Chikungunya fever has become an important public health problem in countries where epidemics occur because half of the cases progress to chronic, persistent and debilitating arthritis. Literature data on specific therapies at the various phases of arthropathy caused by chikungunya virus (CHIKV) infection are limited, lacking quality randomized trials assessing the efficacies of different therapies. There are a few studies on the treatment of musculoskeletal manifestations of chikungunya fever, but these studies have important methodological limitations. The data currently available preclude conclusions favorable or contrary to specific therapies, or an adequate comparison between the different drugs used. The objective of this study was to develop recommendations for the treatment of chikungunya fever in Brazil. A literature review was performed via evidence-based selection of articles in the databases Medline, SciELO, PubMed and Embase and conference proceedings abstracts, in addition to expert opinions to support decision-making in defining recommendations. The Delphi method was used to define the degrees of agreement in 2 face-to-face meetings and several online voting rounds. This study is part 2 of the Recommendations of the Brazilian Society of Rheumatology (Sociedade Brasileira de Reumatologia - SBR) for the Diagnosis and Treatment of chikungunya fever and specifically addresses treatment.
Chikungunya fever has become a relevant public health problem in countries where epidemics occur. Until 2013, only imported cases occurred in the Americas, but in October of that year, the first cases were reported in Saint Marin island in the Caribbean. The first autochthonous cases were confirmed in Brazil in September 2014; until epidemiological week 37 of 2016, 236,287 probable cases of infection with Chikungunya virus had been registered, 116,523 of which had serological confirmation. Environmental changes caused by humans, disorderly urban growth and an ever-increasing number of international travelers were described as the factors responsible for the emergence of large-scale epidemics. Clinically characterized by fever and joint pain in the acute stage, approximately half of patients progress to the chronic stage (beyond 3 months), which is accompanied by persistent and disabling pain. The aim of the present study was to formulate recommendations for the diagnosis and treatment of Chikungunya fever in Brazil. A literature review was performed in the MEDLINE, SciELO and PubMed databases to ground the decisions for recommendations. The degree of concordance among experts was established through the Delphi method, involving 2 in-person meetings and several online voting rounds. In total, 25 recommendations were formulated and divided into 3 thematic groups: (1) clinical, laboratory and imaging diagnosis; (2) special situations; and (3) treatment. The first 2 themes are presented in part 1, and treatment is presented in part 2.
Resumo:No contexto das mutações por que passam o Estado, a sociedade brasileira e as grandes cidades na segunda década do século XXI, e tendo em mente as particularidades da cidade São Paulo. Identifica-se um descompasso entre as políticas de desenvolvimento urbano e os problemas sociais enfrentados por multidões que exigem o direito à moradia, à mobilidade urbana, à liberdade de locomoção. Vislumbra-se por meio da pesquisa a cidade numa concepção social e jurídica. Especialmente no ambiente urbano encontramos conflitos de interesses econômicos, sociais, culturais, ambientais que transcendem a mera noção de espaço individual marcada pelo modelo de democracia representativa construída no distanciamento do povo. Embora desde 1988 haja previsão, na Constituição Federal e no Estatuto da Cidade, quanto às políticas de desenvolvimento urbano. A insegurança urbana e a especulação imobiliária têm sido a regra e, então, ao contrário, tem sido negada a concepção de acesso aos espaços públicos a todos. Contrariamente privilegia-se as grandes avenidas, e o afastamento dos pobres do centro. Essas contradições exigem um estudo metodológico histórico e procedimento dialético com vistas à compressão da problemática urbana atual e seus desafios e passa pela democracia deliberativa-participativa com vistas à função social da cidade.

Abstract:In the context of changes by passing the State, Brazilian society and the major cities in the second decade of this century, and bearing in mind the particularities of the city São Paulo. Identified a mismatch between urban development policies and social problems faced by crowds who demand the right to housing, urban mobility, freedom of movement. O presente estudo analisa aspectos da Lei 12.990/2014, que reserva para negros e pardos vinte por cento das vagas oferecidas em concursos p�blicos da Administra��o P�blica Federal direita e indireta. Para se chegar a algumas conclus�es, necess�rio abordar o tema das a��es afirmativas, com algumas defini��es e apresenta��o de suas origens, al�m de exemplos de medidas dessa natureza que est�o sendo adotadas oficialmente nos �ltimos tempos, para compensar o passivo hist�rico que o Brasil possui em rela��o � popula��o negra. Mas deve-se ter em mente que para aprova��o em concursos p�blicos, � necess�ria obten��o de notas m�nimas nas provas de conhecimento. Da�, a plena efetividade dessa regra depende tamb�m da qualidade da educa��o que a popula��o recebe. Essa an�lise, ainda, n�o pode ser separada da quest�o da situa��o social e econ�mica das pessoas a quem essa lei diz beneficiar, condi��es essas que podem impedir que elas cumpram os demais requisitos exigidos em provas de conhecimento. Afinal, as car�ncias econ�micas e educacionais dos brasileiros ainda s�o latentes. Outro fator prejudicial s�o as distor��es de natureza regional, diante da distribui��o n�o homog�nea da popula��o negra entre os Estados das cinco regi�es brasileiras. Necess�rio tamb�m se apresentar uma an�lise sobre a quest�o da igualdade racial, nos termos tratados em nossa Constitui��o. Todas essas reflex�es podem nos auxiliar a avaliar se h� possibilidade do pleno alcance dos fins almejados pela referida norma legal ou se esta lei se apresenta mais, como uma medida demag�gica com pouco efeito pr�tico na vida das pessoas a quem visa beneficiar.
