The investigations of the EEG during an open study with the antipsychotic drug fluperlapine in acute schizophrenic patients are reported. Due to ethical and practical considerations some of the common pharmacoelectroencephalographic procedures as well as placebo controlled study designs could not be applied in these patients. To overcome at least partly these limitations, intraindividual as well as interindividual correlations were used. They were computed between plasma concentrations of unchanged fluperlapine as well as its metabolite N-oxide fluperlapine, on one hand, and EEG variables, on the other. The intraindividual correlations can be computed either on the first day of the active treatment over various time points of that day (acute effects) or across several appointed treatment days always taking values at the same time during these days (chronic effects). The intraindividual correlations of a set of subjects were submitted to a sign test to obtain an overall result for the relation between the EEG and blood plasma levels of the drug. In this way an acute and a chronic effect of fluperlapine on the EEG could be shown consisting mainly of an increase in slow waves, a decrease in the α-activity and a tendency of β-activity to decrease. A comparison of the correlations between the plasma levels of fluperlapine and the EEG variables with the correlations between the plasma levels of N-oxide fluperlapine and the EEG give rise to the hypothesis that unchanged fluperlapine has a stronger effect on the EEG than its metabolite.
Seit Entdeckung der Steilen Potentiale im EEG wird nach ihrer diagnostischen Bedeutung für die Erkennung von epileptischen Anfallsleiden gefragt. Anhand zweier EEG-Dokumentationen aus Berlin (5001 Erst-EEGs) und Hannover (7094 Erst-EEGs) wird die bedingte Wahrscheinlichkeit für ein epüeptisches Anfallsleiden bei Vorliegen von Steilen Potentialen im EEG berechnet. Es zeigt sich, daß nach Plausibilitäts- und Validitätskorrektur der Schätzwert für die bedingte Wahrscheinlichkeit von 45% auf 63% ansteigt. Die mathematisch-statistischen Korrekturmöglichkeiten werden exemplarisch dargestellt. Die subjektiven Schätzungen anhand einer computergestützten Schätzungsprozedur durch 6 EEG-Experten ergaben im Mittel einen Schätzwert von 63% für die bedingte Wahrscheinlichkeit. Es besteht eine gute Übereinstimmung der Expertenschätzung mit dem korrigierten Schätzwert aus der Dokumentation. Das Ergebnis zeigt, daß etwa 1/3 aller Patienten mit Steilen Potentialen im Erst-EEG kein epileptisches Anfallsleiden haben. Bei Männern ist die Sensitivität von Steilen Potentialen (69%) höher als bei Frauen (58%).
Leitet man ein EEG über ca. 20 Minuten ab und unterwirft es zwecks Untersuchung von Kurzzeiteffekten in Abschnitten von wenigen Sekunden einer Spektralanalyse, so erhält man eine Reihe von mehreren 100 Spektren. Dies ist eine Datenfülle, die am besten in dreidimensionaler Darstellung anschaulich gemacht wird. Damit rhythmische Veränderungen der Grundaktivität, z.B. Schwankungen der Alpha-Aktivität (7,5—12,5 Hz Bereich), nicht in der beträchtlichen Varianz der einzelnen Spektralschätzer untergehen, werden die Stützpunkte mit Hilfe einer digitalen Filterung nacheinander einer Quer- und einer Längsglättung unterworfen.Ein besonderes Kennzeichen der hier präsentierten 3D-Darstellung ist, daß bei Eingabe verschiedener Dreh- und Kippwinkel die Zeitachse stets parallel zur Papierkante des Plotters verläuft, so daß dem Benutzer bzgl. der maximalen Anzahl der darstellbaren Spektren keine Beschränkungen auferlegt werden.
