FORMA DE ESTUDO: Estudo de corte transversal. MATERIAL E MÉTODOS: Os autores avaliaram 55 pacientes que procuraram o Ambulatório de Otoneurologia Geriátrica do Hospital das Clínicas do HCFMUSP com queixa de desequilíbrio e/ou tontura entre agosto de 2001 e março de 2003, caracterizando o perfil dessa população de idosos quanto a idade, sexo, fatores etiológicos relacionados à alteração do equilíbrio, conduta terapêutica adotada e resultados parciais de tratamentos propostos. RESULTADOS: O paciente idoso com alteração do equilíbrio corporal apresentou mais de uma etiologia relacionada ao seu problema em 51% dos casos, sendo as patologias mais freqüentes: Insuficiência Vértebro-Basilar - IVB (40%), Alteração Metabólica relacionada a metabolismo de açúcar e colesterol (40%), Síndrome do Desequilíbrio de Idoso - SDI (30%), Vertigem Posicional Paroxística Benigna - VPPB (14,5%), Vestibulopatia Cervical (7,2%) e Alterações Hormonais (5,4%), entre outras. CONCLUSÃO: Estes achados corroboram dados prévios da literatura e chamam a atenção para a necessidade de uma abordagem multidisciplinar para a compreensão do desequilíbrio e seu adequado tratamento na população idosa.

Vest ibular diseases due to glucose metabolic disorders (GMD) are controversial issues in the medical literature because of lack of objective measures to define cause-effect. Aim: To describe the results of Sensory Organization Test (SOT) in dizziness patients presenting GMD and treated with proper diet. Study Design: Retrospective. Material and Method: Twenty-one dizziness patients presenting GMD and treated with proper diet. To assess the balance function, the patients underwent the Sensory Organization Test from Computerized Dynamic Posturography. Results: The vestibular condition in SOT -V condition -and the equilibrium score (ES) were the two parameters improved in GMD patients treated with proper diet. Conclusion: SOT is a useful test to assess balance control in GMD patients presenting dizziness.

MATERIAL E MÉTODO: Os autores avaliam a prevalência de alterações metabólicas em 325 pacientes que procuraram o Setor de Otoneurologia do Hospital das Clínicas da FMUSP com queixas vestibulares. Os pacientes foram consecutivamente atendidos entre janeiro de 1997 e janeiro de 2002. Foram avaliados os primeiros exames referentes ao protocolo clássico de investigação das tonturas, que compreende a dosagem de LDL colesterol, TSH, T3 e T4 e glicemia de jejum. As alterações encontradas foram então comparadas às observadas na população geral. As alterações mais significativas encontradas foram a elevação dos níveis de LDL colesterol, a presença de níveis alterados de hormônios tireoideanos e a maior freqüência do diabetes mellitus na população estudada. CONCLUSÃO: Conclui-se que é válida a investigação destes dados, visto que são mais freqüentes na população de risco e que a correta abordagem dessas alterações melhora o prognóstico e o controle da tontura.
OBJETIVO: Considerando o aumento da população idosa e conseqüentemente as alterações de equilíbrio a ela relacionadas, realizamos um estudo para avaliar o impacto do tratamento adequado das doenças coexistentes ao desequilíbrio corporal no resultado da Reabilitação Vestibular (RV). MATERIAL E MÉTODOS: 52 idosos com queixa de tontura e/ou desequilíbrio com indicação RV participaram deste ensaio clínico aberto e prospectivo realizado no Ambulatório de Otoneurologia Geriátrica da Instituição, entre 2003 e 2005. Os pacientes do grupo de estudo foram previamente tratados das doenças clínicas diagnosticadas e posteriormente submetidos à RV. Foram então comparados com o grupo total de idosos tratados pela RV no mesmo período. RESULTADOS: Os pacientes do grupo de estudo apresentaram 65 comorbidades diagnosticadas, com média de 1,25 por paciente. A efetividade total foi de 84,5% neste grupo contra 81,8% no grupo controle, sem diferença significante. No entanto, a completa remissão dos sintomas ocorreu em 69,2% dos casos contra 43,18% dos controles, significante para p<0,02. CONCLUSÃO: A diferença na efetividade da RV entre os grupos demonstra a importância do tratamento etiológico das afecções clínicas coexistentes aos problemas de equilíbrio corporal.
