Objetivo: caracterizar as complicações associadas ao uso da via subcutânea na infusão de medicamentos e soluções em cuidados paliativos. Métodos: estudo observacional, prospectivo, realizado na Unidade de Cuidados Paliativos de um hospital geral de ensino. Resultados: foram avaliados 78 pacientes em cuidados paliativos, nos quais foram realizadas 254 punções na via subcutânea para infusão de medicamentos e/ou soluções. A maioria era idosa (87,3%) e encontrava-se desnutrida (69,2%). Em 33,0%, a via foi indicada para controle de sintomas e, em 50,0%, a punção ocorreu na região anterolateral da coxa; 65,4% das punções não mostraram complicação. Dentre as complicações identificadas, 9,4% foram edema e 9,1% hiperemia, sendo que 53,8% delas ocorreram na região deltoidea. A celulite ocorreu em apenas 3,5%. Conclusão: a infusão de soluções pode ser considerada uma alternativa segura, a qual apresenta risco de complicações em sua maioria facilmente reversíveis e com baixo potencial de ocasionar danos aos pacientes.
Resumo O estudo teve o objetivo de compreender as representações de usuários e de profissionais de serviços de saúde mental acerca da sexualidade dos primeiros. Entrevistaram-se individualmente 39 usuários e 54 profissionais de serviços públicos de saúde mental por meio de seis grupos focais. A análise dos dados fundamentou-se na Análise Estrutural de Narração. Pessoas com transtornos mentais (PTM) não representam saúde sexual como direito e encontram dificuldades para se cuidarem em face de estereótipos de gênero e pelo contexto de exclusão e pobreza. Entre os profissionais, as representações foram de negação da sexualidade das PTM, entendendo-a como “fora do normal” e que deve ser reprimida. Diálogos sobre sexualidade com os usuários são quase inexistentes. Os profissionais não estão preparados para assistência integral, o que requer capacitação permanente nos serviços e inclusão do tema na formação básica nas carreiras da área da saúde, de forma interdisciplinar.
The goal of this study was to understand the ways of living and thinking about sexuality of people with mental disorders. Open interviews were conducted with men and women in public mental health services in Brazil. Transcrips were examined based on the proposal of sexual scripts. Major imbalances coming from conceptions of masculinity and femininity in society were identified in the sexual scripts experienced by these men and women. Interviewees have little pleasure in their sexual lives, with recurrent complaints of sexual abuse, even by steady partners; prejudice; and lack of affection in their relationships. Additionally, they were found to have few self-care skills concerning sexual health, in a context marked by social exclusion. The results showed the need to promote sexual health as a human right, and fight gender stereotypes, which cause so much damage to the sexual health of people with mental disorders.Keywords: sexuality; people with mental disorders; sexually transmitted disease; gender; nursing Género y sexualidad de las personas con trastornos mentales en Brasil Resumen: El objetivo de este estudio fue comprender las formas de vivir y de pensar acerca de la sexualidad de las personas con trastornos mentales. Fueron realizadas entrevistas abiertas con hombres y mujeres en servicios públicos de salud mental en Brasil y las mismas fueron examinadas con base en la propuesta de scripts sexuales. En los scripts sexuales experimentados por hombres y mujeres fueron identificados los desequilibrios propios de los conceptos de masculinidad y feminidad de la sociedad. Las y los entrevistados tienen poco placer en sus vidas sexuales, con recurrentes denuncias de abuso sexual, inclusive por parte de sus parejas estables; prejuicios; y falta de afecto en sus relaciones. Además, fueron encontradas escasas habilidades de autocuidado relativas a la salud sexual, en un contexto marcado por la exclusión social. This study aimed to identify and understand adult patients' representations on tube feeding. Data were collected through interviews with sixteen patients who were using feeding tubes, which were analyzed through discourse analysis and Social Representation Theory. We identified a sequence in which representations are reorganized and redefined through experience, in order to turn this treatment tolerable. This sequence begins when patients face the need of a feeding tube, followed by the experience of its use and finally, when deciding about continuing its use or not, when going home. Some representations contribute to treatment adherence and others to its rejection. Getting to know positive and negative ideas about this treatment makes it possible to approach these patients in a more direct way, with a view to better adherence and satisfaction.
