BackgroundThe two largest needle exchange programs (NEPs) in Hungary were forced to close down in the second half of 2014 due to extreme political attacks and related lack of government funding. The closures occurred against a background of rapid expansion in Hungary of injectable new psychoactive substances, which are associated with very frequent injecting episodes and syringe sharing. The aim of our analysis was to predict how the overall Hungarian NEP syringe supply was affected by the closures.MethodsWe analyzed all registry data from all NEPs in Hungary for all years of standardized NEP data collection protocols currently in use (2008–2014) concerning 22 949 client enrollments, 9 211 new clients, 228 167 client contacts, 3 160 560 distributed syringes, and 2 077 676 collected syringes.ResultsWe found that while the combined share of the two now closed NEPs decreased over time, even in their partial year 2014 they still distributed and collected about half of all syringes, and attended to over half of all clients and client contacts in Hungary. The number of distributed syringes per PWID (WHO minimum target = 100) was 81 in 2014 in Hungary, but 39 without the two now closed NEPs.ConclusionsThere is a high probability that the combination of decreased NEP coverage and the increased injection risk of new psychoactive substances may lead in Hungary to a public health disaster similar to the HIV outbreaks in Romania and Greece. This can be avoided only by an immediate change in the attitude of the Hungarian government towards harm reduction.
Research has demonstrated that exercising has health promoting effects. However, if habitual sporting activities become uncontrollable, detrimental health consequences can occur among a minority of individuals. Furthermore, such obligatory exercise can cause serious decline in school/work productivity, as well as financial problems, relationship problems, and poor psychological and physical wellbeing. The aim of the present study was to compare characteristics related to exercise dependence (ED) between recreational exercisers and amateur competitors. A total of 1439 participants (41.4 % male; mean age = 32 years) completed a battery of measures including the Exercise Dependence Scale (EDS), SCOFF, Well-Being Questionnaire, and Rosenberg's Self-Esteem Scale. Results showed that 6.5 % of participants identified themselves as amateur competitive exercisers. Amateur competitors exercised significantly more (6.4 h) than non-competitor recreational exercisers (4.6 h). Amateur competitors also scored significantly higher on the EDS. Significant effects were found between competing and self-esteem concerning ED. Results showed that both features had a strong effect on ED. The study highlights the connection between ED symptoms and lower self-esteem and/or lower levels of subjective wellbeing. These influential factors are worth considering when planning preventive interventions addressing ED for both amateur competitors and non-competitive recreational athletes as well as when promoting sport as a healthy activity.
Bevezetés: A gyermekbántalmazás hazánkban és világviszonylatban is aggasztó tendenciát mutat. A közlemény az Egészségügyi Világszervezet által is népegészségügyi problémának minősített gyermekbántalmazás, az ártalmas gyermekkori tapasztalatok hosszú távú káros következményeit mutatja be. Az alapellátásban dolgozók viszonyulása a bántalmazás megelőzéséhez alapvetően befolyásolja a prevenciós tevékenység minőségét. Célkitűzés: Felmérni az alapellátásban és a szakellátásban dolgozó gyermekorvosok, védőnők, szülésznők és gyermekápolók ismereteit és véleményét a gyermekbántalmazás témaköréről. A tanulmány kontextusba helyezi a gyermekbántalmazással kapcsolatos értékítéleteket és a társadalmi felfogást a transzgenerációs hatások érdekében. Módszer: 49 strukturált kérdést tartalmazó önkitöltős, online kérdőíves felmérést végeztünk, mely kitekintéssel volt a válaszadók szocioökonómiai státuszára, a bántalmazás kérdésköréhez való viszonyulásukra, a téma súlyosságának megítélésére, valamint a törvény által szabályozott jelzőrendszeri kötelezettségükre. Eredmények: Elemzéseink alapján a 195 fős mintán elvégzett kutatás eredményeként megállapítható, hogy az egészségügyi dolgozók a bántalmazást egyértelműen ártalmasnak találják, mégis gyakori jelenségnek érzékelik hazánkban. A válaszadók közel egyötöde szerint a társadalom