Objective: To validate and assess reliability and understanding of the EORTC-C30 quality of life questionnaire and its breast cancer specific module, the EORTC-BR23. Methods: This study was conducted at the AC Camargo Cancer Hospital, São Paulo, Brazil. A total of 100 women diagnosed with breast cancer were interviewed. Internal consistency, confirmatory factorial analysis, convergent validity, construct validity and degree of understanding were examined. Reliability was assessed by comparison of means at times 1 and 2, inter-class coefficient and Bland-Altman graphics. Results: Cronbach's alpha ranged from 0.72 to 0.86 for the EORTC-C30 and from 0.78 to 0.83 for the EORTC-BR23 questionnaire. Most questions were confirmed in the confirmatory factorial analysis. In the construct validity analysis, the questionnaires were capable of differentiating patients with or without lymphedema, apart from the symptom scales of both questionnaires. Both questionnaires presented a significant correlation in most domains of the SF-36, in the convergent validity analysis. Only a few criticisms were reported concerning questions, and the mean grade of understanding was high (C30 = 4.91 and BR23 = 4.89). The questionnaires presented good rates of reliability, with the exception of the functional scale of the C30 and the symptom scale of the BR23. Conclusions: The EORTC-C30 and EORTC-BR23 quality of life questionnaires were validated, presented good rates of reliability and are easily understood, allowing them to be used in Brazil to assess quality of life among women with breast cancer. Background:Heterocyclic amines (HCAs) in cooked meats may play a role in colorectal cancer (CRC) development.Objectives:We aimed to prospectively examine the association between estimated intakes of HCAs and meat-derived mutagenicity (MDM) in two cohorts of health professionals, the Health Professionals Follow-up Study (HPFS) and the Nurses’ Health Study (NHS).Methods:In 29,615 men and 65,875 women, intake of the HCAs 2-amino-3,8-dimethylimidazo(4,5-j)quinoxaline (MeIQx), 2-amino-1-methyl-6-phenylimidazo(4,5-b)pyridine (PhIP), 2-amino-3,4,8-trimethylimidazo(4,5-f)quinoxaline (DiMeIQx), and MDM was estimated using a 1996 cooking questionnaire, the 1994 food frequency questionnaire, and an online database. Cox proportional hazards models were used to estimate hazard ratios (HRs) and 95% confidence intervals (CIs) and to adjust for potential confounders. Estimates for both cohorts were pooled using random-effects meta-analysis.Results:Between 1996 and 2010, 418 male and 790 female CRC cases were identified. Meat mutagen intake was not statistically significantly associated with risk of CRC [highest vs. lowest quintile, pooled HR (95% CI) for MeIQx: 1.12 (0.93, 1.34), p for trend 0.23; PhIP: 1.10 (0.90, 1.33), p for trend 0.35; MDM: 1.03 (0.86, 1.24), p for trend 0.75] or subtypes of CRC defined by tumor location (proximal or distal colon, or rectum). When analyzed by source of meat, PhIP from red but not from white meat was nonsignificantly positively associated with CRC and significantly positively associated with proximal cancers [HR (95% CI) per standard deviation increase of log-transformed intake: PhIP red meat: CRC: 1.06 (0.99, 1.12), proximal: 1.11 (1.02, 1.21); PhIP white meat: CRC: 0.99 (0.94, 1.04), proximal: 1.00 (0.93, 1.09)].Conclusions:Estimated intakes of meat mutagens were not significantly associated with CRC risk over 14 years of follow-up in the NHS and HPFS cohorts. ABSTRACT:The availability of large computerized databases on health turned the record linkage technique into an alternative for different study designs. This technique provides the creation of more complete databases, at low operational costs. Objective: The aim of this study was to improve the quality of information and data completeness through probabilistic and deterministic record linkage between Population-based Cancer Registry of São Paulo (PBCR-SP) for incident cancer cases, death database and drugs/medical procedures database. Methods: We used the database of the PBCRP-SP composed of 343,306 incident cancer cases in the municipality of São Paulo in the period between 1997 and 2005 with ages raging from under 1 to 106 years old, from both sexes. Three databases were used for linkage, namely Improvement Program for Mortality Information in São Paulo city (PRO-AIM), Authorization of Procedures of High Complexity/Cost of Outpatient Information System from the Unified Health System (APAC-SIA/SUS), and Foundation State System of Data Analysis (Foundation SEADE). Crude incidence (CIR) and mortality rates (CMR) were calculated and overall survival analysis was performed using the Kaplan-Meier method. Results: After record linkage, it was possible to observe gain of 4.3% for the CIR and 25.8% for CMR. The overall survival analysis showed that before record linkage there was an underestimation of the probability of being alive for all variables (p < 0.001). Conclusion: The linkage techniques contributed with the improvement of the quality of RCBP-SP information both on completeness of data, as in defining the vital status of the patient. In addition, the results found in this study reflect the ability of databases when worked jointly, providing subsidies for various types of studies and information for planning policies and strategic actions.
