TEMA: impacto da tontura na qualidade de vida (QV). OBJETIVO: adaptar culturalmente o Dizziness Handicap Inventory (DHI) para aplicação na população brasileira, avaliar sua reprodutibilidade e descrever os resultados obtidos à aplicação deste questionário em pacientes com tontura crônica. MÉTODO: o DHI foi aplicado, inicialmente, em 45 pacientes com tontura crônica e hipótese diagnóstica de síndrome vestibular, seguindo as etapas de tradução do idioma Inglês para o Português e adapatação lingüística, revisão da equivalência gramatical e idiomática, adaptação cultural e avaliação da reprodutividade intra e interpesquisadores. A avaliação da reprodutividade foi realizada por intermédio do teste de pesquisadores. A avaliação da reprodutividade foi realizada por intermédio do teste de Wilcoxon para duas amostras dependentes, P < 0,05. O instrumento foi aplicado para um total de 250 pacientes para avaliação do impacto da tontura na qualidade de vida de vestibulopatas crônicos. RESULTADOS: a versão brasileira do DHI (DHI brasileiro) foi bem compreendida pelo população estudada. Não foi verificada diferença estatística significante à avaliação da reprodutibilidade inter-pesquisadores (P = 0,418) e intra-pesquisadores (P = 0,244). Todos pacientes apresentaram prejuízo na QV e os aspectos físicos foram os mais prejudicados, seguidos em ordem decrescente pelos aspectos funcionais e emocionais. Os aspectos funcionais mostraram-se mais comprometidos em indivíduos mais velhos. Nenhuma associação foi verificada entre o gênero e as médias do escore total e de cada um dos aspectos avaliados pelo DHI. CONCLUSÃO: o DHI foi adaptado culturalmente para aplicação na população brasileira (DHI brasileiro), mostrando-se confiável para a avaliação da interferência da tontura na QV. Os pacientes com tontura crônica e hipótese diagnóstica de síndrome vestibular apresentaram prejuízo na QV devido a este sintoma, verificados à aplicação do DHI brasileiro. Os aspectos físicos foram os mais prejudicados.
The THI was translated and culturally adapted in order to be used with the Brazilian population, demonstrating to be a reliable instrument to verify the damage caused by tinnitus in the quality of life of individuals.
Tarefas do dia-a-dia podem ser muito desafiadoras para o equilíbrio do idoso. OBJETIVO: Verificar a associação entre equilíbrio funcional, avaliado pela Berg Balance Scale (BBS) e os dados sociodemográficos, clínicos e de mobilidade (Timed up and go test - TUGT, Dynamic Gait Index - DGI) em idosos vestibulopatas crônicos. MATERIAL E MÉTODO: Estudo de casos com 120 idosos com diagnóstico de disfunção vestibular crônica. Foram utilizados os testes de Mann-Whitney, Kruskal-Wallis seguido do teste de Dunn e Coeficiente de Correlação de Spearman. RESULTADOS: Ocorreram associações =-0,354; e correlações significantes entre a pontuação total da BBS e idade (p<0,001), faixa etária (p<0,001), número de doenças (p=0,030), número de =-0,287; p=0,001), número de medicamentos (p=0,014), número de doenças (=-0,274; p=0,002), quedas recorrentes (p=0,010), tendência a medicamentos (quedas (p=0,002), diagnóstico topográfico de vestibulopatia central (p<0,001) =-0,709; periodicidade da tontura (p=0,039), TUGT (<0,001) e DGI =-0,748; p(<0,001). CONCLUSÕES: O equilíbrio funcional de idosos vestibulopatas crônicos avaliados à BBS é mais comprometido quando associado ao avançar da idade, faixa etária mais idosa (80 anos ou mais), aumento do número de doenças, presença de cinco ou mais doenças, polifarmacoterapia, quedas recorrentes, tendência a quedas, vestibulopatia central, tontura diária, comprometimento da mobilidade e marcha.
