Objectifs : Dans ce travail, les auteurs présentent leur expérience de cancer de la tête du pancréas, vu aux urgences, à propos de 85 cas colligés au service des urgences chirurgicales viscérales, durant une période de six ans (2000)(2001)(2002)(2003)(2004)(2005)(2006). Matériels et méthodes : Il s'agit d'une étude rétrospective sur une période étalée du 1 er janvier 2000 au 30 juin 2006, le service des urgences chirurgicales viscérales a pris en charge 85 malades porteurs du cancer de la tête du pancréas diagnostiqué sur les données de l'imagerie ou sur des constatations peropératoires. Résultats : L'âge moyen de nos malades, au moment du diagnostic, est de 60 ans. Il s'agissait de 54 hommes et de 31 femmes. L'ictère était le premier signe amenant le malade à consulter. La majorité de nos patients était admise dans un tableau d'angiocholite (57,7 %), avec un état général souvent altéré. L'échographie abdominale a apporté le diagnostic de cancer de la tête du pancréas chez 45 malades (52,9 %). La tomodensitométrie abdominale a été réalisée chez tous nos patients, et elle a montré une tumeur de la tête du pancréas dans la majorité des cas (98 %), dont 55 % avaient une tumeur dont la taille était entre 3 et 6 cm. Sur le plan thérapeutique, il y avait sept cas d'abstention chirurgicale (8,2 %), huit laparotomies exploratrices (9,4 %), 48 dérivations biliaires (55,3 %) et 17 duodéno-pancréatectomies céphaliques (20 %). Les suites postopératoires immédiates ont été marquées par l'apparition de complications chez six patients. Par ailleurs, le taux de mortalité postopératoire était de 9,7 % (sept cas). Conclusions : Le cancer de la tête du pancréas est une affection de mauvais pronostic, car son diagnostic est souvent tardif, où tout geste d'exérèse chirurgicale est impossible. La chirurgie demeure le seul traitement potentiellement curatif.
