OBJECTIVE:To analyze the technical conditions for medical product reprocessing in hospitals. METHODS:This was a descriptive study on multiple cases in materials and sterilization centers at four hospitals in the city of Salvador, Southeastern Brazil, in 2010. Semi-structured interviews were conducted and direct observations were made, based on a regulatory model for medical product reprocessing that was drawn up with the technique of consensus verifi cation as the reference standard. The analytical categories used in this study were: management of the practice of medical product reprocessing; reprocessing protocols; monitoring of the sterilization process; and capacity for product tracking. These categories were scored according to their degree of conformity with the regulatory model for reprocessing used. RESULTS:General inadequacy of the technical conditions for medical product reprocessing was observed, with regard to the structural conditions of the materials and sterilization centers studied and the work process conditions, along with organizational and managerial diffi culties that interfered with the practices analyzed. CONCLUSIONS:The practices of medical product reprocessing in the hospitals analyzed constitute a risk for the patient users. Risk management systems need to be introduced in these hospitals, with greater sanitary control by the state, in order to protect patients' health.
Medical device reprocessing is extremely important and complex, thus requiring both the operational skills for its implementation and the technical qualification of the professionals involved in the activity. The typical issues of medical device reprocessing involve technical, economical and regulatory aspects when involving either the so-called reusable articles or those considered as for a single use. The objective of the present study is to propose a new regulatory model for medical device reprocessing in Brazil that would, on the one hand, satisfy the requirements for quality and safety, as recommended in the literature and, on the other hand, prove to be operational under the conditions prevailing in Brazilian hospitals. The elaboration of the present normative proposal was based on the Consensus Conference technique among specialists in the area. Guided by the contribution of these specialists, a proposal is put forth of a regulatory model for reprocessing medical products, so as to address some previously identified gaps in the normative body currently used in Brazil.
Reprocessamento de produtos médicos: da política regulatória à prática operacionalThe reprocessing of medical products: from regulatory polices to operational practices Resumo É crescente o arsenal tecnológico utilizado nas intervenções do cuidado à saúde, ampliado continuamente com a introdução de novos produtos e artigos. Os problemas relacionados à reutilização de produtos médicos, tanto os considerados como reusáveis quanto os de uso único, abrangem aspectos de natureza técnico-operacional, econômica, ambiental, jurídica, política e ética. Este estudo tem o objetivo de contextualizar os sistemas de regulação dos produtos médicos, e analisar as implicações operacionais para a prá-tica hospitalar brasileira. Trata-se de um artigo de revisão bibliográfica, realizada sem restrição de tempo e idioma, utilizando as bases de dados da Web of Science, Bireme, Scielo e Lilacs, com descritores específicos. A contextualização dos esquemas regulatórios de produtos médicos no mundo e no Brasil e as regras básicas das normatizações existentes sobre o reprocessamento desses produtos são as fontes de subsídio que este estudo utiliza para analisar as implicações operacionais sobre essas práticas em hospitais brasileiros. Problems associated with the reutilization of medical products, both reusable and of single use, affect policies and related technical-operational, economic, political, ethical, legal, and environmental matters. This study aims to contextualize the regulatory systems of medical products, and analyze the subsequent operational implications for Brazilian hospital practices. The article consists of a bibliographic review, carried out without time and language restriction, utilizing the Web of Science, Bireme, Scielo and Lilacs databases, with the support of specific descriptors. This study uses the contextualization of regulatory plans for medical products across the world and in Brazil and the existing condition of standardization of the reprocessing of these products as the assessment sources with which to analyze the operational implications for these practices in Brazilian hospitals.
Introdução: O reuso de produtos de uso único é uma realidade mundial e implica em grandes problemas. Objetivo: Analisar o sistema regulatóriode reprocessamento de produtos médicos de uso único a nível internacional, incluindo o brasileiro. Método: Revisão narrativa da literatura, utilizandobases de dados com descritores específicos. Resultado: Internacionalmente, as políticas de reuso de produtos médicos de uso único tendem a prevenção dedanos. As regulamentações variam desde protocolos bem estruturados, como o norte‑americano, o australiano e o alemão, à ausência de normatização anível nacional, como identificado em países desenvolvidos como Canadá, Japão e alguns países da União Europeia. Conclusão: Os controles regulatóriosexistentes apresentam lacunas que dificultam sua implementação tanto para os serviços de saúde quanto para os fabricantes. Uma metodologia alternativaseria a de um sistema regulatório de produtos de uso único centrado no controle dos processos em lugar dos atuais focados no controle do produto.Palavras‑chave: Segurança do paciente. RESUMO Objetivo: Descrever o perfil epidemiológico da hepatite B e identificar a área geográfica de residência dos indivíduos acometidos pela doença, no município de Salvador, Bahia, no período de 2007 a 2012. Material e Métodos: Estudo epidemiológico de caráter descritivo, a partir de dados secundários do Sistema de Informação de Agravos e Notificação (SINAN) que inclui todos os casos notificados e confirmados no município de Salvador, Bahia. Resultados: Foram notificados e confirmados 9.100 casos de Hepatites Virais no estado da Bahia. Ao longo dos anos de estudo, Salvador contribuiu com 535 casos, correspondendo a 20,26% das notificações no estado. A análise ratificou alguns aspectos reportados na literatura, a exemplo de que os maiores acometidos pela infecção do vírus da Hepatite B (VHB) são homens na faixa etária compreendida entre 20 a 49 anos. A respeito da via de transmissão sexual apresentou maior frequência, sendo que nos anos de 2009 a 2011, ambos apresentaram aproximadamente 40% de casos registrados. Sobre a localização geográfica de residência o distrito sanitário que apresentou o maior número de casos foi o Cabula Beiru com 65 casos confirmados, seguido do Pituba Barra Rio vermelho e Subúrbio Ferroviário, divisão territorial demarcada por distorções no padrão econômico. Conclusões: Embora exista a disponibilidade da vacinação contra hepatite B, os casos têm aumentado significativamente no município, evidenciando a necessidade de políticas públicas que desencadeia ações de controle e subsidie o planejamento das estratégias de promoção e prevenção do agravo.