A ocupação do espaço geográfico é determinada historicamente pelo modelo socioeconômico e pelo dinamismo de suas relações sociais, políticas e ideológicas. O objetivo deste trabalho é avaliar a distribuição espacial e o efeito de indicadores socioeconômicos no adoecimento e morte por câncer de boca e orofaríngeo no Município de São Paulo, Brasil, no período de 1997 a 2008. Os dados foram coletados no Registro de Câncer de Base Populacional e no Programa de Aprimoramento das Informações de Mortalidade - PRO-AIM e georreferenciados pelos softwares Terraview e GeoDa. O referencial teórico para avaliação dos resultados foi baseado na teoria de Milton Santos. As taxas de incidência apresentaram um índice de autocorrelação Global de Moran de 0,226 e as taxas de mortalidade de 0,337. A Incidência de câncer de boca e orofaríngeo não apresenta um padrão espacial bem definido no Município de São Paulo, mas é bastante desigual no que se refere à Mortalidade, concentrando as suas menores taxas na área central, mais rica e economicamente menos desigual.

Neste estudo foi utilizada a base de dados do RCBP-SP, composta por 343.306 casos incidentes de câncer do município de São Paulo, registrados no período de 1997 a 2005, com idades que variaram de menos um a 106 anos, de ambos os sexos. Para o linkage probabilístico, realizado no programa Reclink III, foram utilizadas a base de dados do PRO-AIM e APAC-SIA/SUS. Foram calculados os valores da área sob a curva, sensibilidade e especificidade para determinar o ponto de corte do escore de maior precisão na identificação dos pares verdadeiros. Na análise das topografias, verificou-se que o ponto de corte no escore 18 apresentou boa acurácia, com valores de sensibilidade que variaram de 73,7 a 96,7% e de especificidade de 98,5 a 99,4%. Conclui-se que, acima do escore 18 encontravam-se quase a totalidade dos pares verdadeiros, enquanto que abaixo deste, menos de 1% dos registros vinculados eram verdadeiros.Palavras-chave: sistemas de informação; registro médico coordenado; neoplasias. AbstractThe aim of this study was to propose cut-off points for scores calculated in the probabilistic record linkage process for several cancer topographies. In this study we used the PBCR-SP database composed of 343,306 incident cancer cases from the municipality of São Paulo, registered from 1997 through 2005, aged from less than one to 106 years, of both sexes. PRO-AIM and APAC-SIA/SUS databases were used to probabilistic record linkage using Reclink III software. Area under the curve, sensitivity and specificity values were calculated to determine the cut-off point with the highest accuracy in identifying true matches. In the topography analyses, it was found that the cut-off at score 18 showed good accuracy, with sensitivity ranging from 73.7 to 96.7% and specificity ranging from 98.5 to 99.4%. We concluded that above score 18 nearly all true pairs were found. Whereas, below this cut-off, less than 1% of linked records were true matches.
Resumo: Introdução: O câncer de mama representa 25% de todos os tipos de câncer, sendo o de maior incidência na população feminina. Com o avanço dos tratamentos e diagnósticos precoces, as taxas de sobrevida têm aumentado e tal contexto reflete nos afastamentos laborais, tendo em vista que a maioria das mulheres se encontra em idade produtiva. Os principais motivos responsáveis pelos afastamentos do trabalho são as complicações físicas decorrentes do tratamento cirúrgico. Objetivo: Objetivou-se investigar as implicações cirúrgicas do tratamento do câncer de mama na atividade profissional, caracterizar o número de afastamentos laborais e investigar a relação entre o tipo de cirurgia nos afastamentos do trabalho e seus principais motivos. Método: Estudo transversal envolvendo 74 mulheres diagnosticadas com câncer de mama. Para a coleta dos dados, foi utilizada uma entrevista semiestruturada, que abordou as questões clínico-cirúrgicas, os dados sociodemográficos, o comportamento laboral e o tratamento fisioterapêutico. Os dados foram organizados no Microsoft Excel mediante frequências e o teste de qui-quadrado, sendo considerado p ≤ 0,05. Resultados: O lado esquerdo foi o mais acometido pela doença (51%), a cirurgia radical modificada do tipo Madden foi a mais comum (50%) e 93,2% submeteram-se à linfadenectomia axilar. As complicações operatórias mais frequentes foram dor, problemas cicatriciais, alteração da sensibilidade, limitação da ADM, linfedema e seroma. Apenas 58% delas fizeram fisioterapia e 60% afastaram-se das atividades profissionais, 23% abandonaram o trabalho, 26% modificaram as funções do trabalho e 14% se aposentaram por motivo de doença. Conclusão: Este estudo sugere a existência de uma relação direta do tratamento sobre os afastamentos laborais.
