Abstract. This article illustrates the 4-month follow-up projective case study of John, a 16-year-old boy hospitalized for below-threshold psychotic symptoms. Emphasizing the relevance of a psychodynamic approach to emerging psychosis, the authors briefly describe the psychoanalytical framework of the adolescent process theory as well as the French School projective method. The aim of the study is to show that this method of assessment does not fall into the criticized standpoints of naïve complacency or of excessive zealotry against early detection and intervention. On the contrary, the complementary use of both the Rorschach and the Thematic Apperception Test, alongside clinical interviews, was valuable in uncovering the complex heterogeneity of adolescent functioning. Results revealed core psychotic impairments of self-representation and thought processes as well as non-negligible psychological changes and transformation, which allowed for engagement in therapeutic treatments and reinforced a more positive prognostic outcome. Limitations regarding the short-term interval and the qualitative analysis of discrepancies are discussed.
Cet article s’attache à repérer la clinique de l’adolescence et ses résonances théoriques dans l’œuvre du créateur de la psychanalyse. Nous suivons la chronologie des Œuvres complètes de Sigmund Freud en explorant les incises du Tome 1, non sans situer le contexte professionnel de Freud au début de sa carrière, marquée par la théorie héréditaire de son temps. Alors qu’il n’a pas encore donné toute sa place à la sexualité infantile, sa quête d’une étiologie de la névrose passe par la caractère éruptif de l’adolescence, sollicitant les champs de la neurasthénie et ce qu’il nomme « l’hystérie d’occasion ».
L’article présente une construction théorique pour penser les manifestations d’allure psychotique à l’adolescence. Entre l’archaïque et l’actuel, le paradigme de la psychose pubertaire s’affranchit de la référence hâtive à la vulnérabilité schizophrénique pour redonner une valeur processuelle à la notion de potentialité. La psychose pubertaire dévoile un caractère éminemment frontalier et sa symptomatologie traduit des modalités de traitement qui s’étendent sur le spectre d’un travail du négatif nécessaire mais incertain.
L’adolescence est un processus dynamique qui entraine de profonds remaniements somatiques et psychiques et conduit l’adolescent vers un nécessaire travail de subjectivation. La psychose pubertaire, dans ce contexte critique, est une rupture du développement qui expose les fêlures de ce processus en révélant les potentialités psychotiques antérieures et en menaçant de faire dévier les chemins de la structuration. Après une brève réflexion théorique sur la notion de psychose pubertaire, comprise comme une double potentialité psychopathologique, ainsi qu’une présentation succincte de l’apport des travaux de l’École de Paris à la question de la psychose à l’adolescence, les auteurs illustrent leurs propos par l’étude clinique et projective du cas d’Amandine, patiente rencontrée dans le cadre d’une recherche doctorale. Les résultats montrent l’hétérogénéité de cette clinique sujette à des aménagements à la fois psychotiques et limites et mettent en évidence l’existence d’un mouvement mélancolique, point d’orgue pour la compréhension du déclenchement des pathologies adolescentes.
