferation and cells activities.
The authors report a case of a 40-year-old woman who developed claudication of the right limb 3 months after extracorporeal shock wave lithotripsy (ESWL) owing to a pyelic calculus. Patient had no previous vascular disease. Arteriography revealed a 12-cm-long 80% stenosis of the right common, external, and internal iliac arteries; the rest of the arterial tree had no detectable pathology. Arterial complications related to ESWL have been reported before in patients with aortic aneurysms or very intense calcifications. To the authors' knowledge this is the first report of ESWL-induced injury in a patient without previous arterial pathology.
