IntroduçãoHELLP é a sigla usada para descrever a condição de paciente com pré-eclâmpsia grave que apresenta hemólise (H), níveis elevados de enzimas hepáticas (EL) e contagem baixa de plaquetas (LP).A síndrome HELLP é o achado extremo do espectro de alterações que ocorrem na hipertensão induzida pela gestação/pré-eclâmpsia 7 e como seus sinais e sintomas são confundidos com os da préeclâmpsia grave (dor epigástrica ou no quadrante superior direito, náusea e mal estar), as formas leves podem passar desapercebidas se não é feita a correta avaliação laboratorial. Assim, o diagnóstico é feito apenas quando a síndrome HELLP está bastante avançada 6 . Essa síndrome se acompanha de aumento da morbidade e SUMÁRIO A síndrome HELLP é uma complicação grave da pré-eclâmpsia que determina aumento de morbidade e mortalidade maternas e perinatais. São descritos dois casos de recorrência dessa síndrome, sendo que em um deles ocorreu morte materna. Este trabalho alerta para o risco aumentado de síndrome HELLP na gestação seguinte. A morbidade e mortalidade maternas são dependentes da gravidade da doença, ao passo que a morbidade e mortalidade perinatais dependem mais da idade gestacional 4,5,9 . Enquanto em centros de cuidados terciários a mortalidade materna é de apenas 1 a 2% 3,10 , em outros centros é mais elevada, oscilando ao redor de 24% 8 . Alguns trabalhos da literatura afirmam que pacientes com história de síndrome HELLP provavelmente apresentam risco significativo de desenvolvimento da pré-eclâmpsia, com ou sem síndrome HELLP, na próxima gestação. Sullivan et al. 12 analisaram, retrospectivamente, a gestação seguinte de 507 casos de síndrome HELLP, encontrando recorrência da mesma em 19% dos casos. Considerando os casos em que a contagem de plaquetas tinha sido menor que 50.000/mm 3 , 44% manifestaram a síndrome novamente. Sibai et al. 11 analisando o prognóstico materno em relação à gestação seguinte de 341 mulheres que tiveram síndrome HELLP encontraram 4% de risco de recorrência dessa complicação.
