INTRODUÇÃO: Os cuidados com as preservações dos nervos laríngeos recorrentes e das glândulas paratireóides, nas tireoidectomias, continuam desafiando os cirurgiões em razão das graves complicações que podem ocorrer quando são manipulados inadequadamente. O trabalho teve como objetivo, no curso das tireoidectomias totais e parciais, estabelecer protocolo anatômico-cirúrgico das relações anatômicas entre os nervos laríngeos recorrentes com as artérias tireóideas inferiores, identificar-preservar esses nervos e as glândulas partireóides. MÉTODO: Os registros fotográficos durante os trans-operatórios foram obtidos de 79 pacientes submetidos às tireoidectomias totais e parciais (lobectomia total direita ou esquerda e istmectomia), respectivamente, com as identificações dos nervos e artérias bi ou unilateralmente, propondo expor as relações anatômicas entre essas estruturas, operados entre janeiro de 2005 e julho de 2006. RESULTADOS: Os registros fotográficos foram adaptados aos desenhos esquemáticos para estabelecer os principais pontos de referências anatômicas dos nervos laríngeos recorrentes em relação às artérias tireóideas inferiores, totalizando 116 nervos laríngeos recorrentes. CONCLUSÃO: A íntima relação dos nervos laríngeos recorrentes e as artérias tireóideas inferiores podem se apresentar de diversas formas, incluindo as anomalias congênitas, como a do laríngeo não recorrente, as duplicações e triplicações pré-laríngeas do nervo, sem dúvida, algumas vezes, dificultando a identificação dessas estruturas. São imperativas as claras identificações dessas estruturas e das glândulas paratireóides para preservá-las, no curso das tireoidectomias totais e parciais, a fim de evitar as complicações pós-operatórias.
OBJETIVO: Construir um protocolo das relações anatômicas topográficas do ramo externo do nervo laríngeo superior (NLSE) com a artéria tireóidea superior, para minimizar as falhas das propostas descritas na literatura. MÉTODOS: Foram dissecadas bilateralmente as regiões cervicais de 57 cadáveres frescos, no Instituto Médico-Legal do Estado do Amazonas, com identificação fotográfica dos NLSE e respectivas relações com a artéria tireóidea superior. A partir dos dados obtidos elaborou-se classificação: Tipo1: NLSE não identificado; Tipo 2: NLSE cruza a artéria tireóidea superior a distância maior de 1,5cm do limite cranial do lobo da glândula tireóide; Tipo 3: NLSE cruza a artéria entre 1cm e 1,5cm do limite cranial do lobo da glândula tireóide; Tipo 4: NLSE cruza a artéria a menos de 1cm do limite cranial do lobo da glândula; Tipo 5: NLSE cruza, látero-medialmente, abaixo do limite cranial do lobo tireóideo. Como suplemento classificatório, os tipos 2, 3 e 4 receberam subclassificação: A - quando se encontrava parcial ou totalmente envolto no tecido conjuntivo superficial peri-muscular ou intra-muscular, no constritor inferior da faringe, em mais da metade de seu percurso, após cruzar a artéria tireóide superior; B - quando se encontrava fora destes tecidos. RESULTADOS: Tipo 1, 11,88%; Tipo 2, 13,86%; Tipo 3, 34,65%; Tipo 4, 38,61% e Tipo 5, 0,99%. CONCLUSÃO: Os NLSE mais próximos ao limite cranial (Tipos 3 e 4) percorriam, em sua maioria, o tecido frouxo peri-muscular ou intramuscular, no pólo superior da tireóide, o que neste estudo, desobrigam identificação sistemática do nervo nas tireoidectomias (p=0,075).
OBJETIVO: O estudo foi transversal e retrospectivo por meio dos registros fotográficos, nos transoperatórios de 53 tireoidectomias totais e parciais, realizadas entre janeiro de 2002 a agosto de 2006, em pacientes portadores de doenças benignas e malignas da tireóide. RESULTADOS: Foram obtidas imagens de 111 glândulas paratireóides. Das 67 superiores, 65 (97%) estavam envolvidas pelo fino e irregular tecido adiposo peri-glandular. Das 44 inferiores, 41 (93,1%) estavam parcial ou totalmente envoltas pela esparsa gordura aderida à cápsula tireóide. A escolha da via de acesso e o cuidado para evitar hemorragias são dois itens que evitam maiores dificuldades. Acrescem-se os rigores da técnica com a ligadura do pedículo superior, sendo realizada em primeiro lugar, seguida da mobilização do lobo tireóideo em sentido medial. Esses gestos possibilitam a melhor identificação das paratireóides. CONCLUSÃO: É indispensável conservar o tecido adiposo aderido à cápsula tireóidea, onde se alojam mais de 90% das paratireóides. O manuseio cirúrgico dessa delgada lâmina adiposa provoca a mudança da cor das paratireóides, tornando-as amarronzadas, destacando-as entre o amarelo-ouro da gordura e impondo maior atenção ao cirurgião.
