Lactobacilli supposedly have low pathogenicity; they are seldom detected in blood culture. Lactobacillus rhamnosus GG, which originates indigenously in the human intestine, became available for use as a probiotic in 1990 in Finland. We evaluated the possible effects of the increased probiotic use of L. rhamnosus GG on the occurrence of bacteremia due to lactobacilli. Lactobacilli were isolated in 0.02% of all blood cultures and 0.2% of all blood cultures with positive results in Helsinki University Central Hospital and in Finland as a whole, and no trends were seen that suggested an increase in Lactobacillus bacteremia. The average incidence was 0.3 cases/100,000 inhabitants/year in 1995-2000 in Finland. Identification to the species level was done for 66 cases of Lactobacillus bacteremia, and 48 isolates were confirmed to be Lactobacillus strains. Twenty-six of these strains were L. rhamnosus, and 11 isolates were identical to L. rhamnosus GG. The results indicate that increased probiotic use of L. rhamnosus GG has not led to an increase in Lactobacillus bacteremia.
The incidence of human infections caused by Campylobacter jejuni and Campylobacter coli, the main bacterial agents of gastrointestinal disease, has been increasing worldwide. Here, we review the role of poultry as a source and reservoir for Campylobacter. Contamination and subsequent colonization of broiler flocks at the farm level often lead to transmission of Campylobacter along the poultry production chain and contamination of poultry meat at retail. Yet Campylobacter prevalence in poultry, as well as the contamination level of poultry products, vary greatly between different countries so there are differences in the intervention strategies that need to be applied. Temporal patterns in poultry do not always coincide with those found in human infections. Studies in rural and urban areas have revealed differences in Campylobacter infections attributed to poultry, as poultry seems to be the predominant reservoir in urban, but not necessarily in rural, settings. Furthermore, foreign travel is considered a major risk factor in acquiring the disease, especially for individuals living in the northern European countries. Intervention strategies aimed at reducing Campylobacter colonization in poultry and focused at the farm level have been successful in reducing the number of Campylobacter cases in several countries. Increasing farm biosecurity and education of consumers are likely to limit the risk of infection. Overall, poultry is an important reservoir and source of human campylobacteriosis, although the contribution of other sources, reservoirs and transmission warrants more research.
Lactobacillus bacteremia is a rare entity, and its clinical significance is poorly defined. We have reviewed the risk factors and outcome for 89 case patients with Lactobacillus bacteremia. Species characterization was done in 53% of the cases, revealing 25 L. rhamnosus strains and 22 other Lactobacillus species. In 11 cases, the strain was identical with the probiotic L. rhamnosus GG. In 82% of the cases, the patients had severe or fatal comorbidities. Predisposing factors to bacteremia were immunosuppression, prior prolonged hospitalization, and prior surgical interventions. No significant differences were observed in these predisposing factors or clinical features between patients with cases associated with the various Lactobacillus species, other than higher C-reactive protein values in patients with L. rhamnosus bacteremia. Mortality was 26% at 1 month and was 48% at 1 year. In multivariate analysis, severe underlying diseases were a significant predictor for mortality (odds ratio [OR], 15.8), whereas treatment with antimicrobials effective in vitro was associated with lower mortality (OR, 0.22). We conclude that lactobacilli in blood cultures are of clinical significance and that their susceptibility should guide decisions about antimicrobial treatment.
Campylobacter jejuni and C. coli are the most frequent causes of bacterial gastroenteritis in Finland (www.ktl.fi/ttr). The number of laboratory-confirmed cases has nearly doubled over a 10-year period (2,197 cases in 1995 to 4,002 cases in 2005). Campylobacter species are frequently isolated from a wide variety of sources, including poultry, cattle, pigs, sheep, cats, dogs, wild birds, and water. Most Campylobacter infections are sporadic, and the relative contributions of different sources of infection remain unknown. Risk factors for Campylobacter infection, as identified by case-control studies, include eating or preparing raw or undercooked meat, especially chicken meat, drinking unpasteurized milk or untreated water, contact with domestic animals and pets, and foreign travel (14,18,26). A recent case-control study (19) revealed swimming in natural bodies of water to be a novel risk factor. In addition, eating undercooked meat and drinking from dug wells were independent risk factors for domestically acquired Campylobacter infections in Finland (19).Discriminatory typing methods for use in the study of the molecular epidemiology and population genetics of Campylobacter isolates are crucial to better understand the epidemiology and ecology of the organism. Once such information is obtained, it could be used to develop intervention strategies to limit the numbers of human infections. Dingle et al. (6) developed a multilocus sequence typing (MLST) scheme for C. jejuni, which has been shown to be a valuable tool for studying the diversity and population genetics of Campylobacter isolates.
Changes in Helicobacter pylori seroprevalence were studied determining IgG and IgA antibodies of 408 randomly selected adults aged 15-74 years living in Vammala, Finland in 1973 and of 504 similarly selected subjects in 1994. Seroprevalence increased by age at both time points. The age-adjusted seroprevalence rate was clearly lower in 1994 than in 1973 (31 vs. 56%, P = 0.001). Paired serum samples of 224 subjects collected in 1973 and 1994 showed that the antibody status remained unaltered in 92%; 4% seroconverted and 4% seroreverted within the 21 years. The decrease in the seroprevalence rate in the population and the persistence of individual antibody status over two decades support a difference in H. pylori infection rates among birth cohorts over time rather than continuous acquisition of new infections with advancing age. Thus the risk of helicobacter infection in Vammala, Finland has been highest in childhood and continuously decreased at least for the last five decades.
Waterborne outbreaks associated with contamination of drinking water by Campylobacter jejuni are rather common in the Nordic countries Sweden, Norway, and Finland, where in sparsely populated districts groundwater is commonly used without disinfection. Campylobacters, Escherichia coli, or other coliforms have rarely been detected in potential sources. We studied three waterborne outbreaks in Finland caused by C. jejuni and used sample volumes of 4,000 to 20,000 ml for analysis of campylobacters and sample volumes of 1 to 5,000 ml for analysis of coliforms and E. coli, depending on the sampling site. Multiple samples obtained from possible sources (water distribution systems and environmental water sources) and the use of large sample volumes (several liters) increased the chance of detecting the pathogen C. jejuni in water. Filtration of a large volume (1,000 to 2,000 ml) also increased the rate of detection of coliforms and E. coli. To confirm the association between drinking water contamination and illness, a combination of Penner serotyping and pulsed-field gel electrophoresis (digestion with SmaI and KpnI) was found to be useful. This combination reliably verified similarity or dissimilarity of C. jejuni isolates from patient samples, from drinking water, and from other environmental sources, thus confirming the likely reservoir of an outbreak.
SUMMARYBackground: H. pylori is the major cause of chronic gastritis, and a risk factor for peptic ulcer and gastric cancer. Aim: To investigate the effect of probiotic supplementation on the tolerance and efficacy of H. pylori eradication treatment in a randomized, double-blind, placebo-controlled trial. Methods: A total of 338 volunteers were screened for H. pylori infection. The eligibility criteria were met by 47 subjects whose H. pylori infection was verified at the outset and re-evaluated after the treatment by the 13 C-urea breath test and by enzyme immunoassay
ReA is common following Campylobacter infection, with an annual incidence of 4.3 per 100000. At the population level, acute ReA is mild, more frequent in adults, and not associated with HLA-B27. Besides C. jejuni, C. coli can trigger ReA.
