PKU Muhammadiyah Cangkringan Primary Clinic serves outpatient and inpatient services. The patient service process was still using microsoft excel, from patient registration to reporting. The problem that occurs was the accumulation of files due to not implemented the medical record file retention process, and the medical record file retention information system does not yet exist. This study aims to design a web based medical record file retention information system. The prototype design method was used as a method for developing web based information systems. The design using visual studio code editorial text and interface design with the help of a bootstrap framework, the system uses the laravel framework, and the database uses mysql. The result, a web based medical record file retention information system that can facilitate officers and doctors in viewing patient forms that have been retained, can save scans of medical record forms before they are destroyed, and can display scan results forms that have been uploaded into the information system. This information system requires patient data to see a list of visits of the last 2 years which is used to determine the medical record file that will be carried out the retention process.
Background: Policy about manage medical record had managed the system for reach order administration and improvement health services to show quality of hospital. New policy from Permenkes No.269/MENKES/PER/III/2008 make influence in this hospital policy because some policy using old government policy. At previously research, implementation that policy had many problem with some factor problem to fill in the main diagnose and the accurate code diagnose. The purpose of research is to know the policy to fill in the main diagnose and the accurate diagnose code at medical record in PKU Muhammadiyah hospital Yogyakarta. Method: This was non experiment research, this is deskriptif kualitatif and kuantitatif data as supporting to explain. The subject of research was internis, head of medical record, coder and documents medical record patient treatment internal disease at November 2008. Technique sampling for doctor internal disease, head of medical record, and coder was purposive sampling. But Technique sampling for documents medical record was simple random sampling. Kualitatif data tested validity with triangulasi. While for kuantitatif data the accurate diagnose code check using ICD-10. Result: The result of triangulasi identificated some factor problem to fill in the main diagnose and the accurate code diagnose. There is completeness documents medical record, busy, forget, lazy, not discipline, over burden work, patient APS, new terminology, difference perception, tools not support. The other policy most supporting that activity policy. Conclusion:The policy most important for manage activity and communication between staff most supporting to result of policy implementation. Some factor problem can be overcome if all staff aware that policy is important and put into effect.
The health centre Gondokusuman II was a Yogyakarta Primary Health Centre with two assisted villages, namely Terban and Kotabaru, which has sixteen integrated health post groups for the elderly. Each group of integrated health post sends the results of the patient's health activities once a month to the health centre Gondokusuman II. During this time the reporting used to manage patient data using microsoft excel, but in reporting management found deficiencies namely incomplete reporting, access rights processing elderly patient data can be used by all staff, and in the age column was still filled manually. The purpose of the study was to design a microsoft access 2010 integrated health post based elderly information reporting system in the health centre Nutrition Consultant Unit Gondokusuman II. This study uses a prototype design method as a method of developing management information systems based on microsoft access. The subject of this research was the integrated health post officers of the elderly in the nutrition consultant unit of the health Gondokusuman II. In this study started from the needs of users, applications and information system prototypes that are used as microsoft based desktop access. This prototypes was approved by users, complete filling becomes more complete, access rights can be set, and the age calculation was automatic.
Latar belakang: Klinik pratama dan pelayanan persalinan Citra Madina merupakan pengembangan dari Bidan Praktek Mandiri yang menyelenggarakan pelayanan untuk pemeriksaan umum, wanita hamil, persalinan, pemeriksaan fisik, tindakan medis, dan hasil anamnesa. Sistem pengelolaan rekam medis berbasis kertas ketika proses pendaftaran dan pengisian formulir rekam medis, serta menggunakan microsoft excel saat menyimpan data sosial pasien dan alergi obat. Sistem yang ada memungkinkan riwayat data pelayanan tidak berkesinambungan. Tujuan: Merancang sistem informasi rawat jalan dan pelayanan persalinan berbasis web.Metode: Penelitian kualitatif mengikuti tahapan development sistem, metode waterfall. Responden dalam analisis kebutuhan sistem yaitu petugas pendaftaran rawat jalan, dokter, bidan, dan apoteker.Hasil: Rancangan sistem informasi rawat jalan dan pelayanan persalinan berbasis web yang memiliki beberapa fasilitas yaitu login, pengelolaan data pasien umum, pemeriksaan ibu hamil dan melahirkan, data kelahiran bayi, data obat, data pembayaran, dan pelaporan.Kesimpulan: Perancangan sistem informasi rawat jalan dan pelayanan persalinan berbasis web mampu memaksimalkan keamanan dari orang-orang yang tidak berhak mengakses sistem. Pengembangan sistem pada bidan praktek mandiri menjadi klinik pratama ini dapat digunakan untuk mengolah data pemeriksaan pasien umum, pemeriksaan ibu hamil, ibu melahirkan dan catatan kelahiran bayi, serta mampu mengolah laporan kunjungan, laporan penyakit, laporan obat dan laporan pembayaran.
Tuberculosis (TB) and diabetes mellitus (DM) are both important health issues. A bidirectional association between them has been demonstrated. The link of DM and TB was more prominent in developing countries where TB is endemic and the burden of diabetes mellitus is increasing. The association between diabetes and tuberculosis may be the next challenge for global tuberculosis control worldwide. Proper planning and collaboration are necessary to reduce the dual burden of diabetes and TB. In order to encourage the implementation of TB prevention programs, data and information are needed in the information system. An integrated and integrated TB programming information system. Spatial analysis with an information system is a device that can detect of hight risk areas, so it can help for handling and control. TB patients with DM 2014 in the Kulon Progo regency, there were 16 people. Spatial analysis using GeoDa TM software version 1.6.6, and ArcGIS version 10.1.The spatial interaction model is a classical regression model that there have signiicant correlation between DM population with pulmonary TB-DM incidence, with p value = 0.03776, but no spatial dependence. TB control strategy through TB development program. TB program storage program and DM holder in Dinas Kesehatan want to do monitoring to monitor lung tuberculosis patient with history of DM.
