To investigate the toxicity profile and establish an optimal dosing schedule of zotiraciclib with temozolomide (TMZ) in patients with recurrent high-grade astrocytoma.Experimental Design: This two-stage phase 1 trial determined the maximum tolerated dose (MTD) of zotiraciclib combined with either dose-dense (Arm1) or metronomic (Arm2) TMZ using a Bayesian Optimal Interval design; then a randomized cohortexpansion compared the progression-free survival rate at 4 month (PFS4) of the two arms for an efficient determination of a TMZ schedule to combine with zotiraciclib at MTD. Pharmacokinetics (PK) and pharmacogenomic (PG) profiling were included. Patient-reported outcome was evaluated by longitudinal symptom burden.Results: Fifty-three patients were enrolled. Dose-limiting toxicities were neutropenia, diarrhea, elevated liver enzymes, and fatigue. MTD of zotiraciclib was 250mg in both arms and thus selected for the cohort expansion. Dose-dense TMZ plus zotiraciclib (PSF4 40%) compared favorably with metronomic TMZ (PFS4 25%). Symptom burden worsened at Cycle 2 but stabilized by Cycle 4 in both arms. A significant decrease in absolute neutrophil count and neutrophil reactive oxygen species production occurred 12-24 hours after an oral dose of zotiraciclib but both recovered by 72 hours. PK/PG analyses revealed that the CYP1A2_5347T>C (rs2470890) polymorphism was associated with higher AUC inf value.Conclusions: Zotiraciclib combined with TMZ is safe in patients with recurrent highgrade astrocytomas. Zotiraciclib-induced neutropenia can be profound but mostly transient, warranting close monitoring rather than treatment discontinuation. Once validated, polymorphisms predicting drug metabolism may allow personalized dosing of zotiraciclib.
Fundamento: Malformações congênitas da valva mitral são lesões complexas. Em crianças e adolescentes, os anéis protéticos devem ser evitados. Desde 1975 não usamos anéis protéticos, empregando-se a técnica de Wooler nas anuloplastias. Objetivo: Análise da evolução clínica tardia após o tratamento cirúrgico das anomalias mitrais congênitas, com e sem malformações associadas, em crianças até 12 anos de idade, tratadas com técnicas reparadoras e reconstrutivas sem suporte anelar. Casuística e Métodos: Foram avaliados 21 pacientes operados entre 1975 e 1998. A média de idade foi de 4,67 ± 3,44 anos; sexo feminino em 47,6%; a insuficiência esteve presente em 57,1% (12 casos), estenose em 28,6% (6 casos) e dupla lesão em 14,3% (3 casos). O tempo de perfusão foi 43,10 ± 9,50 min; tempo de isquemia 29,40 ± 10,50 min. O seguimento clínico na insuficiência foi feito em 12 pacientes com média de 41,52 ± 53,61 meses; no grupo de estenose em 4 pacientes com média de 46,39 ± 32,02 meses. Resultados: No grupo de insuficiência 10 pacientes estavam assintomáticos. Controle ecocardiográfico em 9 pacientes, (seguimento 37,17 ± 39,51 meses) 6 pacientes apresentaram refluxo leve, 1 ausência de refluxo e 2 com refluxo moderado; 1 paciente foi reoperado aos 48 m após a primeira operação, sendo feita nova plastia. No grupo da estenose, 4 pacientes, todos assintomáticos, sendo 2 sem medicação, com seguimento ecocardiográfico, média 42,61 ± 30,59 meses, gradiente médio entre 8 e 12 mmHg. No grupo de dupla lesão, 1 paciente foi reoperado para implante de bioprótese aos 43 meses da primeira operação, 1 paciente aos 75 meses da operação encontrava-se em classe funcional II e a ecocardiografia mostrou estenose e insuficiência leve. Não foram relatados episódios de endocardite nem tromboembolismo. A mortalidade operatória foi de 9,5% (2 casos). Não houve mortalidade tardia. Conclusões: a estenose mitral apresenta maiores dificuldades de correção, pelas anormalidades valvares e pela gravidade de lesões associadas. A correção da insuficiência sem suporte anelar apresenta bons resultados a longo prazo.
