Los errores innatos del metabolismo representan un problema emergente de salud pública que requiere estrategias puntuales y esfuerzos continuos. Actualmente, el tamiz neonatal es el único método para la detección temprana de este tipo de enfermedades porque permite identificar a los recién nacidos afectados antes de que se genere el daño irreversible que causan estas enfermedades. En los últimos seis años, el panel de detección en la Secretaría de Salud ha pasado de 1 a 6 enfermedades. El financiamiento del programa ha sido fundamental para el desarrollo del tamiz neonatal porque lo ha garantizado como proceso integral, incrementando gradualmente los recursos asignados a esta política pública. La contratación de servicios especializados e integrales ha permitido una mejora sustancial en los indicadores del tamiz neonatal, agilizó la entrega de resultados, garantizó el acceso a pruebas confirmatorias al 100% de casos sospechosos en menos tiempo, lo que en conjunto ha conseguido reducir el tiempo de inicio del tratamiento y, por tanto, la limitación del daño o prevención de la discapacidad.PALABRAS CLAVE: Tamiz metabólico; errores innatos del metabolismo
INTRODUCCIÓN: El hipotiroidismo congénito es la causa más común de discapacidad intelectual susceptible de prevención. En México, el tamiz neonatal de esta enfermedad es obligatorio. La prevalencia al nacimiento de hipotiroidismo congénito fue descrita en México, hace más de 10 años.OBJETIVO: Presentar información actualizada de la prevalencia de hipotiroidismo congénito del Programa de Tamiz Neonatal de la Secretaría de Salud y compararla con la reportada previamente en el país.MATERIALES Y MÉTODOS: Estudio retrospectivo de recién nacidos tamizados en las unidades médicas de la Secretaría de Salud del 1 de enero de 2017 al 31 de mayo de 2018. Se cuantificó la hormona estimulante de la tiroides (TSH) en sangre neonatal. Se revisó la bibliografía de la prevalencia al nacimiento de hipotiroidismo congénito (publicaciones del 2000 a la fecha).RESULTADOS: En 1,267,122 muestras adecuadas para el procesamiento bioquímico se encontraron 3,337 con TSH >10 mU/mL. Se confirmaron 923 casos, de los que 564 (61.1%) fueron niñas. La prevalencia al nacimiento fue de 7.3 por 10,000 recién nacidos. En la revisión bibliográfica solo se encontraron dos estudios con datos nacionales desglosados por entidad federativa, con una prevalencia de 4.1 y 4.3 x 10,000 recién nacidos.CONCLUSIÓN: La prevalencia al nacimiento de hipotiroidismo congénito en los recién nacidos de la Secretaría de Salud de México fue de 7.3 x cada 10,000 recién nacidos tamizados, lo que significa que uno de cada 1,373 tiene esta condición; las niñas son las más afectadas. INTRODUCCIÓN: La deficiencia de glucosa-6-fosfato deshidrogenasa (dG6PD) es la enzimopatía hereditaria más frecuente en el mundo. La mayoría de los casos son asintomáticos y algunos llegan a padecer anemia hemolítica aguda, ictericia neonatal o anemia hemolítica crónica no esferocítica. En países de Asia y el Mediterráneo y con alta incidencia de malaria, se practica el tamiz neonatal para la dG6PD, pero no existe un consenso universal para su implementación. OBJETIVO: Describir los valores de la actividad de G6PD en papel filtro en recién nacidos, analizar la prevalencia y la repercusión en el número de localizaciones a efectuar en el programa de tamiz neonatal nacional.MATERIAL Y MÉTODOS: Estudio retrospectivo de los resultados del programa de tamiz neonatal de la Secretaría de Salud de México. Se analizó la actividad de la glucosa- 6-fosfato deshidrogenasa dependiendo del tiempo de tránsito (tiempo trascurrido desde la obtención de la muestra de sangre del talón del recién nacido hasta su llegada al laboratorio). Se analizó la prevalencia según la actividad enzimática residual.RESULTADOS: De 1,076,918 recién nacidos tamizados, 343,272 tuvieron tiempo de tránsito adecuado (≤ 6 días). La actividad enzimática desciende significativamente después de ese tiempo. La prevalencia nacional al nacimiento fue de 4.26%, con porcentajes máximos en Veracruz, Nuevo León y Tabasco (21,20 y 15%, respectivamente). CONCLUSIÓN: 4.26% de los recién nacidos en México tienen dG6PD; es decir, un caso por cada 23 tamizados, lo que implica un elevado número de sujetos para localizar y evaluar en los programas de tamiz neonatal y en las instituciones que llevan a cabo el seguimiento clínico.PALABRAS CLAVE: Glucosa 6-fosfato deshidrogenasa, deficiencia de glucosa 6-fosfato deshidrogenasa, tamiz neonatal, prevalencia.
