Five patients with reticular erythematous mucinosis were studied by skin biopsy and phototesting. Light microscopy showed a dermal mononuclear cell infiltrate (composed largely of lymphocytes), separation of collagen bundles and fragmentation of elastic fibres. Histochemical stains showed increased dermal mucin with a profile consistent with hyaluronic acid. In one case, the ultrastructural distribution of mucin was demonstrated using colloidal iron staining, electron microscopy and X-ray microanalysis. In two cases, virus-like tubular aggregates were identified in the cytoplasm of dermal endothelial cells and pericytes. Immunofluorescent staining for immunoglobulins, fibrin and complement was negative.
Summary
A review of all the new cases of erythema annulare centrifugum presenting to this department from January 1976 to June 1981 has given information on 24 cases. No consistent abnormality was found in any of the investigations performed. Close association of systemic disease was noted in four instances, but in none of these cases was there any clear evidence of causality. The mean period of follow‐up from onset of disease was 5–9 years. No instances of malignancy associated with erythema annulare centrifugum have been identified in this group of patients over the period of observation.
Nine members of four families, each of whom suffered from lichen planus, are described. The disease appears to differ from the nonfamilial form in its earlier age of onset, tendency to chronicity, increased severity, and the frequent presence of "atypical" disease.
A case of pyoderma gangrenosum which occurred in a patient with non-Hodgkin's lymphoma is described. The lesion started in the foot where dye was introduced for a lymphangiogram.
