Os métodos ativos de aprendizagem necessitam de processos avaliativos igualmente coerentes com o novo perfil profissional. A Faculdade de Medicina de Marília (Famema) vem utilizando o portfólio reflexivo como instrumento de avaliação formativa no curso médico. Este trabalho propõe-se a analisar a percepção dos estudantes quanto ao seu uso. Trata-se de um estudo qualitativo cujos dados foram coletados pela técnica de grupo focal, à qual se seguiram os passos da técnica de análise de conteúdo na modalidade temática. Definiram-se, então, as categorias temáticas: "O portfólio enquanto espaço que oportuniza reflexão sobre a prática e a aprendizagem ativa"; "Um instrumento de avaliação pro cessual e dialógica"; "O imaginário biomédico e a resistência às mudanças permeando a construção do novo modelo" e "O reconhecimento da necessidade de mudanças". Pode-se considerar que o portfólio reflexivo constitui um instrumento que contribui para a formação profissional, embora sua utilização requeira constantes aproximações e ajustes.
The events of ischemia-reperfusion injury trigger a systemic inflammatory response that can lead to cellular damage and even organ failure. Such effects are noted in the post-operative recovery, especially with the use of cardiopulmonary bypass. Nowadays, we know that leukocytes play an important role in this process. Therefore, this study addresses the role of leukocytes in the pathophysiology of ischemia-reperfusion injury and activation of inflammatory cascades through this process and seeks to help in understanding these mechanisms as well as bringing contributions on the therapeutic approaches that may mitigate them. This retrospective review was performed from scientific papers published over the past ten years, in Portuguese and English, indexed in international databases of Medline, SciELO and related classical texts. Tais repercussões são notadas no pós-operatório de cirurgias, em especial, com o uso de circulação extracorpórea. Sabe-se, atualmente, que os leucócitos exercem importante papel neste processo. Assim, este estudo aborda o papel dos leucócitos na fisiopatologia das lesões de isquemia-reperfusão e a ativação das cascatas inflamatórias por esse processo e procura auxiliar na compreensão destes mecanismos assim como trazer contribuições acerca das abordagens terapêuticas que possam atenuá-los. Esta revisão bibliográfica retrospectiva foi realizada a partir de documentos científicos publicados nos últimos dez anos, em português e inglês, indexados em bases de dados internacionais Medline e SciELO e de textos clássicos relacionados. O fenômeno da profissionalização de pessoas com deficiência é complexo e perpassa as contradições do sistema capitalista, a estrutura e interações sociais, o comportamento e psiquismo humano. Entre os desafios para a profissionalização de pessoas com deficiência elencam-se: postos precários, informais, baixa ou nenhuma remuneração, baixas escolaridade e qualificação técnica. A legislação traz avanços, porém o cumprimento é frágil e reducionista. Este trabalho apresenta possibilidades de contribuições da psicologia nos diferentes contextos: junto aos sujeitos com deficiências e seus familiares, à comunidade empresarial, às instituições e à sociedade civil, por meio da educação. Discute-se o tema expondo as limitações a partir dos diferentes sujeitos sociais e as contradições que coexistem nas políticas públicas, mas, sobretudo, aponta-se caminhos e propõe-se na psicologia importantes instrumentos para a inclusão. Palavras-chave: Pessoas com deficiência; capacitação professional; emprego.The phenomenon of professionalization of disabled people is complex and goes beyond the contradictions of the capitalist system, social structure and interactions, behavior and the human psyche. Among the professionalization challenges for people with disabilities, we list the following: insecure and informal positions, minimal or no compensation, low education and qualifications. Legislation brings advances but its compliance is fragile and reductionist. This paper presents potential contributions from psychology, through education, in different contexts: together with disabled persons and their families, the business community, institutions, and civil society. We discuss the professionalization of people with disabilities; the limitations are exposed considering the different social subjects and the contradictions that exist in public policies. But mainly, direction is offered and important instruments in psychology are proposed for greater inclusion. Background: The arterial in line application of the leukocyte inhibition module (LIM) in the cardiopulmonary bypass (CPB) limits overshooting leukocyte activity during cardiac surgery. We studied in a porcine model whether LIM may have beneficial effects on cardiac function after CPB.
Systemic administration of immune modulating antibodies may play an important role in reducing neutrophil hyperactivity, for example, in patients undergoing cardiac surgery with extracorporeal circulation or in trauma patients. However, this strategy has extremely high costs and is often associated with severe adverse effects. We developed the Leukocyte-Inhibition-Module (LIM), an extracorporeal circulation (ECC) device housing a polyurethane matrix with covalently bound Fas (CD95; APO-1) stimulating antibodies to rapidly prevent neutrophil hyperactivation. A feasibility study with 14 patients undergoing cardiac surgery with the use of immunogenic ECC without (n = 5) and with (n = 9) LIM (venous line) was performed. Our data show that the usually observed ECC associated perioperative increase in neutrophils (control) was prevented by LIM (p = 0.023). Moreover, the increase of the proinflammatory markers tumor necrosis factor (TNF)-alpha and polymorphonuclear elastase was limited by LIM (p = 0.038 and p = 0.002). In both groups, no significant changes in liver enzymes or in clotting were detected after surgery, and up to 12 months follow up, no unusual complications were reported. This study shows for the first time to our knowledge the feasibility, efficacy, and safety of a new cost effective, immune management strategy in patients with aberrant immune activation by exposing the blood stream to immobilized agonistic anti-Fas antibodies.
