Zusammenfassung Hintergrund Trotz Brandschutzmaßnahmen und Aufklärung erleiden ca. 0,5 Millionen Menschen pro Jahr in Deutschland Verbrennungen. Im Jahr 2018 mussten fast 2000 Schwerstbrandverletzte in deutschen Verbrennungszentren behandelt werden. Brandverletzungen stellen eine schwerwiegende Erkrankung mit hoher Mortalität und langfristigen Folgen für den Patienten dar. Methodik und Ergebnisse Ziel dieses Übersichtsartikels ist es, eine Zusammenfassung der derzeit gültigen Leitlinie zur Behandlung von Verbrennungen im Erwachsenenalter (https://www.awmf.org/leitlinien/detail/ll/044–001.html) zu präsentieren, wobei der Fokus auf folgenden Aspekten liegt: 1. der Akutbehandlung der Brandwunde am Unfallort, 2. der Entscheidungsfindung zur Verlegung in ein Brandverletztenzentrum, 3. der chirurgischen Wundversorgung mit Debridement und Hauttransplantation und 4. der Nachsorge. Die S2k-Leitlinie basiert auf aktueller Literatur und dem Konsens von 11 an der Behandlung Brandverletzter beteiligter Fachgesellschaften. Zusammenfassung Dieser Übersichtsartikel enthält gewichtete Empfehlungen zur Behandlung von Brandwunden erwachsener Patienten in Deutschland. Der Artikel soll Ärzten und medizinischem Personal in der Akutsituation vor Ort und in der Klinik die Entscheidung über die notwendige Behandlung erleichtern. Hiermit soll die Behandlungsqualität für Patienten verbessert werden.
Zusammenfassung Ver?nderungen in der Medizin, wie bspw. der Zuwachs der Wissensbasis und die Verschiebungen in der Epidemiologie, haben naturgem?? auch Auswirkungen auf das Selbstverst?ndnis von Facharztgruppen. Dies und die aktuelle Theoriebildung zu Behinderung und Funktionsf?higkeit hat erheblichen Einfluss auf die Praxis der Physikalischen und Rehabilitativen Medizin (PRM). In dem vorliegenden Artikel soll der T?tigkeitsbereich des Facharztes f?r PRM beschrieben und an klinischen Beispielen demonstriert werden. Nach internationaler Definition umfasst das von Fach?rztinnen und Fach?rzten der PRM betreute Patientenspektrum einen weiten Bereich verschiedener Krankheiten und Funktionsst?rungen. In Deutschland sind die h?ufigsten in der PRM versorgten Symptome muskuloskelettale Funktionsst?rungen, chronische Schmerzen, Erkrankungen des Nervensystems und psychosoziale Problemlagen. Im station?ren Bereich sind es gravierende Funktionsst?rungen nach schweren Unf?llen und komplexen operativen Eingriffen, nach zerebralen Insulten, Sch?del-Hirn-Trauma und Querschnittl?sionen. In der akutstation?ren Versorgung und in der Fr?hrehabilitation liegen die Schwerpunkte auf immobilisations- und muskuloskelettalbedingten St?rungen wie Muskelatrophien, Critical-Illness-Polyneuropathien, schwere Dekonditionierung, Hirnfunktionsst?rungen einschlie?lich Schluckst?rungen sowie Kontrakturen, Schmerzen und Spastizit?t. Das diagnostische Vorgehen umfasst regelhaft die Diagnose der Funktionsst?rung zugrunde?liegenden Erkrankung, die Erfassung von Art und Umfang der Funktionsst?rung, die Erfassung von Art und Umfang der Aktivit?ts- und Teilhabeeinschr?nkung, und die Erfassung der als Kontextfaktoren zusammengefassten Umwelt- und persongebundenen Faktoren. Wie in allen ?rztlichen Fachgebieten ist eine sorgf?ltige Diagnostik, Therapie- und Rehabilitationsplanung eine wesentliche Vo?raussetzung f?r den Erfolg der Interventionen. Entsprechend der Varianz der Zielstellung wird in der PRM ein breites Spektrum von Interventionen angewendet (medikament?se Therapien, physikalisch-medizinische Therapien und rehabilitative Interventionen, Hilfsmittelversorgung und technische Hilfen, edukative und soziale Interventionen) 1. Ein weiteres Grundprinzip ist die interdisziplin?re und multiprofessionelle Zusammenarbeit. Aufgaben- und T?tigkeitsfelder der PRM finden sich im Akutkrankenhaus, in den station?ren, teilstation?ren und ganztagsambulanten Rehabilitationseinrichtungen und im ambulanten/poststation?ren Bereich. Insgesamt zeigt sich, dass sich das Fachgebiet der PRM in Deutschland gut etabliert und entwickelt hat. Dabei besteht ein wachsender Bedarf an koordinierten, auf Basis ?rztlicher Diagnosestellung und sozialmedizinischer Analyse konzipierter Interventionen, und zwar sowohl im ?klassischen? Bereich der Rehabilitation als auch im Akutkrankenhaus und in der Niederlassung.
Most polytraumatized burn injury persons aged 30-40 years who have suffered a burn or a burn-like injury are stigmatized by serious functional deficits, temporary loss of their social independence, injury-associated psychological reactions of different levels, and unusual reversible deficits in their higher cortical functions. The result is a prolonged stage of acute treatment and rehabilitation as well as very often the lifelong need for highly specialized care by physicians and other medical professionals. Accordingly, the deficits in any individual case are high and in spite of the low case rate (approximately 300-400 cases/ year) these accidents have a high impact on society as a whole, resulting in a significant financial burden for the social security system, insurance funds, and pension funds.