Background: Congenital mitral valve disease is associated with complex deformities of mitral apparatus. Annular support should be avoided in children and adolescents. Since 1975 we have employed non-supported techniques for mitral repair. Objective: This paper presents long term clinical results for repair in congenital mitral valve disease in children under 12 years of age. Patients and Methods: This series comprises 21 patients operated from 1995 to 1998. Mean age 4.6 ± 3.4 years. Female sex 47.6% mitral regurgitation in 57.1% (12 pt), stenosis 28.6% (6 pt) and mixed lesion 14.3% (3 pt). Perfusion time was 43.1 ± 9.5 min and ischemic time 29.4 ± 10.5 min. Follow-up time was 41.5 ± 53.6 months for the regurgitation group (12 pt) and 46.3 ± 32.0 months for the stenosis group (4 pt followed). AbstractExperience with infliximab (monoclonal antibody with strong anti-inflammatory action) for treatment of ocular inflammatory disease secondary to rheumatic diseases and refractory to disease-modifying antirheumatic drugs (DMARDs) is described. We evaluated the cases of a 50 year-old patient with active rheumatoid arthritis (RA) and a 37 year-old patient with myeloperoxidase antineutrophil cytoplasmic vasculitis antibody (MPO ANCA) without noble organ affectation, both with bilateral scleritis and left eye corneal perforation with iris prolapse. They received infliximab 3-5 mg/kg IV at 0, 2, 6, and 8 weeks. Infliximab was effective and safe for treatment of scleritis associated to RA and MPO ANCA positive vasculitis refractory to treatment with both DMARDs and high dose corticosteroids. Corneoscleral tissue grafts developed favorably with infliximab.
El recién nacido (RN) gemelo es de alto riesgo para morbilidad y mortalidad. Se le clasifica con curvas de crecimiento intrauterino (CCIU) de fetos únicos, pero no existen curvas peruanas para este grupo. OBJETIVO: Determinar el crecimiento intrauterino de un grupo seleccionado de gemelos peruanos y compararlo con patrones de RN únicos. MATERIAL Y MÉTODO: Estudio descriptivo, transversal y prospectivo durante 13 años (1992-2004) en el Hospital Hipólito Unanue de Tacna, Perú. De 518 gemelos, se seleccionó 282 gemelos (54%) sin factores qie alteraron el crecimiento intrauterino. Se encontró promedios, desviación estándar y percentiles 10, 50 y 90 de peso, talla y perímetro cefálico por edad gestacional de 32 a 41 semanas, los que fueron corregidos con polinomio de segundo orden. Se comparó valores de percentiles y promedios con curvas de RN únicos, se consideró significativo cuando p<0,05. RESULTADOS: De 36 909 recién nacidos vivos, 518 fueron gemelos, de los que se seleccionó 282. El grupo más representativo fue la semana 38 (percentiles 10 y 90 de peso fueron 2275 y 3180 g; de talla, 45,8 y 48,4 cm y perímetro cefálico, 323 y 354 mm, respectivamente. Comparando al gemelo con el RN único a la semana 38, la diferencia de peso en el p50 fue 452g. De los parámetros somatométricos estudiados, el peso fue el más comprometido (44,3%) de gemelos se localizó bajo el m10 de la curva de peso de gemelos con RN únicos, el comportamiento fue similar hasta la semana 34, a partir de la cual el crecimiento se deterioró significativamente. Al clasificar a los RN gemelos, se encontró una frecuencia de PEG de 12,.%, que se incrementa a 37% con las curvas de RN únicos. CONCLUSIÓN: Las curvas de crecimiento intrauterino de gemelos difieren significativamente del crecimiento de RN únicos. Se recomienda emplear tablas específicas para gemelos a fin de evaluar adecuadamente su crecimiento intrauterino.