A tontura é considerada um dos sintomas mais comuns entre adultos, podendo interferir na qualidade de vida dos pacientes que apresentam este sintoma. Objetivos: Descrever os resultados obtidos à aplicação do DHI brasileiro na população estudada e compará-los com a conclusão do exame vestibular dos respectivos pacientes. Aim: To describe the results obtained from the application of the Brazilian version of the DHI and compare them with the conclusion of the vestibular test of the same patients. Study design: Clinical randomized. Material Method: Twenty five consecutive patients with chronic dizziness complaint were submitted to this questionnaire at the Otoneurology Clinic of the Universidade Federal de São Paulo. These patients were female and male adults with ages ranging from 44 to 88 years. Results: All patients presented handicap in their quality of life due to dizziness, mainly in the functional aspects. The functional aspects were worse (differences were statistically significant) in patients with peripheral vestibular hypofunction syndrome than the patients with peripheral vestibular hyperfunction syndrome. Conclusions: 1. Patients with chronic dizziness present abnormal quality of life because of dizziness, in relation to the physical, functional and emotional aspects, verified with the application of the Brazilian version of the DHI. Patients with peripheral vestibular hypofunction syndrome present worse quality of life because of dizziness related to the functional aspects than the patients with peripheral vestibular hyperfunction syndrome, verified at the application of the Brazilian version of the DHI.Palavras-chave: tontura, vertigem, qualidade de vida, síndromes vestibulares.
A farmacoterapia é opção importante no tratamento das vestibulopatias periféricas. OBJETIVO: Identificar a medicação que otimiza a terapia integrada da vertigem (TIV) na doença de Ménière e em outras vestibulopatias periféricas. MATERIAL E MÉTODO: Estudo de casos em que pacientes com doença de Ménière ou outras vestibulopatias periféricas receberam TIV com betaistina, cinarizina, clonazepam, flunarizina, Ginkgo biloba ou sem medicação durante 120 dias. RESULTADOS: Na doença de Ménière, TIV com qualquer um dos medicamentos foi mais eficaz do que TIV sem medicação, após 60 dias; a betaistina foi mais efetiva que todas as outras drogas, após 60 e 120 dias. Nas outras vestibulopatias periféricas, diferenças significantes foram observadas entre TIV com betaistina, cinarizina, clonazepam ou flunarizina e TIV sem medicação após 60 dias e todas as drogas foram mais efetivas que TIV sem medicação após 120 dias; betaistina, cinarizina ou clonazepam foram igualmente efetivos e betaistina foi mais efetiva que flunarizina e Ginkgo biloba. Os tratamentos foram bem tolerados. CONCLUSÕES: TIV incluindo medicação é mais efetiva que sem medicação na doença de Ménière ou em outras vestibulopatias periféricas. Betaistina foi o medicamento mais efetivo na doença de Ménière e tão eficaz quanto cinarizina ou clonazepam em outras vestibulopatias periféricas.
Summary Benign Paroxysmal Positional Vertigo (BPPV) can cause falls, especially in the elderly. Aim to study whether or not elderly patients with BPPV have a reduction on their falls after the particle repositioning maneuver (PRM). Materials and Methods retrospective study including elderly with BPPV who had fall(s) during the last year. All patients were submitted to the PRM according to the affected semicircular canal (SCC). After the abolition of positioning vertigo and nystagmus, the patients were submitted to a 12 month follow-up and were investigated about the number of fall(s). Wilcoxon's test was performed to compare the number of fall(s) before and after 12 months of the PRM. Results One hundred and twenty one patients were included in the study. One hundred and one patients presented involvement of the posterior SCC, 16 of the lateral and four of the anterior. We noticed a reduction on the number of falls, with statistically significant difference when all the patients were analyzed together (p<0.001), the posterior canal BPPV patients (p<0,001) and the lateral canal VPPB patients (p=0.002). We also found a tendency of statistically significant difference for the anterior canal BPPV patients (p=0.063). Conclusion BPPV elderly patients had indeed a reduction on the number of falls after the PRM.
The prevalence of dizziness in São Paulo was found to be 42%. It affects daily activities in 67% of symptomatic patients, but only 46% of them seek medical help.
