O presente trabalho teve como objetivo comparar ex vivo, em 12 pares de ossos, a ação das forças de compressão axial e de flexão em dois métodos de fixação de fratura diafisária transversal em fêmur de cães com peso entre 5 e 10kg. Um com a utilização de dois pinos intramedulares de Steinmann de 2,0mm de diâmetro e outro com a utilização de um método de transfixação cortical interna com pinos de Steinmann de 2,0mm de diâmetro, de forma que os mesmos formem uma tala ao redor do osso e fixando-os com fios de aço inoxidável de 0,6mm de diâmetro em cerclagem. Os ossos fraturados e estabilizados após redução dos fragmentos, foram submetidos às forças em uma prensa Soloteste®, sendo que os do grupo G2 (transfixação esquelética interna) apresentaram superioridade estatisticamente significativa, quanto à rigidez do dispositivo de fixação, em relação ao grupo G1 (pino intramedular).

Factors contributing to the delay or prevention of the natural wound healing process include infection and ineffective conventional treatment. Alternative therapies, such as the maggot debridement therapy (MDT), may be helpful for successful treatment in these cases. Aiming to disseminate information about the possibility of using other species of flies for wound treatment, besides the best known Lucilia sericata Meigen, 1826 (Diptera, Calliphoridae), we report here a successful MDT case with the application of Cochliomyia macellaria (Fabricius, 1775) larvae to treat an infected wound with extensive area of necrotic tissue in a dog. Five sterile larvae were applied to each square-cm of lesion and kept on the animal for only 48 h. The healing was successful, from both qualitative and quantitative points of view. The ratio of wound healing (RWH) reached almost 50% on the 5th day and 100% on the 14th day after MDT. Although the overall animal prognosis had been unfavorable, mainly due to the sepsis, the patient began to recover and had improved clinical condition from the fifth day after MDT. This study shows the importance and effectiveness of MDT in promoting faster and more complete healing of a complex wound.

RESUMOObjetivo: Descrever uma técnica de pericardiotomia de emergência, denominada Tração Ligamentar (TL), para diminuir o tempo necessário ao início da Massagem Cardíaca Interna (MCI). Para a MCI necessita-se de toracotomia de emergência e pericardiotomia, ambas em tempo mínimo. A técnica comumente empregada corresponde a pericardiotomia em "T", cuja execução depende da apreensão do pericárdio com uma pinça de Allis. Este pinçamento é difícil, dificultando a reanimação do paciente. Métodos: Utilizou-se 20 cadáveres de cães, divididos em dois grupos de animais, sendo o Grupo Ipericardiotomia em "T" (n=10) e Grupo II -técnica proposta (n=10). A técnica de TL consistiu na tração do ligamento frenicopericárdico e da secção do pericárdio próximo ao seu ápice. A incisão foi alongada pelos dedos enquanto eram nela introduzidos e permitiu, também, o correto posicionamento do coração na mão do operador, bem como o pronto início da MCI. Resultados: O Grupo I apresentou tempo de execução de 21,79 ± 0,88 segundo, e o Grupo II de 8,58 ± 1,38 segundo, sendo p<0,0001, (altamente significativo). Conclusão: A técnica de pericardiotomia por TL impede um tempo maior de isquemia cerebral, por iniciar prematuramente a circulação sangüínea, contribuindo para a sobrevida. Pericárdio.ABSTRACT Purpose: Describe a technique of emergency pericardiotomy, named as Ligament Traction (LT), to reduce the necessary time to begin the Internal Cardiac Massage. To perform the ICM an emergency toracotomy and pericardiotomy are necessary, both in remote time. The technique usually employed is the "T" pericardiotomy, whose execution depends on the apprehension of the pericardium with an Allis forceps. This apprehension is difficult and complicates the reanimation of the patient. Methods: Twenty canine corpses were divided into two groups: Group I -"T" pericardiotomy (n=10), and Group II -the LT technique (n=10). The LT consisted on the traction of the pericardiumphrenic ligament and the section of the pericardium next to its apex. The incision was elongated with the introduction of the fingers, also allowing the positioning of the heart in the hand of the operator and the immediate beginning of the ICM. Results: Group I presented an execution period of 21.79 ± 0.88 second, and Group II of 8.58 ± 1.38, with p<0.0001 (highly expressive). Conclusion: The technique of pericardiotomy by Ligament Traction concur to outliving, because it avoids a larger time of cerebral ischemia, due to the early beginning of the circulation. Objectives: Reducing the amount of harvested donor skin (HDS; autograft) needed for definitive wound closure can improve outcomes in patients with severe burns.Consensus for what constitutes a clinically significant reduction in the amount of HDS is not available. Our objective was to convene a Delphi Consensus Panel (DCP) to determine the threshold of clinical significance for reduction in the amount of HDS to achieve definitive closure in deep-partial-thickness (DPT) and full-thickness (FT) burns. Methods: A two-round DCP (defined a priori as . 70% panelist agreement) was conducted among 15 burn physicians with at least three years of post-residency experience and total career experience of caring for $ 500 patients. Round 1 consisted primarily of open-ended questions about clinical decision making and treatment practices for DPT and FT. Round 2 required evaluation of treatment scenarios to gain consensus regarding thresholds for clinical significance for the reduction in amount of HDS for treatment of DPT and FT burns. Results: Fourteen (93.3%) panelists completed both rounds. In Round 1, most panelists reported that reduction of HDS was important for both DPT (n=10; 66.7%) and FT (n=9; 60.0%) burns with the percent total body surface area (%TBSA) being the most important factor in clinical decisions. The most important outcomes after definitive closure were scar quality/ cosmesis and functioning. In Round 2, consensus was achieved in a meaningful reduction in the amount of HDS in improving patient outcomes (n=14; 100%). Consensus was also achieved for a threshold for clinical significance for reduction in HDS: 50% reduction for DPT [20 %TBSA] and FT [10 %TBSA] burns (n=13; 92.9% and n=12; 85.7%, respectively) and 35% reduction for mixed DPT and FT burns [45 %TBSA] (n=14; 100%). Conclusions: Thresholds for clinical significance of HDS reduction in DPT and FT burns were established via the DCP.

Realizou-se, em sete cães adultos, o autotransplante renal esquerdo associado à nefrectomia contralateral para avaliação da técnica de ureteroneocistostomia extravesical modificada pela sondagem ureterovesical peroperatória. Durante a sutura do ureter na bexiga, foi mantida uma sonda uretral na região da anastomose ureterovesical, o que facilitou a realização da técnica cirúrgica e permitiu a confecção de anastomose de diâmetro adequado. A avaliação do rim transplantado e do ureter correspondente foi feita mediante ultra-sonografia a cada sete dias durante as seis primeiras semanas de pós-operatório, período em que também se fez o exame clínico diário de todos os animais. No último dia (42º) da avaliação a curto prazo, realizou-se a urografia excretora. Em seguida, manteve-se acompanhamento clínico periódico dos sete cães por um período mínimo de quatro meses, sem que fosse detectada qualquer alteração digna de nota. A técnica de reconstrução do trato urinário utilizada mostrou-se eficiente, não se observando nenhuma complicação urológica no pós-operatório de todos os animais.
