Published evidence suggests that obesity impairs cognition. Development of chronic low-grade inflammation (CLGI) represents the earliest consequence of obesity. The present study investigated the association between obesity and fluid intelligence impairment and assessed the potential mediating role of CLGI and psychological (depression/anxiety symptoms), lifestyle (exercise) and physiological (metabolic dysfunction indices) factors in this association. Clinically healthy participants (n 188), grouped as per BMI, underwent cognitive (General Ability Measure for Adults), psychological (Beck Depression Inventory-II and State-Trait Anxiety Inventory) and activity (Godin leisure-time physical activity) measurements. Biochemical parameters included the following: (a) indices of CLGI (high-sensitivity C-reactive protein, erythrocyte sedimentation rate and fibrinogen); (b) insulin resistance (Homeostasis Model Assessment of Insulin Resistance index); (c) adiposity (plasma adiponectin). An inverse association between elevated BMI and fluid intelligence was observed, with obese participants displaying significantly poorer performance compared with age-matched normal-weight peers. Structural equation modelling results were consistent with a negative impact of obesity on cognition that was mediated by CLGI. The results of the present study support the hypothesis that reduced general cognitive ability is associated with obesity, an adverse effect mainly mediated by obesity-associated activation of innate immunity.
Background
Although diffusely abnormal white matter (DAWM) is commonly seen in multiple sclerosis (MS), it is rarely considered in clinical/imaging studies.
Purpose
To evaluate quantitative markers of microstructural changes in DAWM of patients with clinically isolated syndrome (CIS) and relapsing–remitting MS (RR‐MS) in relation to MS lesions and degree of neurocognitive impairment, by using a multi‐echo spin echo (MESE) Proton Density PD‐to‐T2 sequence.

Thirty‐seven RR‐MS patients, 33 CIS patients, and 52 healthy controls.

Assessment
Long T2, short T2, and myelin water fraction (MWF) values were estimated as indices of intra/extracellular water content and myelin content, respectively, in DAWM, posterior periventricular normal appearing white matter (NAWM), and focal MS lesions, classified according to their signal intensity on T1 sequences. Patients were, also, administered a battery of neuropsychological tests.
Statistical Tests
Comparisons of T2 and MWF values in DAWM, NAWM, and MS lesions were examined, using two‐way mixed analyses of variance. Associations of Grooved Pegboard performance with T2 and MWF values in DAWM and NAWM were assessed using Pearson correlation coefficients.
Results
T2 and MWF values of DAWM were intermediate between the respective values of NAWM and T1 hypointense focal lesions, while there was no difference between the respective values of DAWM and T1‐isointense lesions. T2 values in DAWM were strongly associated with visuomotor performance in CIS patients.
Data Conclusion
Intra/extracellular water and myelin water content of DAWM are similar to those of T1‐isointense lesions and predict visuomotor performance in CIS patients.