elfogadó, és enyhén ítéli meg a jelenséget. A bántalmazás megítélésében iskolai végzettség szerint szignifikáns különbséget találtunk a magasan és az alacsonyabban iskolázottak között (p = 195), valamint szignifikáns különbséget találtunk a jelzőrendszeri működés tekintetében a munkahely településtípusa (p = 0,020) és a munkakör (p = 0,044) kategóriáiban. Következtetés: A kapott eredmények rávilágítottak arra, hogy az egészségügyi dolgozók preventív tevékenysége a transzgenerációs hatások megtörésében kulcsfontosságú. A szerzők szeretnék felhívni a figyelmet arra, hogy az ellátás minőségének javulása jelentősen függ a szakdolgozók tudásától. Közleményünkben egy olyan új kezdeményezésről számolunk be, amelynek tárgya egy nemzetek közötti BSc ápolóképzési program kidolgozása Transatlantic Curriculum in Nursing néven, amely elvezet az Európában és az Amerikai Egyesült Államokban egyaránt érvényes úgynevezett kettős diploma kiadásához. A jelöltek -tanulmányaik sikeres teljesítése esetén -mind az európai, mind az amerikai partner diplomáját megkapják. A projekt célja kulturálisan kompetens, nemzetközi munkaerő képzése, az ápolóhallgatói csereprogram gyakorlatának fejlesztése, a kettős diploma modelljének kidolgozása földrajzi, politikai és kulturális határoktól függet-lenül, valamint evidenciákon alapuló ápolási standardok terjesztése a mindennapi gyakorlatban. A programban részt vevő intézmé-nyek: Semmelweis Egyetem Egészségtudományi Kara, Nazareth College (Rochester, NY, Egyesült Államok), Laurea University of Applied Sciences (Tikkurila, Finnország). A projekt keretében tervezett fontosabb tevékenységek: kölcsönös hallgató-és oktatócse-re, közös tantervfejlesztés és képzési folyamat, közös standardok meghatározása. A program várható eredményei a következők: standardizált modell kidolgozása a nemzetközi ápolóképzési programok fejlesztésére, kivitelezésére vonatkozóan; az intézményi munkakultúra fejlesztése; nyelvi és szakmai kulturális készségek fejlesztése; új típusú ápoló szakemberek modelljének megteremtése, akik olyan szintű és irányú nemzetközi szakmai, kulturális és nyelvi kompetenciákkal rendelkeznek, amelyek szükségesek a globális programokban való részvételhez. The objective is to prepare an internationally and culturally competent workforce; develop the practice of nursing students' exchange programs; process the model of dual degree independent of geographical, political or cultural borders; spread the evidence-based nursing standards in the daily practice. The partners in this initiative are Semmelweis University in Budapest, Hungary, Nazareth College of Rochester, NY, USA and Laurea University in Tikkurila, Finland. Similarly to other countries in the Eastern European Region, the situation of medical assistants in Romania is fairly difficult. Due to the lack of personnel, health professionals are typically overwhelmed with work. The Quality of Life Research Centre at Sapientia Hungarian University of Transylvania conducted a non-representative survey among medical assistants in Romanian state-owned healthcare institutions. The sample size is 312 Romanian and Hungarian speaking nurses from six counties. The present study problematizes the mental health of medical assistants, more precisely burnout and depression, along demographic, social, and labour market features. The determinants of burnout and depression are being searched for among these structural and situational variables, and their controlled impact is being assessed with linear regression. Results indicate an excessive risk of burnout and depression for nurses with lower-qualification working in outpatient care. Differences in the mental health of medical assistants echo to a large extent social inequalities, so that controlled for covariates, household income has a significant impact upon burnout and depression. From demographic agents, the protective effect of partnership is outstanding, and the number of supportive relationships is a protective factor of its own right against both burnout and depression. Female assistants are more at risk for depression but not for burnout, whereas workload increases the risk of burnout but not of depression. The analysis takes sides of the distinctness of burnout and depression. Although both syndromes are largely influenced by social features, burnout seems to be more situationally influenced whereas depression more structurally affected.
