in a population with different socioeconomic limitations, RYGB maintained satisfactory results regarding weight loss, but inefficient follow-up may compromise the final result, especially with regard to nutritional deficiencies.

Morbid obesity is considered a serious public health problem generating high human and financial costs. The situation that promotes the highest morbidity after bariatric surgery is the development of cholelithiasis, especially in the first 6 months. Given this process, ursodeoxycholic acid is increasingly being used in the postoperative period due to a reduction in the rate of cholelithiasis. However, there are discussions of which prophylactic method to use. In this context, a systematic review was performed to evaluate the efficacy and use of ursodeoxycholic acid in preventing cholelithiasis after bariatric surgery. After methodological analysis, 6 articles were selected. These studies suggest that administration of 500 to 1000 mg of UDCA for a period of 6 months, depending on the technique used, is significantly associated with decreased incidence of cholelithiasis, with good adherence, costeffectiveness and few side effects.

Resumo: Relato de caso de um paciente com hérnia diafragmática direita crônica. Paciente em emergência de hospital referência de trauma com obstrução intestinal grave. Hérnia diafragmática pós-traumática é uma injúria pouco encontrada na prática médica, principalmente quando acomete o hemidiafragma direito. As rupturas do lado direito estão associadas a alta morbi-mortalidade e, portanto, o subdiagnóstico de lesões à direita pode ser devido a uma maior mortalidade pré-hospitalar ou a um atraso no diagnóstico intra-hospitalar, muitas vezes em consequência do grau variável de acometimento traumático. A ruptura diafragmática contusa, por ser uma condição pouco frequente, deve ter alto índice de suspeição nos pacientes vítima de trauma tóraco-abdominal, independente do mecanismo de trauma. A associação com lesões múltiplas e graves é frequente, apresentando considerável taxa de morbi-mortalidade, devido aos riscos de complicações imediatas e tardias envolvidas no atraso diagnóstico.

Objetivo: analisar os fatores relacionando complicações da Apendicite Aguda (AA) com a pandemia de coronavírus (COVID-19) dos pacientes atendidos em um hospital terciário da macrorregião do Cariri (HRC). Métodos: estudo transversal e retrospectivo, comparando os atendimentos por AA no ano de 2019 com o ano de 2020. As variáveis elencadas foram número de dias com dor, idade, fase evolutiva, incisão cirúrgica, uso de dreno, tipo de acesso cirúrgico, internamento em UTI, uso de antibióticos, tempo de internação e complicações. Resultados: a AA foi mais prevalente no adulto jovem. Houve aumento do tempo médio para busca hospitalar, dos casos de AA Complicada e de incisões medianas durante a pandemia de COVID-19. A técnica operatória e o uso de drenos não sofreram influências no período em estudo. O uso de antibioticoterapia foi mais prevalente no ano de 202o, bem como o internamento em UTI, infecção de ferida operatória e hemicolectomia. Conclusão: a pandemia de COVID-19 determinou demora na procura médica resultando em tratamento tardio, elevação na permanência hospitalar e infecções de sítio cirúrgico.

This article aims to evaluate the effectiveness and use of laparoscopy in penetrating abdominal trauma, thus providing a better definition of the role of laparoscopic approach in patients with PAD.  A systematic review was performed by searching indexed articles in the Virtual Health Library and the Medical Literature Analysis and Retrieval System Online database (MEDLINE / PubMed), in which analysis after final analysis included 07 articles.  In this study we identified seven articles that evaluated the use and contributions of the laparoscopic approach in patients with TAP, in which it was identified that the use as a diagnostic approach is widely used due to its benefits and safety.  Regarding its therapeutic approach it was determined that in the largest of the studies there is indication, usefulness, reliability and accuracy of its use, however, for better results is essential experience of the surgical team in laparoscopic technique.Keywords: Abdominal trauma;  Laparoscopy and Penetrating Trauma;  Abdominal injury;  Laparoscopy;  Penetrating wound.