Η παχυσαρκία οδηγεί σε χρόνιες μεταβολικές διαταραχές, οι οποίες με τη σειρά τους έχουν συνδεθεί με γνωστικά ελλείμματα. Η ανάπτυξη της χρόνιας χαμηλού βαθμού φλεγμονής (chronic low grade inflammation-CLGI) αποτελεί την πρώτη φυσιολογική συνέπεια της παχυσαρκίας. Στην παρούσα διδακτορική διατριβή διερευνήθηκε η σχέση μεταξύ παχυσαρκίας και γνωστικών ελλειμμάτων (ρέουσας νοημοσύνης) και αξιολογήθηκε ο διαμεσολαβητικός ρόλος της CLGI. Επίσης εξετάστηκε ο ρόλος ψυχολογικών, τρόπου ζωής αλλά και φυσιολογικών (δείκτες μεταβολικής δυσλειτουργίας) παραγόντων. Νέοι και μέσης ηλικίας, κλινικά υγιείς συμμετέχοντες εντάχθηκαν σε τρείς ομάδες (νορμοβαρείς, υπέρβαροι, παχύσαρκοι) ανάλογα με τον Δείκτη Μάζας Σώματος τους και πραγματοποιήθηκαν γνωστικές μετρήσεις (τεστ Γενικής Νοητικής Ικανότητας Ενηλίκων), ψυχολογικές (κατάθλιψη/άγχος) και μετρήσεις φυσικής δραστηριότητας (ερωτηματολόγιο σωματικής δραστηριότητας Godin). Οι βιοχημικοί δείκτες που εξετάστηκαν περιελάμβαναν α) δείκτες CLGI (υψηλής ευαισθησίας C-αντιδρώσα πρωτεΐνη, ινωδογόνο, ταχύτητα καθίζησης ερυθρών αιμοσφαιρίων) β) αντίσταση στην ινσουλίνη (μοντέλο ομοιόστασης HOMA-IR) και γ) δείκτη παχυσαρκίας (ολική αδιπονεκτίνη ορού).Οι αναλύσεις σε σχέση με τη γνωστική ικανότητα έδειξαν ότι οι παχύσαρκοι συμμετέχοντες παρουσίασαν σημαντικά χαμηλότερες επιδόσεις στο τεστ ρέουσας νοημοσύνης συγκριτικά με αντίστοιχους ηλικιακά νορμοβαρείς και υπέρβαρους, ακόμα και ελέγχοντας ως προς την κατάθλιψη, το άγχος, το επίπεδο σωματικής δραστηριότητας και τους μεταβολικούς δείκτες (ινσουλίνη, HOMA-IR, αδιπονεκτίνη). Επιπλέον, με βάση τα αποτελέσματα μοντέλων δομικών εξισώσεων φάνηκε ότι οι αρνητικές επιπτώσεις της παχυσαρκίας στην γνωστική ικανότητα διαμεσολαβείται από την CLGI. Επομένως, τα αποτελέσματα της παρούσας μελέτης υποστηρίζουν την υπόθεση ότι η μειωμένη γενική γνωστική ικανότητα που σχετίζεται με την παχυσαρκία, είναι μια δυσμενής συνέπεια που σχετίζεται με την ενεργοποίηση επαγόμενων από την παρουσία παχυσαρκίας ενδογενών ανοσολογικών διεργασιών, οι οποίες με τη σειρά τους ενδεχομένως να έχουν άμεσες βλαπτικές συνέπειες στον κεντρικό νευρικό σύστημα. Πλέον, γίνεται ολοένα πιο αποδεκτό ότι οι αρνητικές επιδράσεις της παχυσαρκίας στη γνωστική ικανότητα ανακύπτουν πολύ νωρίτερα από ότι πιστεύαμε στο παρελθόν και δεν διαμεσολαβούνται αποκλειστικά και μόνο από τις τυπικές κλινικές συνέπειες της, όπως η υπέρταση, ο διαβήτης και η αρτηριοσκλήρυνση. Η συχνότητα εμφάνισης της παχυσαρκίας στην Ελλάδα αυξάνεται με ανησυχητικό ρυθμό, τόσο σε ενήλικες όσο και σε παιδιά. Το γεγονός αυτό εγείρει σοβαρούς προβληματισμούς όχι μόνο για την ανάπτυξη των τυπικών ιατρικών προβλημάτων που σχετίζονται με την παχυσαρκία, αλλά και την εμφάνιση γνωστικών δυσλειτουργιών, με πολύ πιθανή την μελλοντική σοβαρή αύξηση των ποσοστών επιπολασμού άνοιας πρώιμης έναρξης στον ελληνικό πληθυσμό.
Purpose
This study aims to identify common and distinct hemodynamic and functional connectivity (FC) features for self-rated fatigue and depression symptoms in patients with clinically isolated syndrome (CIS) and relapsing–remitting multiple sclerosis (RR-MS).
Methods
Twenty-four CIS, 29 RR-MS patients, and 39 healthy volunteers were examined using resting-state fMRI (rs-fMRI) to obtain whole-brain maps of (i) hemodynamic response patterns (through time shift analysis), (ii) FC (via intrinsic connectivity contrast maps), and (iii) coupling between hemodynamic response patterns and FC. Each regional map was correlated with fatigue scores, controlling for depression, and with depression scores, controlling for fatigue.
Results
In CIS patients, the severity of fatigue was associated with accelerated hemodynamic response in the insula, hyperconnectivity of the superior frontal gyrus, and evidence of reduced hemodynamics–FC coupling in the left amygdala. In contrast, depression severity was associated with accelerated hemodynamic response in the right limbic temporal pole, hypoconnectivity of the anterior cingulate gyrus, and increased hemodynamics–FC coupling in the left amygdala. In RR-MS patients, fatigue was associated with accelerated hemodynamic response in the insula and medial superior frontal cortex, increased functional role of the left amygdala, and hypoconnectivity of the dorsal orbitofrontal cortex, while depression symptom severity was linked to delayed hemodynamic response in the medial superior frontal gyrus; hypoconnectivity of the insula, ventromedial thalamus, dorsolateral prefrontal cortex, and posterior cingulate; and decreased hemodynamics–FC coupling of the medial orbitofrontal cortex.
Conclusion
There are distinct FC and hemodynamic responses, as well as different magnitude and topography of hemodynamic connectivity coupling, associated with fatigue and depression in early and later stages of MS.