INTRODUCCIÓN: La detección temprana de la fibrosis quística mediante tamiz neonatal tiene una repercusión clínica positiva; su implementación es compleja y no existe un protocolo universal para llevarla a cabo. La cuantificación del tripsinógeno inmunorreactivo es el inicio de todos los algoritmos actuales de tamiz neonatal.OBJETIVO: Reportar los resultados preliminares de la aplicación de un algoritmo de tamiz neonatal para fibrosis quística en la Secretaría de Salud de México.MATERIAL Y MÉTODOS: Estudio retrospectivo de los resultados de la aplicación de un algoritmo de tamiz neonatal consistente en la cuantificación del tripsinógeno inmunorreactivo (IRT/IRT) seguida de la prueba del sudor para la detección de fibrosis quística en el marco del programa de tamiz metabólico neonatal de la Secretaría de Salud de México.RESULTADOS: Se obtuvieron 1,267,122 muestras aptas para el procesamiento bioquímico de sangre en papel filtro de 1,273,727 recién nacidos; 3,216 muestras resultaron con valores sanguíneos de tripsinógeno inmunorreactivo superiores al percentil 99.5. Solo en 54% de los casos se logró tomar la segunda muestra de tripsinógeno inmunorreactivo en el tiempo adecuado y se practicaron 1,787 pruebas de sudor. Se identificaron 202 casos altamente sugestivos de fibrosis quística (1:6,273 recién nacidos) que se enviaron para seguimiento médico. La gran cantidad de muestras extemporáneas favoreció el gran número de pruebas del sudor. Estos resultados permiten identificar las principales dificultades que enfrenta la implementación de este tamiz mediante el algoritmo IRT/IRT/ST.CONCLUSIÓN: La incorporación de este primer algoritmo de detección de fibrosis quística al programa de tamiz neonatal en México representa un logro, susceptible de perfección.PALABRAS CLAVE: Tamiz neonatal; fibrosis quística; CFTR; tripsinógeno inmunoreactivo; IRT.
INTRODUCCIÓN: La hiperfenilalaninemia, cuya forma más grave es la fenilcetonuria, es un error innato del metabolismo de la fenilalanina y es el trastorno de aminoácidos más frecuente en el mundo. Cuando no se trata tempranamente causa discapacidad intelectual permanente. En México, la detección oportuna de esta enfermedad se incorporó al programa de tamiz neonatal de la Secretaría de Salud en 2011.OBJETIVO: Reportar la prevalencia al nacimiento de la hiperfenilalaninemia fenilcetonuria en México.MATERIAL Y MÉTODOS: Estudio retrospectivo y prospectivo, efectuado del 1 de enero de 2017 al 31 de mayo de 2018 en muestras del talón de recién nacidos vivos en unidades de la Secretaría de Salud de México. Se cuantificó la fenilalanina en sangre seca mediante ensayo inmunofluorométrico. Se consideró con hiperfenilalaninemia- fenilcetonuria a todo recién nacido con muestras confirmatorias de concentraciones elevadas de fenilalanina en sangre, tirosina baja y cociente fenilalanina-tirosina mayor de 2. Se analizaron los casos sospechosos, casos confirmados y la proporción de casos no localizados. El análisis estadístico se realizó con el programa Comprehensive R Archive Network.RESULTADOS: Resultaron adecuadas para la cuantificación 1,267,122 (99.4%) muestras de recién nacidos vivos, y de éstas 793 resultaron sospechosas y se confirmaron 46 casos de la enfermedad. La hiperfenilalaninemia-fenilcetonuria coexistió en 21 estados del país.CONCLUSIÓN: La tasa de prevalencia al nacimiento de hiperfenilalaninemia fenilcetonuria en México fue de 3.6 casos por cada 100,000 recién nacidos vivos, lo que equivale a un enfermo por cada 27,546 recién nacidos tamizados.