Introdução: A incidência de câncer de tireóide vem aumentando em todo o mundo e não há um consenso sobre as razões deste fato. O município de São Paulo apresenta altos coeficientes de incidência desta doença, mas ainda não foi analisada sua tendência e nem sua distribuição espacial. Objetivos: Descrever os coeficientes de incidência (1997-2010) e de mortalidade (1981-2010), analisar a tendência dos coeficientes de incidência e mortalidade, segundo sexo, faixa etária, tipo morfológico (incidência), bem como os efeitos da idade, período e coorte, e examinar a distribuição espacial. Métodos: Este é um estudo ecológico. Foram analisados os casos novos de câncer de tireóide diagnosticados no período de 1997 a 2010 fornecidos pelo Registro de Câncer de Base Populacional de São Paulo e os óbitos por câncer de tireóide ocorridos entre 1981 e 2010 fornecidos pelo Sistema de Mortalidade do Ministério da Saúde (SIM-MS) e pelo Programa de Aprimoramento das Informações de Mortalidade (PRO-AIM). Foram calculados os coeficientes bruto e padronizado de incidência e de mortalidade, foi analisada a tendência destes coeficientes através do modelo de regressão, da mudança percentual anual e do modelo idadeperíodo-coorte. Para a análise espacial foram criados mapas temáticos, calculado o índice de Moran e, para as variáveis com padrão cluster foi calculado o índice local de associação espacial (LISA) e estimados modelos de regressão, tendo como variável dependente os coeficientes e como variáveis independentes os indicadores socioeconômicos (IDH, taxa de alfabetização, coeficiente de Gini e número de moradores por domicílio). Resultados: O coeficiente médio de incidência (1997-2010) foi para o sexo feminino de 17,77 por 100.000 e para o sexo masculino 4,46 por 100.000. Ambos apresentaram tendência crescente. O coeficiente médio de mortalidade (1981-2010) foi para o sexo feminino de 0,50 por 100.000 e para o masculino 0,30 por 100.000, ambos apresentaram tendência decrescente. O tipo histológico papilífero apresentou tendência crescente em ambos os sexos. Para incidência (ambos os sexos) e para mortalidade feminina, os efeitos de idade-período e coorte ofereceram o melhor ajuste; para mortalidade apenas a idade. Na análise espacial incidência apresentou padrão cluster para homens e mulheres. Os modelos finais foram explicados pela índice de desenvolvimento humano e pela média de moradores por domicílio. Conclusão: Na cidade de São Paulo há um aumento da incidência do câncer de tireóide, possivelmente causado pelo diagnóstico precoce e/ou pela exposição a fatores de risco para o desenvolvimento do carcinoma papilífero. Por outro lado, a mortalidade vem decaindo, provavelmente pelo diagnóstico precoce.
Results During the study period, 39 676 scans were recorded on 21 089 patients. The number of scans and patients scanned differed in relation to quintiles of deprivation, with increasing numbers of scans and patients associated with increasing area-level deprivation. Significant associations were also seen between deprivation and age at scan, age at first scan, type of CT scan, and the number of scans per patient. Conclusion Social inequalities exist in the numbers of young people undergoing CT scans with those from deprived areas more likely to do so. This is likely to reflect the rates of injuries in these individuals and implies that certain groups within the population may receive higher radiation doses than others due to medical procedures. Introduction Conflicting evidence exists for associations between birth weight and childhood physical activity (PA) levels. It is important to know what other, potentially modifiable, factors influence PA in children given its' association with adiposity. Our aim was to identify predictors of childhood PA levels in the Gateshead Millennium Study (GMS), a population based cohort of 1029 infants born in 1999e2000 in Gateshead, Northern England. Methods Throughout infancy and early childhood, detailed information was collected. Assessments at age 9 yrs included body composition, objective measures of habitual PA (using accelerometers during waking hours). Mean total volumes of PA (accelerometer count per minute, cpm) and moderate-vigorous intensity PA (MVPA), and the percentage of time spent in sedentary behaviour (% SB) were quantified and direct and mediating associations analysed within path models. Results Significant differences were seen in all three outcomes between males and females (p<0.001). No direct significant associations were seen with birth weight. Increased paternal age was associated with significant increases in % SB and decreases in cpm and MVPA (p<0.033). Associations with BMI at 9 yrs were in the expected directions. Increased time spent in sports clubs was significantly associated with decreased % SB (p¼0.02) and increased MVPA (p¼0.01), but not cpm (p¼0.13). Conclusions Although we found no evidence for an effect of birth weight on PA, path models suggest indirect effects mediated through BMI. Having an older father appeared to have a negative impact on the child's PA levels, while participation in sports clubs increases time spent in MVPA, but not cpm. P1-492
